Sexually transmitted diseases in Italy: clinical returns versus statutory notifications.

Sexually Transmitted Diseases (STD) surveillance has caught the attention of the Italian public health authorities during the past decades. The spread of HIV infection increased the awareness of the need for a better STD surveillance system. This paper reports a retrospective survey of STD clinics in Italy, intended to form an epidemiological base for further development of STD surveillance. The data for 1986-87 and 1988 on a predefined set of diseases, all of them sexually transmitted, were collected from 85 of 127 centres contacted. The data obtained offer the first national figures for some STD not included in the statutory notification list, such as chlamydial infection, herpes and genital warts, as well as HIV infections. Those data show an increase in time frequencies. For gonorrhoea and syphilis, it was possible to compare our data with statutory notifications, showing a large gap between notified and reported cases. This gap is not stable in time (increasing in time) or in geographical area (increasing from north to south).

[Breast metastases of skin melanoma].

Although breast metastases are not frequent, the most common ones are from cutaneous melanoma. In this paper two cases of breast metastases from malignant melanoma are reported. The literature is reviewed and the diagnostic and therapeutic problems related to metastatic mammary neoplasms are analyzed.

[Amalgam fillings and T-lymphocyte changes].

Dental amalgam and nickel alloys have been considered quite safe. Previous authors reported the effect of dental amalgam and nickel alloys on human T-lymphocytes modifications after amalgam dental fillings, into dose-dependence of any modifications and into possible temporary. Eight patients were subjected to dental care with amalgam dental fillings. Drawings of blood were executed at start, fifteen days after late fillings and two months later. The results about modifications of T-lymphocytes were not univocal. We believe, at now, that temporary modifications of the immunity seem to be related to a cytotoxic mechanism.

Serum B2-microglobulin and human immunodeficiency virus infection in intravenous drug users: a cross-sectional study.

Beta 2-microglobulin level has been evaluated in 183 male intravenous drug users and 50 apparently healthy blood donors. The aim of the study was to assess the possible association between high values of the peptide and both HIV infection and disease progression in intravenous drug users. Beta 2-microglobulin values were found significantly higher in AIDS patients and other symptomatic subjects than in asymptomatic, both seropositive and seronegative, intravenous drug users. Among asymptomatic drug users, no significant difference in the values was found.

Analysis of chewing movement using elliptic Fourier descriptors.

Mandibular kinesiography measures and records the plot of jaw movements in the three spatial planes; this technique not only offers to investigators information about the dynamics of chewing, but also opens important prospects of quantifying dysfunction. At present, chewing patterns are compared on a qualitative basis only, and their morphologic analysis is not accurate. It is therefore necessary to use suitable mathematical algorithms that can manage the data supplied by modern image analyzers without losing the large amount of information they contain. A mathematical model that, by using a Fourier description of the plots of jaw movements, analyzes and characterizes different chewing classes has been developed. As an example of the possible applications, the variability of the exclusively morphologic aspect of the frontal plane plots of the chewing cycle of some subjects under standard conditions was studied and quantified. A morphologic distance between the Fourier coefficients that characterize each plot and the coefficients of a reference plot was also calculated. On these bases it will be possible to define and measure a "normal" chewing cycle under standard conditions and to make an eventual diagnosis of dysfunctional chewing based on a true morphoquantitative evaluation. Moreover, by deepening the analysis of the more occlusal part of the cycle, the model could aid in the identification of unapparent malocclusions.

[Critical review of 121 squamous cell epitheliomas of the lip. Role of surgery in the treatment of primary lesions and of the loco-regional lymph nodes].

The Authors report their own experience about carcinoma of the lip and evaluate the results of surgical treatment regarding T and N variables. 121 patients were operated on: 109 of them (90.08%) healed, 7 (5.79%) died. We had 2 local (1.65%) and 3 regional (2.48%) recurrences. Metastases in nodal specimens accounted for 3 patients (11.54%) of the 26 with elective lymphadenectomy. Only 8 (34.78%) of the 23 nodal dissections done because of macroscopic involvement at the operation were found to be histologically involved.

A national voice for information about HIV infections: the Italian state AIDS hot-line.

Because of the variety of problems connected with the AIDS epidemic, there is an urgent need for qualified information and counselling. In June 1987, The Italian Ministry of Health established a hot-line, acting within the National AIDS Operational Centre. Through the analysis of more than 39,000 calls received during the first year of activity, a clear outline has been obtained of the emotional impact of the disease on Italian public opinion, and of the problems raised. 67% of callers belong to 20-39 age group; the male/female ratio is 2:1. Interesting correlations have been found between the content of the questions and the sex, age, and type of caller. The vast majority of questions concerning basic concepts and notions about HIV infections are asked by no-risk-related subjects. During the first year, the percentage of questions asked by subjects actually at risk increased, and the questions themselves have become more specific.

The Italian AIDS 'hot-line': providing information to the people.

The Italian AIDS hot-line has recently completed its first two operational years. Established as an emergency service, it was initially overloaded with calls, under the emotional impact of a new disease. As the average number of calls settled to around a hundred per day, a wider approach aimed at the concerns raised by callers has been possible. A particular effort has been made to handle psychological and social aspects of HIV infections. A large proportion of the questions analyzed (62,500) deal with doubts about transmission (30%), reliability of diagnostic tests (15%), and handling individual situations. Some categories of callers, such as drug users, parents or relatives of HIV-infected people, and seropositives often require experienced telephone counselling. HIV-positive callers follow the proportional distribution of AIDS cases in Italy: intravenous drug users forming the majority, followed by homosexuals and finally heterosexual subjects (reaching 10%, a value in accordance with trend projections). The demand for advice has increased over time, together with an evident refinement of question content.

Extra-ribosomal spacer sequences in Triturus.

We show that, in Triturus vulgaris meridionalis, sequences homologous to the rDNA "non-transcribed" spacer (NTS) are clustered at chromosomal loci where they are not associated with 18 S or 28 S rDNA genes: these sequences are referred to as the extra-ribosomal spacer sequences. Genomic clones containing such extra-ribosomal spacer sequences have been isolated. As shown by restriction mapping, these clones appear to consist mostly of repetitive BamHI fragments that are, in turn, internally repetitious and highly homologous to each other. The structure of the clones was confirmed by nucleotide sequence analysis, which also demonstrates the high degree of conservation between the BamHI elements and the homologous NTS sequences. An intriguing 12 base-pair homology between the extra-ribosomal spacer sequences and a Xenopus NTS enhancer sequence is reported. The possibility that a repetitive octanucleotide motif found within the BamHI elements could act as a recombination hotspot by virtue of its similarity with the Escherichia coli chi sequence is discussed.

Massive hemolysis in Clostridium perfringens infections.

Over a 14-month period at the Cleveland Clinic Foundation, 424 strains of Clostridium were isolated; of these, 52 strains were Clostridium perfringens isolated from 41 patients. Eight strains of C. perfringens were isolated from the blood of six patients; five of these patients had neoplastic disease and three developed massive intravascular hemolysis with rapidly developing shock and death. Clinical details are given on three patients with fatal Clostridium perfringens sepsis, and the nature of presentation and pathophysiologic mechanisms are discussed.

Two fatal cases of type E adult food-borne botulism with early symptoms and terminal neurologic signs.

Type E botulism, one of the least common forms of botulinal intoxication on the East Coast of the United States, is described for two elderly patients with chronic underlying disease. Both patients consumed tainted kapchunka, a salted, ungutted whitefish. Gastrointestinal symptoms and signs were prominent, but neurologic complaints, although noted soon after the consumption of the fish in one patient, did not progress until late in the course of the patient's illness. One patient exhibited both urinary retention, which was reported mainly in one outbreak of type E botulism (M.G. Koenig, A. Spickard, M.A. Cardella, and D.E. Rogers, Medicine [Baltimore] 43:517-545, 1964), and muscular fasciculations, which have been rarely reported.
