Detection of Parietaria judaica airborne allergenic activity: comparison between immunochemical and morphological methods including clinical evaluation.

Airborne pollen grains and small plant particles of respirable size are the main causes of rhinitis and asthma in pollinosis patients. Consequently, it could be useful to quantify atmospheric variations in these biological aerosols and their allergenic activity as a basis for establishing correlations with the clinical symptoms in these allergic subjects. Our study was conducted in Naples from May to August 1992 and from May to August 1993 and concentrated on Parietaria judaica (Par j) the most important hay fever-provoking plant in southern Italy. The aim of this study was to evaluate the day-to-day variation in counts of Par j. pollen, the day-to-day variation in allergenic activity specific for this plant, and to relate these two variables to symptom scores in patients with respiratory allergy to Par j. The airborne pollen grains were collected by a Hirst-like volumetric trap and examined through an optical microscope, while the allergenic activity of particles trapped on glass fibre filters in a high volume air-sampler was determined by immunochemical assay. The two devices were placed on the flat roof of the 'A. Cardarelli' Hospital. The results of this preliminary study suggest that both methods could have useful clinical relevance, since these two biological parameters were significantly correlated with the symptom scores of allergic patients. However, pollen count with morphological quantification of grains/m3 of air is a more simple technique.

Respiratory distress in a newborn with primary ciliary dyskinesia, situs inversus and Turner syndrome.

A report is given of a newborn girl with situs inversus and Turner syndrome that presented respiratory distress. The patient had a mosaic karyotype 45,X/46,X + mar (80%/20%). Ciliary motion analysis demonstrated a total absence of ciliary motion whereas, ultrastructural studies revealed typical features of primary ciliary dyskinesia (PCD) (absence or short outer/inner dynein arms in 90% of the cilia). We regard this rare combination (PCD, situs inversus and Turner syndrome) as a coincidental occurrence.

[Lumbosacral plexus block by the paravertebral route].

The authors present a detailed account of a technique of peripheric anesthesia of the lower limb, obtained with a single transcutaneous injection, blocking lumbar and sacral plexus, by a paravertebral way. Encouraging results have been reached in the treatment of both old (no cerebral or cardiorespiratory depressant effects) and young patients, which are frequently under day-hospital treatment. According to their experience, this technique is simple, easy to perform and less dangerous if compared to anesthesia obtained by spinal approach.

[A uterine rupture during peridural anesthesia].

A 31 years old multiparous woman was delivered by caesarean section at her first pregnancy. For her third, spontaneous, full-term labour an epidural analgesia was realized with 0.125% epinephrined bupivacaine and fentanyl (1 mg per kg). In spite of its initial efficacy, pains appeared only localized at the caesarean scar. An uterine rupture was suspected, due probably to the use of oxytocin. The operating room was immediately prepared for an emergency caesarean section. As none other pejorative clinical manifestation was present, vaginal delivery was pursued and an healthy boy was taken out after application of forceps. The uterine revision and the secondary laparotomy, performed under epidural anesthesia, confirmed an uterine break, not very wide. The end result was satisfactory both for the mother and the child. The use of epidural analgesia and oxytocin are discussed from the literature and the clinical facts. The absence of internal tocography is an arguable point in this case report.
