The prevalence of immediate positive skin tests to Dermatophagoides pteronyssinus and grass pollen in schoolchildren.

The prevalence of immediate positive reactions to prick testing with house dust mite (Dermatophagoides pteronyssinus) and grass pollen allergens was determined in a random sample of 303 children aged between 8 and 14 years from two Southhampton schools. One hundred and two (33-7%) showed positive reactions, fifty to both D. pteronyssinus and grass pollen, thirty to D. pternyssinus only and twenty-two to pollen only. Allergic symptoms were present in fifty-one of the 102 children with positive skin tests; amongst thirty children with D. pteronyssinus skin sensitivity by itself, only six had sumptoms suggestive of allergic disorder. Significantly more children with positive than with negative tests were reported by their parents to have suffered from recurrent bronchitis during early childhood.

Glycoproteins secreted by sublingual glands of the echidna (Tachyglossus aculeatus).

The portion of echnida sublingual glycoproteins which could be solubilized was fractionated by electrophoresis in polyacrylamide gel and by filtration on a column of Sephadex G-100. Analyses for carbohydrate and amino acid are given and are compared with those of the submaxillary mucins of cattle, dog, pig and sheep. The molar ratio of galactosamine to sialic acid was roughly 1:1 in the echidna fractions, the sialic acid was N-acetyl-neuraminic acid, and there was very little neutral sugar. Proline was relatively high in amount, and isoleucine, half-cystine, methionine, leucine, tyrosine, and phenylalanine were absent or present to a very small extent. The material was weakly antigenic in the rabbit.

The impact of nursing grade on the quality and outcome of nursing care.

The large industry which has grown up around the estimation of nursing requirements for a ward or for a hospital takes little account of variations in nursing skill; meanwhile nursing researchers tend to concentrate on the appropriate organisation of the nursing process to deliver best quality care. This paper, drawing on a Department of Health funded study, analyses the relation between skill mix of a group of nurses and the quality of care provided. Detailed data was collected on 15 wards at 7 sites on both the quality and outcome of care delivered by nurses of different grades, which allowed for analysis at several levels from a specific nurse-patient interaction to the shift sessions. The analysis shows a strong grade effect at the lowest level which is 'diluted' at each succeeding level of aggregation; there is also a strong ward effect at each of the lower levels of aggregation. The conclusion is simple; you pay for quality care.

Tolerance in gambling: an objective measure using the psychophysiological analysis of male fruit machine gamblers.

"Excitement" has often been referred to as the gambler's drug although until recently there was little evidence to substantiate such claims. This study involved the systematic monitoring of the psychophysiology of fruit machine gambling using heart rate measures in 30 adolescent male gamblers. The study was designed to test heart rate differences between regular and nonregular fruit machine gamblers (i.e., between subjects) and differences against the players' own baseline rates (i.e., within subjects). Results showed that there were no heart rate differences between regular and nonregular gamblers although during gambling, both groups' heart rates increased by approximately 22 beats per minute. It was also found that nonregular gamblers' heart rates did not decrease significantly after gambling whereas regular gamblers did. This finding could be argued as the first study to demonstrate an objective measure of gambling tolerance.

A new method for locating saliva stains-spotty paper for spotting spit.

A convenient method of locating saliva stains has been devised. Filter paper covered with blue coloured starch fragments is laid on the material to be tested. Salivary amylase hydrolyses the starch on the areas of paper corresponding to saliva stains, which are then delineated as clear zones whereas negative areas remain speckled with blue particles.

Survival of Apligraf in acute human wounds.

Apligraf consists of bovine collagen dermis seeded with allogeneic male fibroblasts and keratinocytes. It is been shown to promote healing, but the length of persistence and pathological features have not been characterized previously in acute wounds. Forty-eight deep dermal wounds were created and Apligraf, a split-skin graft (SSG), or a dressing was applied. Biopsies of wounds were taken for immunohistochemical analysis and polymerase chain reaction was performed to detect the Y chromosome from Apligraf cells in 14 female wounds. Male allogeneic DNA was detected in wounds for the first 4 weeks. All subsequent time points were negative apart from one biopsy at 6 weeks. The wounds took 4-9 weeks to heal, with the Apligraf exhibiting no features of engraftment. This was in contrast to the rapid healing seen in the SSG control group. Histology revealed a more intense cellular infiltrate, but less vascularization below Apligraf compared with controls. Evidence of an epidermal-mesenchymal interaction was observed. This is the first article to elucidate the survival of Apligraf allogeneic cells in acute wounds in immunocompetent human subjects for up to 6 weeks and demonstrates that in the management of acute surgical wounds, Apligraf has a role only as a temporary biological dressing.
