Characteristics of the antihistamine effect of TAK-427, a novel imidazopyridazine derivative.

OBJECTIVE AND DESIGN: The characteristics of the antihistamine effect of the new antiallergic compound TAK-427 were investigated. MATERIALS AND METHODS: In vitro binding assay of [(3)H] pyrilamine was performed using recombinant human histamine H(1) receptors (rhH(1)R). In vivo studies were performed in male ICR mice or Hartley guinea pigs. Drugs were administered orally 1 h before examinations. Determinations were made of histamine-induced skin reaction, ex vivo measured radioligand binding to brain and lung H(1) receptors, pentobarbital-induced sleeping time, passive cutaneous anaphylaxis (PCA) reaction, and antigen-induced itch-scratch responses (ISRs). RESULTS: TAK-427 inhibited ligand binding to rhH(1)R with an IC(50) value of 17.3 nmol/l. TAK-427 inhibited histamine-induced skin reactions in guinea pigs and mice with an ID(50) value of 0.884 and 0.450 mg/kg, p.o., respectively; significant inhibition associated with 10 mg/kg of TAK-427 was still observed 24 h after dosing in guinea pigs. TAK-427 showed as high selectivity for peripheral H(1) receptors as terfenadine and epinastine did, which was evaluated by ex vivo measured radioligand binding. Even at 300 mg/kg, TAK-427 did not affect pentobarbital-induced sleeping time in mice. TAK-427 significantly inhibited PCA in mice and guinea pigs, and also inhibited antigen-induced ISRs in guinea pigs. CONCLUSIONS: These results suggest that TAK-427 may have a long-lasting antihistamine activity with minimum sedative side effect and suppress acute phase allergic reactions.

[Activation of the complement system and cytokine production by radiographic contrast media in vascular endothelial cells in vitro].

The direct effect of four different radiographic contrast media (RCM) on the release of C3a and C5a and the production of IL-1 alpha and TNF-alpha from vascular endothelial cells was examined in vitro. The test RCM were as follows: diatrizoate (ionic monomer), iopamidol (nonionic monomer), ioxaglate (ionic dimer), and iotrolan (nonionic dimer). These were added to serum-free medium and adjusted to a final concentration of 1% (2.8 mg Iodine/ml). Human microvascular endothelial cells were stimulated by serum-free medium containing the test RCM for eight hours. After incubation, the media were aspirated and assayed for the concentrations of C3a, C5a, IL-1 alpha and TNF-alpha. Finally, the cells were harvested by trypsin, and their viability was determined by the dye-exclusion method. Diatrizoate and iotrolan had higher C3a release than the control (p < 0.05). No increase in C5a, IL-1 alpha or TNF-alpha levels was observed with any of the tested RCM, and there was no significant difference in cell viability with any of the tested RCM. The results of this study suggest that diatrizoate and iotrolan activated the complement system through the alternative pathway by directly stimulating vascular endothelial cells. These observations suggest that a direct effect of RCM on vascular endothelium might play a role in the pathogenesis of local drug eruptions due to RCM.

MRI in carcinomatous encephalitis.

We report a rare case of miliary brain metastases presenting with symptoms similar to encephalitis ("carcinomatous encephalitis"). Contrast-enhanced MRI demonstrated miliary metastases more distinctly than other imaging methods and reproduced the pathological features.

Superselective embolization for bleeding from duodenal diverticulum: a case report.

Duodenal diverticulum is a well-known pathological entity. The majority of patients with this condition are asymptomatic. Although hemorrhage has been described, it is an infrequent complication. We report a patient who presented with massive upper gastrointestinal bleeding with hypovolemic shock, originating from a duodenal diverticulum. The diagnosis was made by emergency angiography. Superselective arterial embolization was performed with a successful outcome. To the best of our knowledge, superselective embolization for hemorrhage originating from a duodenal diverticulum has not previously been described in the literature.
