Microcirculatory effects of somatostatin in acute pancreatitis.

Somatostatin, a 28-amino-acid inhibitory polypeptide has been advocated for the treatment of upper gastrointestinal bleeding and acute pancreatitis. This study examines the effect of somatostatin in acute hemorrhagic pancreatitis in piglets (n = 12), weighing 8-12 kg. Six animals served as controls, and received only fluid resuscitation (0.9%, NaCl, 20 ml/kg/h). Six animals received somatostatin treatment consisting of a 15 micrograms/kg bolus i.v. given simultaneously with the induction of pancreatitis, and treatment continued with an intravenous infusion (15 micrograms/kg/h) for 5 h. Cardiac output, heart rate, blood pressure, arterial pO2, hematocrit and serum amylase were recorded before and each hour during the experiment. Regional blood flow in the gastrointestinal area was measured using the microsphere method. The microspheres labelled with three different isotopes were administered before the experiment and at 2 and 5 h, respectively. There was a significant decrease in the cardiac output (p less than 0.05) and an increase in systemic blood pressure in the somatostatin-treated group (p less than 0.025). Pancreatic blood flow decreased by 43% following somatostatin infusion. The decreases at 2 and 5 h were highly significant (p less than 0.005). Blood flow to the mucosal but not muscular region of the stomach was decreased by somatostatin. This study suggests that somatostatin might be harmful in acute pancreatitis due to its adverse effects on pancreatic blood flow and cardiac output. However, somatostatin may be effective in reducing gastrointestinal bleeding. If the drug is used clinically, careful monitoring of the cardiac output is necessary.

[Action-research on the prevention of abuse in the very young child. Methodology and initial results].

Teams of pediatricians and psychiatrists are leading a joint action-research in public health which intends to develop and evaluate original modes of prevention of abuse and grave neglect concerning infants with medico-psychosocial high risk factors. The framework of this study is a randomized controlled trial on the modes of prevention. The sample, consisting of infants under one year of age and selected from their health certificate of the 8th day and 9th month, is divided into two groups: the treated group who benefited from specific preventive interventions and the control group who benefited from classic surveillance. The research was longitudinal and was centered on the study of mother-infant interactions at 3 months, 1 year, 2 years and 3 years of age. The dynamics in the action-research is illustrated by the evolution of the tool for assessment of early interactions and the refocusing of the research. From the analysis of the data from the phase of feasibleness, which included 114 infants, three main results have come out: definition of a "danger" pointer: dysharmonious mother-infant interactions at 3 months; a new risk factor was brought to light: "disrupted grand-parental family"; father's role in mother-infant interactions. These first results have had a major impact on the method for the active phase which is now in progress and intends to evaluate the new modes of prevention.

Emergency bypass system: analysis of gas transfer.

A portable emergency bypass system using a membrane oxygenator device (CPS) was evaluated. The ability of the CPS to supply the oxygen transfer needs of five anesthetized dogs consistently over six hours and the system's effects on hemoglobin concentration, platelet count, and degree of hemolysis were assessed. The animals maintained spontaneous heart beats, pump flows averaged 100 ml/kg/min; mean arterial pressures were maintained at from 114 to 144 mm Hg. Immediate dilution of hemoglobin and platelet levels occurred in the first 30 minutes. Further hemodilution was limited during the first two hours, although three of the animals required transfusions during the six-hour period to maintain their hematocrits. Plasma free hemoglobin did not significantly increase during the six hours. Baseline oxygen consumption data obtained in three animals ranged from 44.5 to 96 (ml oxygen/min/m2). Oxygen and carbon dioxide transfer measurements during the first hour of bypass were not significantly different from measurements during the last hour of perfusion. The study suggests that optimal use of the CPS could supply much if not all of a patient's basal oxygen transfer requirements for at least six hours.

Regional cardiac tamponade: a hemodynamic study.

In 10 dogs, atrial tamponade, ventricular tamponade and then combined atrioventricular (AV) tamponade were produced at 10, 15 and 20 mm Hg intrapericardial pressure. Cardiac output decreased significantly at each level of cardiac tamponade; the changes in cardiac output and mean aortic pressure were comparable with atrial and ventricular tamponade. Combined atrial and ventricular tamponade produced significantly greater increases of right and left atrial pressure and significantly greater decreases of cardiac output than did either atrial or ventricular tamponade. During atrial tamponade only, a significant pressure gradient developed between the venae cavae and the right atrium. Compression of both ventricles by tamponade has a much greater hemodynamic effect than does compression of either ventricle alone. Compression of the entire heart has a greater hemodynamic effect than does compression of the atria alone or the ventricles alone. Compression of the great veins has a potential effect in tamponade, demonstrable when the ventricles could fill normally.

Hemodynamics of transcutaneous cardiac pacing.

Transcutaneous cardiac pacing has recently been rediscovered as a rapid means of initiating emergency cardiac pacing. Potential myocardial injury from extended transcutaneous pacing could adversely affect cardiac hemodynamics during pacing. This canine study compares the hemodynamics of transcutaneous and transvenous cardiac pacing in animals with induced chronic heart block. One to two weeks following chemical ablation of the His bundle, hemodynamic measurements were made during 60 minutes of transcutaneous and 5-minute periods of conventional right ventricular endocardial pacing. Cardiac index and output were found to increase significantly (P less than 0.005), and systemic vascular resistance was found to decrease significantly (P less than 0.005) from baseline values with both pacing techniques. A hemodynamic difference between pacing techniques was evident only for mean arterial blood pressure; pressure measurements during transvenous pacing were slightly greater than those during transcutaneous cardiac pacing. The hemodynamic measurements were found to be stable during a 60-minute period of transcutaneous cardiac pacing. This study demonstrates that transcutaneous cardiac pacing is as effective hemodynamically as conventional transvenous pacing in animals with induced chronic heart block.

[Protein and amino acid metabolism in the digestive tract of growing bulls. 6. Apparent digestibility of the amino acids entering the duodenum and of amino acid from feed].

After studies with young bulls provided with duodenal cannulas the apparent digestibility (AD) of the individual amino acids getting into the duodenum (AAD) was ascertained on the basis of 28 different rations. The AD of the AAD is defined as follows: AAD--AAfaeces/AAD X 100. The results thus obtained show a relative constancy, only insignificantly impaired by the consideration of the dependence of the AD of the AAD on the AA concentration in the ration. The average value for the AD of the sigma AAD was 72.5 +/- 4.9%. The dependence of the AD of the sigma AAD on the sigma AA concentration in the ration is: y = 67.8 + 0.51x +/- 4.6; r = 0.355x. In addition, the relation between AD AAD and AD AAfeed protein' each in % of AA intake and AA concentration in the ration, was studied and reported on.

[Protein and amino acid metabolism in the intestinal tract of growing young bulls. 7. Protein estimation and evaluation of protein supply in growing ruminants by the apparently digestible total protein in the duodenum based on rations].

The following values were derived from experimental studies of the crude protein (CP) and amino acid (AA) metabolism of growing fattening bulls, the detailed results of which were described in installments 1-6, for the ascertainment of the content of apparently digestible total protein at the duodenum (ADTPD): a) partial method: Degradation quotaCP = NPN + 0.69 X g pure protein/CP; g AD feed PD = g CP (1 -decomposition quotaCP) X 0.72 or g AD feed PD = 0.31 X g pure protein X 0.72; g ADBPDE = 0.149 X g AD org. matter X 0.80 X 0.72 (ADBPDE = apparently digestible pure bacteria protein at the duodenum, dependent on energy release in the reticulo-rumen; g ADBPDN = g CP X (degradation quotaCP) X 0.80 X 0.72 (ADBPDN = apparently digestible pure bacteria protein at the duodenum, dependent on CP supply of rumen microbes); g AD total PD = g AD feed PD + ADBPDE g AD total PD = g AD feed PD + ADBPDN; b) summarizing method: AD total PD = 0.429 + 10.9 X 0.80/CP in % AD org. matter X (CP intake) X 0.72; AD total PD = 0.196 + 16.6 X 0.80/pure protein % AD org. m. The requirement of AD total PD was calculated under consideration of the endogenous urine N, skin N and hair N losses as well as protein retention per kg live weight assuming a utilization of 0.70 for this partial performance. The practical application of this balance method is demonstrated in comparison to the digestible CP-system.

[Protein and amino acid metabolism in the digestive tract of growing young bulls. 4. Apparent digestibility of NH3-free raw protein reaching the duodenum and critical evaluation of apparently digestible raw protein].

The experimental determination of the apparent digestibility (AD) of the NH3-free crude protein at the duodenum (NH3 fr. CPD) showed a value of 65.7% at a standard error of +/- 3.6% for the section of the digestive tract between proximal duodenum and end of rectum. This standard error can only be insignificantly diminished due to the existing relation between AD of NH3 fr. CPD in dependence on the CP concentration in the ration (y = 56.4 + 0.598 x +/- 3.17; r = 0.501xx) so that this relation has no practical significance. The criticism of AD CP is correct from a point of view of digestion physiology. There is only agreement between AD PC and AD NH3 fr. CPD in a CP concentration range between 12.5 and 14.0% CP in the DM, which, however, applies to a large range of the production sphere in the rearing of young bulls. It excludes, however, the low live weight range with high live weight gains.

[Protein and amino acid metabolism in the digestive tract of growing young bulls. 2. Feed protein flow into the duodenum determined with 2,6-diaminopimelic acid as a marker for crude bacterial protein].

The experimental ascertainment of the pure feed protein flowing into the duodenum on the basis of the calculation of the difference between the NH3-free crude protein flowing into the duodenum and the bacterial crude protein determined by means of DAPA showed the following results after the testing of 28 different rations at a dry matter intake adequate to the production level: With a variation of the pure protein in the crude protein content of the ration between 40 and 90%, the quota of feed protein flowing into the duodenum is 52.5%, and with a variation of the pure protein in the crude protein content between 80 and 90% it is 40.6%. The quota of feed protein flowing into the duodenum shows a negative correlation to the apparent digestibility of the organic matter (y=231.7 -2.52x +/- 14.5). With a DM-intake adequate to the production level the quota of feed protein flowing into the duodenum is neither influenced by the flow rate (kg digesta/kg DM-intake) nor by the 'dilution rate' (g bacteria-free DMD/kg live weight 0.75/h).

[Protein and amino acid metabolism in the digestive tract growing young bulls. 1. Flow of bacterial raw protein in the duodenum determined by 2,6-diaminopimelic acid as a marker].

The influence of the composition of the rations on the content of diamino pimelic acid (DAP) and N in the bacteria dry matter and on the flow of bacteria crude protein into the duodenum, determined with DAP as marker, was determined on the basis of experimental investigations with young bulls provided with duodenal cannulae and with 28 different rations whose details are described. At a production level corresponding to dry matter intake and a variation range of the crude fibre content of between 11.3 and 29.1% in the DM, a content of N-free extractives between 50.9 and 77.4% in the DM, a content of soluble carbohydrates ibetween 5.3 and 6.4% in the DM, a crude protein content of between 6.4 and 17.1% in the DM and a pure protein content of between 4.9 and 15.5% in the DM, the DAP content of the bacteria DM amounts the 0.350 g 100 g DM +/- 0.090 the N-content of the bacteria DM amounts to 7.37 +/- 1.08 g/100 g DM, there are no relations between DAP- and N-content in the bacteria DM and the content of the individual carbohydrate fractions of the ration, there are positive relations between DAP- and N-content of the bacteria DM, the flow of bacterial crude protein into the duodenum amounts to 133 +/- 14 g/kg truly fermentable organic matter or 130 +/- 14 g/kg apparently digestible organic matter, there is a negative relation between bacteria crude protein at the duodenum (BCPD)/kg truly fermentable organic matter and the crude fibre content of the ration, there is a positive relation between BCPD/kg truly fermentable organic matter and N-free extractives and soluble carbohydrate content as well as the digestibility of the organic matter of the ration, there is no specific influence of the flow rate (kg digesta/kg intake of org. matter) or the dilution rate (g bacteriafree org. matter D/kg LW075/b) on BCPD/kg truly fermentable org. matter, there is a dependence of BCPD/kg truly fermentable org. matter on crude protein concentration in the ration in the concentration range of 6.4-9.0% crude protein in the ration (provided endogenous CP equivalents are used).

[Protein and amino acid metabolism in the gastrointestinal tract of young bulls. 3. Flow of NH3-free raw protein into the duodenum].

The amount of NH3-free crude protein getting into the duodenum corrected by the endogenous crude protein quota was determined on the basis of 28 differently composed rations for growing bulls in the live weight range of 140-460 kg provided with duodenal re-entrant cannulae. The experimental results were generalized by means of regression analysis and can be summarized as follows: The amount of NH3-free crude protein getting into the duodenum can partially be calculated with the regression equation (Formula: see text) resp. NH3-free crude protein D = 155 app. dig. org. matter + 0.262 pure protein +/- 42. The remaining dispersion amounts to +/- 6% of the mean value. The coincidence between the values calculated with these equations and those measured experimentally is very good. There is an interaction between the pure protein of feed getting into the duodenum in % of pure protein intake (y) and the bacteria crude protein D/kg app. dig. org. matter (x) characterized by the equation y = 116.8-0.52 x +/- 12.9. The amount of NH3-free crude protein duod. is not influenced by the flow rate (kg digesta/kg intake of org. matter) and not by the "dilution rate' (g bacteria free org. matter duod./kg live weight 0.75/h) either at a DM intake adequate to the production level. Apart from the partial estimation of the NH3-free crude protein duod. a evaluation based on the crude protein and pure protein concentration in app. dig. org. matter of the ration is possible.

The hemodynamic effects of cardiac tamponade: mainly the result of atrial, not ventricular, compression.

We studied the hemodynamic effects of surgically induced regional cardiac tamponade in anesthetized dogs. Tamponade restricted to either the right or the left ventricle was compared with tamponade of either ventricle and both atria. Intrapericardial pressures were elevated to approximately 20 mm Hg. With tamponade of the right ventricle alone, aortic pressure rose from 161 +/- 3.8 to 164 +/- 3.4 mm Hg (p greater than .05) and cardiac output fell from 149.4 +/- 16.1 to 134.9 +/- 11.9 ml/kg/min (p greater than .05). However, tamponade of the right ventricle plus both atria decreased mean aortic pressure from 152.5 +/- 3.6 to 115.9 +/- 8.7 mm Hg (p less than .01) and cardiac output fell from 118 +/- 14.8 to 38.9 +/- 4.8 ml/kg/min (p less than .01). With tamponade of the left ventricle alone, aortic mean pressure changed significantly from 158.5 +/- 6.1 (control) to 148.9 +/- 5.0 mm Hg (tamponade) (p less than .05) and cardiac output was 135.5 +/- 28.3 (control) and 111 +/- 24.7 ml/kg/min (tamponade) (p greater than .05). However, when the atria were included, mean aortic pressure fell significantly more from 155.5 +/- 5.4 to 105.5 +/- 10.4 mm Hg (p less than .01) and cardiac output fell from 142.2 +/- 16 to 47.8 +/- 6.4 ml/kg/min (p less than .01). Atrial pressure rose when the atria were included, but not with tamponade of the left ventricle alone. Right but not left atrial pressure rose slightly with isolated right ventricular tamponade. We conclude that the principal hemodynamic effects of cardiac tamponade are not the result of compression of either the right or the left ventricle, but are the consequence of compression of the atria and/or the venae cavae and the pulmonary veins.

[Growth studies of SMR bulls under different feeding conditions. 4. Nutrient and energy retention in the body].

In long-term feeding experiments with bulls of the species Holstein-Friesians, the relation between live weight development and nutrient and energy retention was investigated on a high, medium and low level of feeding intensity. For this purpose a total of 206 animals was butchered at the ages 0, 4, 9, 12, 15, 16.5 and 18 months and the complete bodies were analysed. This showed that fat retention was considerably more influenced by the feeding intensity than protein retention. On the basis of the same body weight the energy content per kg live weight gain is not constant but increases with the growing feeding intensity.

[Growth studies of Holstein-Friesian bulls under different conditions of feeding intensity. 5. Derivation of energy requirement values].

From two long-term individual feeding experiments with groups of differentiated feeding intensity, energy requirement values were derived which take into consideration live weight development and which are in accordance with the measurements of nutrient and energy retention. The suggested energy requirement values show big differences between the intensity levels. It follows from this that energy expenditure from a low to a high feeding intensity first decreases and then, at a very high performance, increases again.

Hemodynamic and regional blood flow distribution responses to dextran, hydralazine, isoproterenol and amrinone during experimental cardiac tamponade.

Four different interventions were examined in dogs with cardiac tamponade. Infusion of 216 to 288 ml saline solution into the pericardium reduced cardiac output from 3.5 +/- 0.3 to 1.7 +/- 0.2 liters/min as systemic vascular resistance increased from 4,110 +/- 281 to 6,370 +/- 424 dynes . s . cm-5. Left ventricular epicardial and endocardial blood flows were 178 +/- 13 and 220 +/- 12 ml/min per 100 g, respectively, and decreased to 72 +/- 14 and 78 +/- 11 ml/min per 100 g with tamponade. Reductions of 25 to 65% occurred in visceral and brain blood flows and in a composite brain sample. Cardiac output during tamponade was significantly increased by isoproterenol, 0.5 microgram/kg per min intravenously; hydralazine, 40 mg intravenously; dextran infusion or combined hydralazine and dextran, but not by amrinone. Total systemic vascular resistance was reduced by all interventions. Left ventricular epicardial flow was increased by isoproterenol, hydralazine and the hydralazine-dextran combination. Endocardial flow was increased by amrinone and the combination of hydralazine and dextran. Right ventricular myocardial blood flow increased with all interventions except dextran. Kidney cortical and composite brain blood flows were increased by both dextran alone and by the hydralazine-dextran combinations. Blood flow to small intestine was increased by all interventions as was that to large intestine by all except amrinone and hydralazine. Liver blood flow response was variable. The most pronounced hemodynamic and tissue perfusion improvements during cardiac tamponade were effected by combined vasodilation-blood volume expansion with a hydralazine-dextran combination. Isoproterenol had as dramatic an effect but it was short-lived. Amrinone was the least effective intervention.

Electrocardiographic and hemodynamic changes in experimental right ventricular infarction.

To investigate the electrocardiographic and hemodynamic changes in isolated right ventricular infarction, 0.25 ml or 0.5 ml of metallic mercury was injected into the right coronary artery of 14 closed-chest dogs. At autopsy, at least 60% of the right ventricle was necrotic in every dog. Hemodynamic observations were made in 11 and electrocardiographic mapping was performed in all 14 dogs. Right atrial pressure rose in 10 and left atrial pressure in nine of the 11 dogs; early right atrial pressure did not exceed left atrial pressure, but late right atrial pressure was greater in four dogs. Although cardiac output and blood pressure fell significantly, circulation was maintained. Twelve of 14 dogs had transient ST-segment elevation in the right precordial leads, and 12 developed right bundle branch block. Abnormal Q waves or R waves of 1 mm or less appeared in the right precordial leads in 13 of the 14 dogs. Since right bundle branch block and abnormal Q waves in the right precordial leads have not been recognized as useful signs in human right ventricular infarction, further investigations are warranted to determine their value in clinical applications.

[Growth studies of fattening hybrid heifers. 3. Studies of the total retention of fattening hybrid heifers under different energy supply].

These investigations show that daily retention in the course of growth is subject to dynamic changes specific for each nutrient. While there are only insignificant differences between daily live weight growth as the practically usable parameter and body weight growth, daily dry matter deposition takes a comparatively different development. The specific development of dry matter deposition depends on the daily retention of protein and fat. Whereas in early age protein retention is higher, fat retention increases in the course of live weight development in dependence on the energy level. Different energy supply results in distinctly differentiated protein, fat and energy retention per day. The highest variation in daily retention could be ascertained for fat retention in dependence on energy supply. A comparison of the cumulated experimental results with model values generalised from the growth function corresponded well.
