[Granulomatous pneumocystis pneumonia].

INTRODUCTION: Granulomatous pneumocystis pneumonia (PCP) is a rarity whose presentation may be misleading. CASE REPORT: We report the case of a patient suffering from chronic lymphatic leukaemia presenting with acute respiratory insufficiency, with a fatal outcome, due to granulomatous PCP. Broncho-alveolar lavage (BAL) remained negative and the diagnosis could only be made later by open lung biopsy. CONCLUSION: Although infrequent, granulomatous PCP should be recognised because in this situation the standard investigation (BAL) may be negative. New, more sensitive, methods of investigation such as the polymerase chain reaction (PCR) may permit earlier diagnosis.

[Cardiocirculatory arrest after administration of combined mifepristone (Mifegyne) and sulprostone (Nalador) for induced abortion. Possible role of coronary vasospasm].

Reversible ventricular fibrillation occurred in a 35 year old multiparous woman after the administration of the association mifepristone (Mifegyne) and sulprostone (Nalador) for therapeutic abortion. A coronary spasm induced by the analogue of the prostaglandin PGE 2x was though to be the causative mechanism and was reproduced by the Ergonovine test. The evolution of the ECG and myocardial scintigraphy suggested a slowly regressive stunned myocardium.

Early cerebral prognosis of anoxic encephalopathy using brain energy metabolism.

Cerebral arteriovenous differences in oxygen content [C(a-v)O2] and glucose content [C(a-v)gluc] were used to predict outcome in 20 patients who remained comatose after resuscitation for cardiopulmonary arrest. Cerebral prognosis was good when C(a-v)O2 values were greater than 1.8 mMol/L and the oxygen glucose index (OGI) was greater than 85%. Our results suggest that cerebral recovery is linked with increased oxidation associated with the beginning of nonglycolytic metabolism.

[Plasma fibronectin. Role in severe infections and intensive care].

Plasma fibronectin (pFN) levels were measured in 52 patients throughout their stay in an intensive care unit. At the same time a close watch was kept for signs of sepsis. Low pFN levels regularly accompanied--but did not precede--infectious episodes and were also found in patients without septic complications. These findings indicate that a fall in pFN does not constitute a factor of increased susceptibility to infection.

[Increase in mean platelet volume in shock-related thrombocytopenia].

In a retrospective study the mean platelet volume (MPV) was measured in citrated plasma of 16 patients with hypovolaemic or infectious toxic shock. Bone marrow examination was carried out in most cases. Eighty p. cent of the patients had an increase in MPV, and the increase persisted even after thrombocytopenia had regressed. Cytological examination of the bone marrow failed to provide evidence that bone marrow deficiency was a predominant factor in the pathogenesis of thrombocytopenia in any of these patients. In this particular patient population, bone marrow examination should be reserved to those cases where persistent thrombocytopenia coexists with normal MPV.

Factor II related antigen and antithrombin III levels as indicators of liver failure in consumption coagulopathy.

In order to detect impaired synthesis of blood coagulation factors associated to consumption coagulopathy, a simultaneous evaluation of factor II-related antigen (II rAg) and of antithrombin III (AT III) was carried out in 16 patients affected with severe defibrination. An in vitro preliminary study on plasma and serum demonstrated that the levels of II rAg and of AT III, assessed by the Laurell technique with Behring antisera, were not reduced by the coagulation process. The patients were, a posteriori, classified into two groups according to the absence (group A) or the presence (group B) of factors predisposing to liver failure such as metastasis, cirrhosis, and prolonged shock. II rAg and AT III levels are significantly correlated; they are in the normal range in group A but reduced in group B. Thus II rAg or AT III level determinations are useful markers in the detection of liver failure associated to the consumption phenomenon. These results also suggest that part of the decreased AT III levels reported in severe cases of disseminated intravascular coagulation may be the consequence of an associated liver failure.

[Haemoperfusion on coated activited charcoal. Experience in French anti-poison centres based upon 60 cases (author's transl)].

Sixty three sessions of haemoperfusion on coated activated charcoal were carried out in 60 cases of poisoning. The indications concerned 2 clinical situations: - massive poisoning with coma (by hypnotics or chlorine-containing solvents) with high extracellular concentrations of the toxic agent, with the aim of reducing the duration of the coma; - intoxications with a high mortality rate, such as those due to paraquat, colchicine, digitoxine, tricyclic antidepressants, and heavy metals, even when plasma levels were low in relation to the dose ingested. Such discordance may be related not only to predominantly intracellular fixation, but also to low intestinal absorption. The effectiveness of haemoperfusion was estimated in terms of the amount of toxic substance extracted, measured either indirectly by repeated calculation of the arteriovenous difference multiplied by the flow rate through the column, or directly by elution of the column. This method for the removal of toxic substances would not appear to be life-saving, is too costly for the extraction obtained and is not free from specific complications. Thrombocytopaenia remains an epiphenomenon, but the frequency of infection is significantly higher than with haemodialysis. An effective and safe method for the vascular extraction of poisons, particularly one favourising removal of toxic substances from intracellular localisation, remains to be found.

[Study of home ventilation in severe respiratory insufficient patients (author's transl)].

Results of our study of 91 cases show that many indications of ventilation at home are debatable:--Tracheostomized patients submitted to continuous ventilation with short disconnected periods;--Patients using the respiratory apparatus more than 4--6 hours per day, through a mouth-piece. The existence of a reserve closely related to the intensive care department and mobile emergency unit (S.A.M.U.) facilitates supervision and eventual technical or medical assistance. In addition, the running cost remains small because of the adaptability of the system. However, it seems of interest to point out a group of 11 patients whose treatment was interrupted either because of their improvement (physical therapy, suppression of tobacco mainly), inadequate indications, or poor cooperation.
