G6PD Varadero. A new variant of glucose-6-phosphate dehydrogenase associated with congenital nonspherocytic hemolytic anemia.

A glucose-6-phosphate dehydrogenase (G6PD) variant was studied in a mulatto patient with chronic nonspherocytic hemolytic anemia. This variant has reduced activity, increased thermolability, a reduced Michaelis constant for glucose-6-phosphate, slightly increased electrophoretic mobility, a biphasic pH activity profile, high 2-deoxyglucose-6-phosphate utilization, normal diamino nicotinamide adenine dinucleotide phosphate utilization and a peak of elution profile after G6PD B. The electrophoretic, kinetic, and chromatographic properties of this erythrocyte G6PD variant allow the conclusion that G6PD Varadero is probably a new variant.

[Late complications of mechanical and biological prostheses. Comparative study].

The late clinical complications subsequent to cardiac valvular surgery were studied in 150 patients in a period covering from January 1976, to December 1981. Mechanical prosthesis were used in 99 patients and biological in 51. Mortality, thromboembolic events, and haemorrhages, were all considerably higher in patients submitted to mechanical than biological prosthesis, whereas the periprosthetic leaks were more often observed in cases with biological prosthesis. With regard to other complications, such as infectious endocarditis, postperfusion and postpericardiotomy syndromes, etc., as well as the surgery definitive results, appreciable differences were not found. This, we can conclude that any of all now available prosthesis, is free of complications, and the election of a particular prosthesis (mechanical or biological) must probably be individualized for each particular patient.

[Early diagnosis of renal ischemia in arterial hypertension by means of urine N-acetyl-beta-D-glucosaminidase (author's transl)].

Essential arterial hypertension, malignant hypertension and renovascular hypertension were studied in 64 patients, divided into 5 groups according to creatinine clearance (Ccreat) and ophthalmic fundus. Urine N-Acetyl-b-d-glucosaminidase (NAG) was expressed in units/mg creatinine in urine. Results were; Group A: Ccreat greater than 60, ophthalmic fundus less than II (n=33), NAG 13.5 + 4.5; Group B: CCreat. greater than 60, ophthalmic fundus III-IV (n=4) NAG 42.4 +/- 12.5; Group C: Ccreat. less than 60, ophthalmic fundus less than II (n = 14) NAG 31.2 +/- 10,5; Group D: (clinically malignant arterial hypertension) Ccreat. less than 60, ophthalmic fundus III--IV (n = 8), NAG 91.1 +/- 55.7 and Group E: (renovascular hypertension) Ccreat. greater than 60, ophthalmic fundus less than II (n = 5), NAG 35.5 +/- 12.9. Only the patients in Group A had NAG within normal limits. Differences were found between groups: A-B (p less than 0.001), A-D (p less than 0.001), A-E (p less than 0.001) and C-D (p less than 0.001). Urine NAG is considered to be an early sign of renal involvement in arterial hypertension, an indication of the severity and a sign of ischemia even when the involvement is unilateral only, and helpful in the management of renovascular hypertension.

Clinical and endocrine spectrum in patients with the 45,X/46,XY karyotype.

Cytogenetic and endocrine studies were performed in five unrelated 45,X/46,XY individuals in an attempt to correlate them with their clinical expression and gonadal morphology. A lack of a consistent pattern between cytogenetic findings and phenotype was observed. Endocrine studies revealed a wide spectrum of hypothalamic, pituitary, and gonadal hormone production as assessed by the base line levels of LH, FSH, T, and delta 4-A and their responses to appropriate exogenous stimulation (LH-RH and HCG). An adequate correlation between endocrine findings with gonadal morphology and phenotype could be established; thus demonstrating that patients with this particular chromosome complement have a functional integrity of the gonadotropin hypothalamic pituitary activity modulated accordingly with the gonadal function of each particular case.

Sarcoidosis associated with autoimmune haemolytic anaemia and paroxysmal nocturnal haemoglobinuria red cell abnormality.

A 29-year-old male with sarcoidosis autoimmune haemolytic anaemia and paroxysmal nocturnal haemoglobinuria is described. Throughout his illness the chest films showed fibrosis in the right hilar region and he had had several pneumonias in the right lung. He had had massive splenomegaly and a splenectomy was performed. He was treated with prednisone and cyclophosphamide. Because his blood group was initially confused, several incompatible blood transfusions were given. Two types of antibody were detected: an autoantibody with "s" specificity and an alloantibody with Rh "D" specificity. Other interesting features in this case revealed at autopsy were a viral pneumonia and Toxoplasma gondii infection of the brain. As far as we know, this is the first reported patient with this unusual association.

Screening for ampicillin resistant Hemophilus influenzae and penicillin resistant Streptococcus Pneumoniae in healthy carriers.

800 samples of pharyngeal exudate were taken from healthy children and 28 healthy intrafamilial contacts of children with meningitis caused by H. influenzae type b. H. influenzae type be was isolated in 8.7 per cent of nursery children, H. influenzae type e in 0.12 per cent and non typifiable H. influenzae in 5.8 per cent. On intrafamilial contacts, 32 per cent had H. influenzae type b and non typifiable in 3.5 per cent. Percent age of ampicillin resistant H. influenzae type b was 14 per cent. No strains were found resistant to chloramphenicol. S. pneumoniae was isolated in 37 per cent of pharyngeal exudate samples. 25.5 per cent of strains tested were penicillin sensitive; 74.5 per cent has a decrease sensitivity. Based on these results we consider chloramphenicol to the the first drug of choice for treatment of severe H. influenzae infections and penicillin for S. pneumoniae infections recommending in the latter higher doses when dealing with central nervous system infections.

G6PD Ciudad de la Habana: a new slow variant with deficiency found in a Cuban family.

A new G6PD variant has been detected in a Cuban male and there is no evidence of associated hematological abnormalities. The main characteristics of this variant, moderate deficiency, slow electrophoretic mobility, increased utilization of the substrate analogues, and a different chromatographic behavior, indicate that it is a variant that has not been previously described.

Evidence of sensitized basophils in renal transplanted patients.

The participation of basophil polymorphonuclear leukocytes in the response to kidney allografts in man has hardly been studied. In 23 patients, followed from the immediate post-transplant period for several years, the number of circulating basophils, expressed either as absolute number or as percentage of leukocytes, was significantly low in relation to that of controls. IgE on the basophil surface was determined by radioimmunoassay in 14 patients. All patients but one had elevated numbers of IgE molecules per basophil, values being higher in patients with cadaver grafts and in patients with a graft functioning at 6 months or less. When the count of circulating basophils was roughly normal, six of the eight patients showed a positive basophil degranulation test against the donor lymphocytes. These results suggest the existence of specific IgE antibodies for HLA antigens in renal transplanted patients.

Chromosomal polymorphism and somatic segregation in Saimiri sciureus.

Quinacrine (Q) and Giemsa (G) banding, and sister chromatid exchange (SCE) studies have been carried out in Saimiri sciureus. In one male and one female studied, the diploid number was 2n = 44 and the karyotype corresponded to that described for the geographical region of Georgetown, Guyana. In the second male three different cell lines were observed, one corresponding to the geographical region of Leticia (Colombia), another to the geographical location of Iquitos (Peru) and a third line with 46 chromosomes. In the three lines, pair A2 was heterozygous, with nucleolar organizers of different size, and pari B6 was also heterozygous, with different amounts of heterochromatin in the short arms. The presence of three different cell lines in this male may be due to a mechanism of somatic segregation. The chromosomal changes involved in the intraspecific polymorphism found in Saimiri have been reinterpreted.

Cerebral venous angiomas.

The literature on venous angiomas is generally confusing, although recent reports confirm the existence of this entity. A survey of the published cases, with addition of two of our own, is presented in the current study. Characteristic histological and angiographic features are described. The value of radioisotope techniques is emphasized with regard to the modifications which these angiomas cause in cerebral circulation.
