Early mother-child relationship. The Copenhagen model of early preventive intervention towards mother-infant relationship disturbances.

The development of interactive patterns in the first year of life has been studied in healthy mother-infant pairs and in infants whose mothers have psychiatric or psychosocial problems, interfering with their relation to the child. During the first 3 months a relationship develops in healthy mother-infant pairs characterized by an intimate interchange of signals and responses between mother and child. The baby develops a basic pattern of social relating. During the next months the baby begins to expect the mother to fulfil it's wishes and still the mother adapts to and supports the baby's expanding capacities and needs. A beginning sense of self and not-self develops. At 10-12 months the infant is able to move around trying to examine everything within range. At this age the infant senses that there exist other wills and minds than it's own. The child looks at the mother and reads her expressions for "yes", "no", and "maybe". The mother can stop or encourage her child by verbal and body signals. At 12-18 months the child has developed a secure attachment apt to protect the child against later adversities. The development during the first year of life may be seriously disturbed if the mother cannot adapt to and support the baby's signals and needs. Relationship disturbances in the first year of life may be related to maternal or infant problems: Low birth weight babies, irritable babies with low stimulus tolerance and babies with withdrawal reactions following maternal alcoholism or drug abuse during pregnancy.(ABSTRACT TRUNCATED AT 250 WORDS)

EMLA-induced analgesia inferior to lignocaine infiltration in curettage of common warts--a randomized study.

The analgesic effect of topical application of EMLA cream for the curettage verrucae vulgaris was compared to local infiltration with lignocaine in 89 patients. The main purpose of this randomized study was to evaluate the patients' overall impression of the treatment. All patients anaesthetized by lignocaine infiltration reported it as 'an acceptable procedure' compared to only 71% of the patients treated with EMLA. It cannot be recommended to use EMLA cream as local anaesthetic for curettage of verrucae vulgaris, presumably because the highly keratotic nature of the common warts does not allow sufficient penetration of epicutaneously applied EMLA.

Young children of schizophrenic mothers: difficulties of intervention.

Casework by Danish local social agencies on behalf of 11 children of chronically ill schizophrenic mothers is retrospectively analyzed, along with documentation from psychiatric hospitals, consulting child specialists, and other health professionals. Findings point to a need for earlier and more precise assessment of the mother's parenting abilities as measured against the severity of her illness and the vulnerability of her child.

Onychomycosis in patients with psoriasis.

The frequency and type of fungal infections of the finger- and toenails was studied prospectively by direct microscopic and culture examinations in dermatological out-patients without psoriasis (n = 41), in patients with psoriasis but no nail involvement (n = 39), and in patients with both psoriasis and nail involvement (n = 39). In these three matched groups the frequencies of dermatophytic nail infections were 10%, 13%, and 13%, respectively, and the frequencies of yeast infections 10%, 10%, and 15%, respectively. These figures were not significantly different. It is concluded that dermatophytic invasion of involved psoriatic nails is not so rare as previously supposed. However, yeast infections are possibly more frequent in involved psoriatic nails as compared with uninvolved and normal nails.

Propylthio-uracil-induced cutaneous vasculitis.

Three cases of cutaneous vasculitis, leukocytopenia and arthralgia, presumably caused by the antithyroid drug Propylthio-uracil, are presented. Acute vasculitis of the superficial and deep dermal blood vessels accompanied by vascular thrombus formation were found in biopsy specimens. Direct immunofluorescence studies demonstrated deposits of C3 or IgM and C3 in the walls of vessels in affected and unaffected skin, suggesting immune complex deposition. The skin lesions and leukocytopenia rapidly disappeared on discontinuation of the drug, while the arthralgia continued for weeks or months.

Pharmacokinetic and clinical comparison of two 8-methoxypsoralen brands.

We compared the Danish 8-MOP brand Meladinine with the Austrial brand Oxsoralen because of our discouraging result with PUVA in treating psoriasis. Nine healthy volunteers participated in the investigation. The plasma level of 8-MOP was measured by a gas chromatogrpahic-mass spectrometric method using isotopic dilution technique. The level obtained with Oxsoralen was on the average 5.8 times higher than that with Meladinine.

Effect of gastric hypersecretion on the canine duodenum.

The neutralization of acid introduced into the duodenum has been found to be less intensive in patients with duodenal ulcer than in controls. The present work studied the possibility that chronic gastric hypersecretion injures the duodenal mucosa and thereby influences the neutralization system. Gastric hypersecretion was provoked for 3 weeks in 3 dogs by a daily injection of a gastrin preparation with prolonged effect. After a subcutaneous injection of this preparation given together with a test meal the acidity of both gastric and duodenal contents was found to increase significantly. After the 3 weeks of gastric hypersecretion the pancreatic bicarbonate response to exogenous secretin was unchanged, while the bicarbonate response to duodenal acidification was decreased from 2.03 mEq/30 min to 1.27 mEq/30 min (p less than 0.05), compatible with an impaired secretin release. Also the concentration of lactase, maltase, sucrase, and alkaline phosphatase in mucosal biopsies from the second part of the duodenum was significantly reduced (p less than 0.001). These results indicate that gastric hypersecretion causes mucosal damage in the duodenum and thereby reduces the release of secretin.

The impact of liberalized abortion legislation on contraceptive practice in Denmark.

During the fifteen months following the 1973 Danish abortion legislation, the mean annual rate of induced abortion was 23 per 1,000 women aged 15-44 compared with a mean annual rate of 14.5 during the 15 preceding months. Though use of legal abortion has increased, the rate is not of a magnitude to suggest large-scale abandonment of contraception. On the contrary, preconceptive fertility control continues to be widely practiced.
