[Tetralogy of Fallot: results of 77 hemodynamic controls after complete correction].

77 patients (representing 91% of all survivors) underwent catheterization within 5 months of surgical repair of tetralogy of Fallot. The data show that residual pulmonary gradient is significantly higher in patients with infundibular and valvular stenoses than in patients with infundibular stenoses only, whereas the method of intracardiac repair (patch) had no influence on residual gradient. Significant pulmonary insufficiency was found almost exclusively when a patch was extended through the pulmonary annulus. Hemodynamic results were excellent in 37% of cases, good in 51%, satisfactory in 3% and unsatisfactory in 9%. Finally, primary intracardiac repair in children below 4 years of age yielded excellent or good results in all cases: this suggests that toal correction should be performed without previous aorto-pulmonary anastomoses in this youngest group of patients.

[Immediate and long-term prognosis of sub-endocardial infarct].

A comparison is conducted of two groups of patients with myocardial infarction: one group of 77 patients with subendocardial myocardial infarction (SEMI) and another of 166 patients with transmural myocardial infarction (TMI). In-hospital mortality, ventricular arrhythmias and a-v blocks are higher in TMI. Late follow-up (mean: 43 months) shows, however, that after discharge SEMI shows a higher mortality and more angina and recurrences of myocardial infarction that TMI. These results, and the ECG localization of the MI recurrences, suggest that SEMI usually reflects severe coronary artery disease.

[Diagnosis of pericardial effusion by echocardiography and radionuclide angiography (author's transl)].

Echocardiography and radionuclide angiography are presently the two first choices non-invasive methods for the diagnosis of pericardial effusion. To evaluate the sensitivity and specificity of these methods, a prospective study of 43 consecutive patients was undertaken. Confirmation by tap or autopsy was obtained in 10 cases; for the remainers, strict clinical signs were requested. The results of both methods were classified in a quantitative manner. Our data confirms the greater sensitivity and specificity of echocardiography compared with radionuclide angiocardiography, when small amounts of effusion are present. However, the correlation between the two methods is satisfactory when a greater amount of fluid is present.

Lymphography as a guide to prognosis in malignant testicular tumours.

Lymphography was performed in 130 patients with malignant neoplasm of the testes. The patients were treated by irradiation, supplemented by chemotherapy when lymph node metastases were present. The survival rates of the various pathologic subdivisions were analyzed according to the lymphography. The following 5-year survival rates were observed: seminoma: normal lymphography 94%, pathologic lymphography 69%; teratocarcinoma: normal lymphography 86%, pathologic lymphography 14%; embryonal carcinoma: normal lymphography 64%, pathologic lymphography 33%.
