Surgical outcome of transconjunctival cryosurgical extraction of orbital cavernous hemangioma.

BACKGROUND AND OBJECTIVE: Transconjunctival cryosurgical extraction (TCE) is not an accepted method for the removal of most intraorbital tumors, and thus is not routinely performed. This study evaluates TCE for orbital cavernous hemangioma (OCH), which, due to its specific vascular structure, is suitable for this technique. PATIENTS AND METHODS: In this retrospective case series, the medical charts of 12 consecutive patients with OCH who underwent TCE were reviewed. The operative complications and the surgical outcome were analyzed. RESULTS: Eleven tumors were intraconal and 1 was extraconal. No tumor involved the orbital apex. In 10 patients (83%), the operation was uneventful. One patient lost vision due to intraoperative cilioretinal arterial occlusion. In another patient, the tumor could not be identified and was subsequently removed via lateral orbitotomy. CONCLUSION: TCE, a relatively minor operation, is as safe as more extensive surgical procedures for the removal of OCH not located in the posterior third of the orbital space.

Normolipidemic xanthelasma palpebrarum: lipid composition, cholesterol metabolism in monocyte-derived macrophages, and plasma lipid peroxidation.

The lipid compositions of 8 normolipidemic xanthelasma palpebrarum (XP) lesions were analyzed using thin-layer chromatography, with the adjacent uninvolved skin used as control. The lesions were found to be composed predominantly of cholesterol, mostly cholesteryl ester, whereas in the control specimens phospholipids predominated. The degradation rates of 125I-low-density lipoprotein (LDL), oxidized LDL, and acetyl LDL, and the rates of intracellular cholesterol synthesis from 1,2-(14)C-acetate, in blood monocyte-derived macrophages (MDM) from 3 normolipidemic patients, were similar to those of MDM from 3 normal control subjects. The mean levels of lipid peroxides and conjugated dienes under basal conditions, as well as following the addition of a free radical-generating compound (2,2-azobis-2-amidinopropane hydrochloride) to the plasma of 14 normolipidemic XP patients were significantly higher than those of 14 age- and sex-matched normal controls. We conclude that the predominant lipid accumulated in normolipidemic XP lesions is cholesteryl ester, but there is no evidence for intrinsic cellular cholesterol metabolism derangement in blood MDM from patients which could account for this. Since macrophage cholesterol accumulation can also result from enhanced uptake of increased levels of oxidized LDL, the increased plasma lipid peroxidation (derived from oxidized LDL) might lead to accumulation of cholesterol in macrophages and formation of foam cells via this mechanism.

[Facial injury by fishing harpoons].

The methods of removing penetrating fish-hooks or harpoons are unique and require understanding of the structures of these instruments and their mechanisms, and methods of neutralization. We describe 2 cases of penetrating spear-gun fishing harpoon to the face and the recommended clinical management.

End-stage fibrosis of the lateral rectus muscle in myopia with esotropia. An ultrastructural study.

The fibrotic lateral rectus muscle and the medial rectus muscle of 4 patients with esotropia and myopia who underwent surgical correction were studied ultrastructurally. All patients revealed underactivity of the lateral rectus muscle. In 3 patients the underactive lateral rectus was completely fibrotic; in 1 patient there was 80% fibrosis, and the rest of the muscle showed different stages of degeneration. The fibrotic lateral recti were composed of collagen fibrils and remnants of degenerated organelles. The collagen fibrils of the lateral recti were of different sizes, and the mean diameter varied from 65 to 92 nm. On the other hand, the interstitial collagen fibrils of nonfibrotic medial rectus muscles of these patients and also of control muscles showed less variation in size, and the mean diameter was smaller as compared with the fibrotic lateral rectus. It is suggested that the collagen fibrils that compose the fibrotic lateral rectus muscle probably develop under conditions which differ from the normal development of collagen fibrils.

Retinal function of the diabetic retina after argon laser photocoagulation assessed electroretinographically.

Electroretinographic (ERG) responses were measured in diabetic patients before, during, and after panretinal photocoagulation treatment with argon laser. The laser applications reduced considerably the amplitudes of the a and b waves of the ERG. Moreover, the relationship between the amplitude of the b wave and that of the a wave was severely affected, resulting in ERG responses of abnormal pattern. The b waves were smaller than expected from the a waves. These findings indicated that the photocoagulation treatment not only destroyed the retinal areas directly illuminated by the laser beam, but also affected the functional integrity of adjacent areas. These additional effects resulted in subnormal signal transmission from the photoreceptors to the proximal retina.

Fine-needle aspiration of orbital lesions.

In seven patients with orbital lesions, fine-needle aspiration (FNA) was performed. No complications were seen in performing this procedure. FNA proved to be an important adjuvant method in the diagnosis of orbital lesions, especially in their pre-operative cytologic definition.

Bee and wasp stings of the eye. Retained intralenticular wasp sting: A case report.

Two different stages should be considered in the management of bee and wasp stings to the eye. The first is the acute stage of activity of the specific insect venom on the structures the eye. The reaction of the eye to the particular insult is considered here in regard to the anterior and posterior segments of the eye. The second stage is that of the retained intraocular foreign body, the inert sting. Little is known about the reaction of the eye to the presence of chitinous sting and its effect on the structures of the eye. We report a follow-up study of a sting retained for 28 years, and emphasise the benign and quiescent course of the case. Guidelines for treatment and management in such cases are described.
