Wound infection after cholecystectomy. Correlation between bacteria in bile and wound infection after operation on the gallbladder for acute and chronic gallstone disease.

OBJECTIVE: To see if there was a difference in the wound infection rates after operation for acute and chronic cholecystitis, and to see if the presence of bacteria in the bile had any influence on those rates. DESIGN: Prospective open study. SETTING: University hospital. SUBJECTS: 213 Patients undergoing cholecystectomy for acute or chronic gallstone disease. MAIN OUTCOME MEASURES: Incidence of postoperative wound infection, and of bile cultures growing pathogenic organism. RESULTS: There was no difference in wound infection rates between patients operated on for acute and those operated on for chronic cholecystitis. The presence of bacteria in the bile did not seem to influence the wound infection rate in either group. CONCLUSIONS: Early cholecystectomy and appropriate antibiotic prophylaxis result in an acceptably low wound infection rate, and the growth of bacteria from bile is not predictive of the development of wound infection.

Oral lesions in haemopathic patients.

This paper deals with the oral manifestations observed in patients with haemolymphopathies and oral complications associated with systemic cytotoxic chemotherapy and/or radiation therapy. We report the percentage of oral manifestations observed in 30 patients treated with cytotoxic drugs for malignant haemolymphopathies or other tumors after 2-4 cycles of systemic combinations chemotherapy. No sex or age differences were recorded except for abnormalities of taste present only in 13% treated patients older than 50. Stomatitis was more frequent in patients with malignant haemolymphopathies than in patient with other neoplastic diseases, probably due to the fact that in the former group neutropenia and thrombocytopenia induced by bone marrow infiltration or by more aggressive combination chemotherapy are most frequent. An oral care protocol for these patients is suggested.

Influence of natural aquatic humic substances on the bioavailability of benzo(a)pyrene to Atlantic salmon.

Uptake and bioconcentration factors of benzo(a)pyrene (BaP) in Atlantic Salmon were determined in water containing natural aquatic humic substances (AHS) and control water with low humic content by measuring rate constants for uptake and depuration. The amounts of partition-like and strongly AHS bound BaP were determined by tube dialysis and cyclohexane extraction, respectively. Uptake and bioconcentration factors (BCF) were observed to decrease in the presence of AHS, the values of the BCFs being 2310 and 1160 for control water and AHS respectively. The rate constant for uptake of AHS bound BaP was found to be 30 percent of that of free dissolved BaP.

[A case of malignant retroperitoneal fibrohistiocytoma].

A case of retroperitoneal malignant fibrous histiocytoma, with particular reference to its clinico-pathologic characteristics, is reported. The primary large lesion, presenting with a cavitary lung metastasis, resulted to be unresectable at surgery because of major abdominal vessels infiltration.

Quantitative determination of Piroxicam and its impurities by HPTLC spectrodensitometry.

A rapid spectrodensitometric method for the determination of Piroxicam and its impurities is described. The procedure involves a HPTLC separation on Silica Gel F254 plates. Quantitative determinations are carried out using a spectrodensitometer at 310 nm. The proposed method is sensitive and the minimum amount detectable of Piroxicam impurities is around 10 ng.

Influence of aquatic humus and pH on the uptake and depuration of cadmium by the Atlantic salmon (Salmo salar L.).

Uptake and release of cadmium by Atlantic salmon at the concentrations near the background values for fresh water have been studied using cadmium labelled with Cd-109. Cumulation constant and biological half-time of release were established for different concentrations of humus and values of pH. The uptake is strongly dependent on the concentration of humus, showing a pronounced maximum at a DOC concentration of 2.7 mg/l. Release of cadmium is relatively slow, the biological half-time exceeds one year in the water without humic substances. Calculated bioconcentration factors range from 130 to several thousands, which may explain the wide differences of bioconcentration factor reported for natural systems. A general equation enabling calculation of bioconcentration factors for various conditions has been derived. An attempt is done to discuss the results from the point of view of physico-chemical forms of cadmium in respective waters. The amount of cadmium adsorbed on the surface of the fish was proved to be insignificant when compared with total concentration of cadmium bioaccumulated in the fish.

Adsorption

Total gastrectomy in the treatment of adenocarcinoma of the cardia. Review of the results in 73 resected patients.

Between January 1, 1971, and December 31, 1978, 73 patients with adenocarcinoma of the cardia were treated by total gastrectomy with distal esophagectomy. The continuity of the alimentary tract was restored by esophagojejunostomy using a Roux-Y procedure in 32 patients and jejunal loop interposition in 41 patients. The early postoperative mortality rate was 18 percent. The overall 5 year survival rate was 26.7 percent, and the 5 year survival rate for stage I and II disease was 91.6 percent and 25 percent, respectively, whereas none of the stage III and IV patients survived more than 4 years. Fifty-eight percent of the patients without nodal involvement lived more than 5 years. The 5 year survival rate was only 9.3 percent in the presence of nodal metastases. Our 5 year survival rates suggest the value of elective total gastrectomy for stage I or II adenocarcinoma of the cardia. Jejunal loop interposition is a time-consuming but functionally superior procedure in comparison to other reconstructive procedures after total gastrectomy.

Adenocarcinoma

Cells with annular nucleus in haemopathic patients.

Cells with annular nucleus (CAN) were observed in the bone marrow of patients with acute myeloid leukaemia (AML), sideroblastic anaemia (SA) and megaloblastic anaemia (MA), and in particular in the bone marrow (7/31) and in the peripheral blood (16/31) of patients with chronic myeloid leukaemia (CML). CAN were seen in only one out of four splenectomized CML patients and the number of CAN in the bone marrow (0.5-1.5%) was smaller than that in peripheral blood (0.5-5%); these data suggest that in CML peripheral blood CAN might be derived from the spleen. CAN were identified as neutrophilic myelocytes, but in a few cases they were characterized also as eosinophilic or monocytic cells. In only one case of CML an agar culture from peripheral blood cells led to the presence of circulating colony forming cells (CFC) capable of generating monocytic and eosinophilic CAN in cultures. Morphocytochemical anomalies were observed in CAN in some cases. Occasionally together with neutrophilic CAN there were neutrophilic metamyelocytes and granulocytes with annular nuclei, suggesting an intrinsic difficulty of CAN to undergo the normal process of segmentation.

Monocytic differentiation of cluster cells in agar culture of normal human bone marrow. A cytochemical investigation.

A cytochemical investigation was carried out on the cells that make up the clusters forming in agar cultures of normal human bone marrow after 3, 4, and 5 days of incubation. All the clusters were found to comprise young monocytes that were PAS, peroxidase, Sudan Black, and alpha-naphthyl and naphthol-ASD-acetate esterase positive. Lysozyme activity, investigated by the cytobacterial test, was absent in all cases. After 3 days of incubation a large number of pairs of lymphomonocytes or monocytes with the same cytochemical characteristics were observed. When a method of double esterase incubation (alpha-naphthyl or naphthol-ASD-acetate and naphthol-ASD-chloroacetate) was performed on the same slide, no cells of the granulocytic line or intermediate cells between these and the monocytes were observed in any of the clusters. These results indicate that there is direct monocytic differentiation of the colony-forming cells cultured. The origins and stages observed in the maturation of the monocytes are discussed.

Agar

Cytochemical study of acute promyelocytic leukaemia.

Five cases of acute promyelocytic leukaemia (A.P.L.) are investigated for peroxidase, PAS, toluidine blue, astra blue, alpha-naphthyl esterase, double esterase incubation (naphthol-AS plus naphthol-AS-D chloroesterase), and cellular lysozyme activity. These cytochemical investigations may contribute further characterization of the morphologic type.
