Comparative investigations on water-soluble crystallins of the embryonic, fetal, and postnatal human lens during development and ageing.

To compare the crystallin composition of embryonic and fetal human lenses with those of postnatal and adult lenses, we investigated the crystallins of lenses of various ages (from the 5th gestational week to 55 years) by gel chromatography, isoelectric focusing, immunodiffusion, and immunoelectrophoresis. Age-related changes were calculated as relative percentages of the different classes and subclasses of crystallins. During prenatal lens development the percentages of both high- and low-molecular-weight alpha-crystallins as well as gamma-crystallins gradually increased, whereas the percentage of beta-crystallins decreased. A considerable change in crystallin composition was found immediately after birth: the relative percentage of beta-crystallins increased, whereas that of gamma-crystallins decreased. Gel-filtration analysis of crystallins from juvenile and adult lenses showed a high-molecular-weight peak, which was not found in extracts from fetal and new-born lenses.

[Rupture of follicular and corpus luteum ovarian cysts--the cause of acute gynecological abdomen].

The symptomatology of the ruptured follicular and corpus luteum cysts varies from clinical occult status to haemorrhagic shock according to clinical studies for a ten years period. The syndrome frequency is higher in women over 30 years of age at rate 1:2 of the follicular to luteum cysts affecting mainly the right ovary. Twenty percent of the patients have been treated conservatively. Preoperative and intraoperative diagnosis coincides in 92% of the cases.

[Abortions on demand in adolescents].

The pregnancy is an integral part in the life of teenagers. When the conception was appeared the adolescents have to choose between interruption of delivery. Psychological status, contraceptive use and sexual behavior, motivation for abortion, operative methods and complications were observed in 463 teenagers, aged 13-17 years, who had an abortion voluntary in the last 6 years (1988-1993). We were found an abortion rate to 2.33% in our Dept. Obstet. Gynaecol. and a trend to increase the number of delivery in teenagers. The medical complications of abortions were: perforatio uteri complicata (0.65%) and endometritis acuta (9.29%), in cases with pregnancies over 16-20 g.a. This investigation was characterized from the teenagers as an stress and will be able to be complicate with a psychological disorders.

[Contraception among adolescents].

Adolescence is a period of which the contraception is difficult, because of lower compliance of the teenagers to this methods. The authors were observed the contraceptive uses in 792 pregnant adolescent girls, 264 of which with abortions (group A) and 528 teen-age mothers (group B). Only 12.88% of all patients have used birth control methods, usually with low and middle effects. We founded only 15 patients (1.89%) practiced hormonal contraception and 1 case (0.13%) with Intrauterine devise. The condoms for men was the most popular and useful in teenagers (4.23%), and Natural family planning methods were practiced from 52 (6.57%) patients. There was the statistically differences between adolescent girls included in group A and B about their contraceptive behaviors and live standards (p < 0.01). By the kind of contraceptive methods, there was a high differences in the teenagers with a hormonal uses (p < 0.05).

Age-related changes of water-soluble proteins of human eye lens during the prenatal period.

Saline extracts from human lenses were studied during prenatal eye development. Using immunodiffusion and immunoelectrophoresis with a specific antiserum against total extract of human adult lenses and monospecific antisera against the individual crystallin classes, it was established that alpha and beta crystallins were present in extracts from eye lenses from 5-week-old embryos while gamma crystallins could be detected in extracts from embryonic lenses at the beginning of the 6th gestational week. When analyzed by isoelectric focussing all alpha and beta crystallin components were found in extracts from 8-week-old embryos, while those of gamma crystallins were shown to appear successively within the period between the 8th and 14th gestational week. Some changes in the relative proportion of the different crystallin classes and subclasses were observed in the course of prenatal eye lens development between the 8th and 26th gestational week, as the percentage of alpha, high molecular weight and gamma crystallins was increased while the proportion of beta crystallins was gradually lowered.

[The risk factors to the fetus and newborn infant with mothers under the age of 16].

The study described risk factors for the fetus and newborn up to one year of age in 271 mothers under 15 years of age, using the alternative analysis. It was found that the body mass of the newborns of under age mothers was 328 g lower (Pt less than 0.0001) than the mean body mass of the whole population in the region, but the frequency of underweight newborns was twice as large. Perinatal infant mortality of the examined group did not differ from the mean mortality of the whole population in contrast to almost three-fold higher mortality in newborns up one year of age. Feeding and morbidity did not differ substantially from those of the remaining children in the region. Malformations were the most frequent cause for postnatal mortality. The authors conclude that inspite of accelerating processes the medico-social risk for still unborn infants of pregnant under age individuals remain rather high.

[Premature sexual activity among girls and an attempt at sex education].

In connection with early sexual activity among juveniles the authors interview 292 girls at the age of 15-18 years in three towns (Sofia, Pleven, Cherven Bryag). Sexual activity is studied in girls as well as their initial information level in taking decision for sexual participation. 38 (13.01) of the girls were sexually experienced as 12 (13.57%) of them had sexual activity more than 2 years. Girls, aged 16 and 17 years, had the highest sexual activity (44.75%). 10 (26.31%) of the sexually active girls had more than 2 partners. 11 (28.95%) of the girls were informed very well, but 4 (10.53%) of them had almost no information. 4 (10.53%) of sexually experienced girls used contraceptives and those were mainly condoms and pills. The necessity of timely sexual education is stressed, taking into consideration that pregnancy is a consequence of early sexual activity at adolescent age.

[Obstetrical surgical interventions in teenagers younger than 16 years of age].

The age of adolescence is not biologically the proper one for performing obstetric interventions regarding both the individual psychosomatic growth of the girl and the requirements of the reproductive and dermatographic practice. The authors analyzed in the present study the incidence of major obstetric operative interventions according to the indications and age distribution on the basis of 10,182 deliveries, 271 of which belonged to teenagers. The incidence of cesarean section was 3%; forceps delivery--2.2%; vacuum extraction--0.4%; assisted breech delivery and breech extraction--2.2%; version--0.4%; inspection of the uterine cavity: manual 1.8%; instrumental--0.7%. The total incidence of major obstetric operative interventions in teenagers was 11% and compared with the operative practice in the clinic in general it was almost equal or even less. The incidence of the small obstetric repairs was significant: perineal lacerations--32.1%, cervical lacerations--2.2%, vaginal lacerations--11%. The low incidence of cesarean section (total incidence for the clinic 6-9%), contrary to expectations, was not due to fetal-pelvic disproportion in the teenagers. The aforementioned results are a part of a study, covering a three year period, designed to clarify problems on different aspects, regarding the medical and social grounds of the pregnancy and delivery in teenagers.

[The development of pregnancy and labor in adolescents].

271 girls under the age of 16 years and 21-under the age of 14 years delivered at the obstetric clinic during the last three years (1985-1987). Compared with the total number of deliveries the frequency of deliveries in teenagers represented 2.7%. 1191 (11.7%) teenagers delivered at the adolescent age (up to 18 years) during the same period of time and 1781 (17.7%) teenagers performed induced interruption of pregnancy. The authors think that this frequency gives sufficient foundation to study obstetric problems in teenagers. The following parameters were examined: age, social status, menarche, pathology and duration of pregnancy, attendance to antenatal clinic, parity, way of delivery, complications of the mother and fetus during delivery. Besides some reservations a general inference is made that advancement and delivery in teenagers according to the material analysed from an obstetric point of view did not present more elements of pathology, encountered after that age. The authors widen their study and outline problems from psychosocial aspect, taking into consideration some medicobiological peculiarities in the course of pregnancy and delivery in this rather young group of teenagers.
