Problems in the psychoanalysis of certain narcissistic disorders.

This paper considers the psychopathology and treatment of a particular group of narcissistic disorders which can be characterized by their use of 'simulation' in their attempt to resolve the conflicts of the Core Complex. The invasive, narcissistic mother's demand for success is responded to with an evident compliance but with a covert rebellion manifested in failure, accompanied by intense feelings of shame and low self-esteem. This is seen in the analysis in the patient's apparently positive response being followed by a Negative Therapeutic Reaction. The structure is seen to be as much concerned with protecting the mother from the infant's destructiveness as it is with protecting the infant from the colonizing mother (cf. Winnicott's 'False Self'). What is regarded as the hallmark of narcissistic disorders, namely shame, is found to be part of a defensive structure concerned with the exclusion of extreme violence and intolerable guilt. The significance of the father is discussed. Difficulties that arise in the analysis of such patients are considered.

Is early onset of gray hair a risk factor?

An office and autopsy study was performed to see if early graying was associated with increased morbidity, earlier age at death, and specific cause of death. 195 consecutive office patients over the age of 40 were studied to see if premature graying of scalp hair (50% or more gray before age 50) was associated with increased incidence of disease before age 50 (P = ns). Their parents' mean ages at death, prematurely gray or not, were compared. For fathers, mean age at death if prematurely gray was 68.27 years; if not prematurely gray, 66.03 years (P = 0.35). For mothers, the values were 70.55 years and 70.37 years respectively (P = greater than 0.50). 874 autopsy patients dying over a 23-year period (1966-1989) were studied to see if the median age at death (of patients 50% or more gray) differed for any of the six categories of disease (myocardial infarction, congestive heart failure, cancer, stroke, pneumonia/bronchitis, or cirrhosis of the liver/GI problems) when compared to the entire autopsy sample of 19 categories of disease (P = ns for each comparison). This dual office and autopsy study provides no evidence to support the contention that early gray hair is a risk factor.

Homosexuality in adolescence.

This paper asserts that homosexuality in adolescence should not be regarded in the same way it would be in adults. Different types of homosexuality emerge during adolescence according to the different psychodynamic conditions which occur at different stages of adolescence, particularly the changing relationship to the individual's parents, and these may subside or make an important contribution to the establishment and nature of an individuals' homosexuality in adulthood. The characteristic 'identificatory facility' of adolescence is discussed and it is suggested that this is retained in the make-up of the adult homosexual. Some comments on the clinical assessment of homosexuality in adolescence are briefly made.

Contraceptives and the conceptus. I. Chromosome abnormalities of the fetus and neonate related to maternal contraceptive history.

The chromosomes of 3080 products of conception, consisting primarily of newborn infants and medically induced abortions, were examined. These were from 3 major groups of women, those who had used oral contraceptives (primarily the combination types), those who had used nonhormonal contraceptives, and a control group of those who had not used any contraceptives. In the newborn study population, the chromosome abnormality rate was 5.4 per 1000 infants (when cases of proven hereditary involvement are excluded the figure drops to 3.6 per 1000), and in the entire induced-abortion study series it was 7.3 per 1000. These rates are very similar to those obtained in other surveys. The rate of chromosomally abnormal conceptuses increases with increasing maternal age in both the newborn and induced-abortion groups. No statistically significant relationships were found between the observed or age-adjusted rates of chromosome abnormalities and contraceptive history (P greater than .10 in all cases). The findings make it clear that the use of oral contraceptives has no large effect on the risk of having a chromosomally abnormal child although the possibility of a small increase or decrease in this risk, of at most about 10 per 1000, cannot be ruled out because of the small number of abnormalities found.

Self-regulated methadone detoxification of heroin addicts. An improved technique in an inpatient setting.

We used a method of detoxifying heroin addicts involving a self-regulated schedule of methadone hydrochloride detoxification in an inpatient setting. This method allows the addict to receive methadone on an "as needed" basis within specified guidelines, thus permitting him to regulate his own detoxification. For this study, 30 chronic heroin addicts were detoxified using this self-regulated detoxification procedure. Measures of length of stay, amount of methadone required, and degree of patient satisfaction indicate that this is a practical means of withdrawing chronic heroin addicts that may have advantages over fixed withdrawal schedules.

Chromosome polymorphism in a human newborn population. II. Potentials of polymorphic chromosome variants for characterizing the idiogram of an individual.

Replicate chromosome preparations of umbilical-cord-blood leukocytes from 376 neonates born at the Albert Einstein College Hospital, Bronx, New York, were stained with C-, Q-, and G-banding methods to determine the frequencies and distributions of the variable chromosome bands. The C-band variants of primarily chromosomes 1, 9, and 16, as well as those of the remaining C, E, and F-group chromosomes, and the brightly fluorescing Q-band variants of chromosomes 3 and 4 and all of the acrocentrics, including the Y, were similarly analyzed. Polymorphism of these chromosome regions was so extensive that the idiogram of each of the 376 newborns of this study had a unique variant pattern, even when only the C- or only the Q-band patterns were compared. The distribution of polymorphic Q-bands in the population sampled was consistent with the expectations of the Hardy-Weinberg law, with the exception of chromosomes 3 and 22, where some deficiency of individuals with "homozygous" Q-band patterns was found. The baseline data presented here reinforce the view that polymorphic chromosome characteristics are very useful markers for characterizing the karyotype of an individual, for pedigree studies, for prenatal chromosome analyses, for population studies, for attempts at gene localizations, and for identifying specific cells or their chromosomes in somatic cell genetic studies.

Out-patient cataract surgery.

A randomized prospective study of 100 consecutive cataract operations was designed to ascertain the need for hospitalization for this procedure. The results clearly indicate that immediate ambulation and discharge of the cataract patient do not detract from the very high success rate of this operation. The random nature of this study adds strong support to those surveys that have similarly concluded that hospitalization and postoperative restrictions are not necessary for the great majority of cataract patients.

The role of the superego in exhibitionism.

The exhibitionist's relationship to his superego is a definitive feature of his general psychopathology of which the exhibitionistic act is only one manifestation. This relationship involves a compelling and persistent active defiance of those aspects of the superego which impose moral and ethical restrictions on behavior and a pervasive resistance to those aspects of the superego which demand adequate standards of functioning and conduct. Some of the implications of this as regards the treatment of exhibitionists are discussed.
