Stromal cellular response in breast tumours and allied lesions.

Histopathological study for stromal cellular response was made on 200 cases of benign and dysplastic lesions and malignancy of breast. Infiltrations with lymphocytes and plasma cells were observed in 64% cases of fibroadenoma, 66.6% cases of giant fibroadenoma, 61% cases of mammary dysplasia and 33.3% cases of gynaecomastia. While mast cell infiltration was not observed in giant fibroadenoma, its presence was observed in 92% cases of fibroadenoma, 77.8% cases of mammary dysplasia and 33.3% cases of gynaecomastia. Lymphocytes and plasma cells infiltrations were observed in 100% of cases of invasive duct carcinoma, medullary carcinoma and fibrosarcoma. Comedocarcinoma showed infiltration with these cells in 75% of cases while that for colloid carcinoma and Paget's disease of breast were observed in 50% of cases of each. Infiltrations with mast cells were observed in 92.3% cases of invasive duct carcinoma, 75% cases of comedocarcinoma and 25% cases of medullary carcinoma. Other malignant conditions of breast did not show mast cell infiltration. Degree of infiltration with lymphocytes and plasma cells was mainly of low grade in benign and dysplastic lesions compared to high degree of infiltration in malignancy. Mast cell infiltration was of low degree in both the types of lesions.

Clinical profile of 100 cases of liver abscess.

One hundred patients with liver abscess were studied for clinical features and complications. They were diagnosed by radiography, ultrasonography, serology and by needle aspiration. A variety of interesting clinical, haematological and ultrasonographic findings were observed. Literature on liver abscess was reviewed and results compared.

Serodiagnosis of amoebic liver abscess by IHA method.

Serological test based on IHA (Indirect Haemagglutination Method) was performed in 100 cases of hepatic abscess. The test was 100% sensitive and 94% specific. The cut off point of antibody titer between normal population and patients with invasive amoebiasis was 1:128. Antibody titer in amoebic liver abscess was 1:5242 +/- 2795. A significant (P less than 0.001) correlation was found between total leucocyte count and antibody titer.

Immunotherapy of genital warts with inosine pranobex and conventional treatment: double blind placebo controlled study.

In a multicentre, prospective, randomised, placebo controlled study of 55 patients with histories of genital warts for at least one year, a four week course of inosine pranobex 3 g a day improved the clinical response to conventional treatment (primarily podophyllin or trichloroacetic (now called trichlorethanoic) acid). Although more patients given inosine pranobex improved than those given placebo, the difference in general response between the two groups was not significant. When other variables (numbers of warts and extent of lesions) were considered, however, the patients given inosine pranobex fared significantly better. These results suggest that inosine pranobex may be worth considering as adjunct to treatment of patients with refractory genital warts.

Seasons of birth and subtypes of schizophrenia.

Many studies show a higher rate of birth of schizophrenic patients during the winter and early spring months. This is particularly prominent in studies involving northern European countries and the northern part of the United States. The interpretations for this apparent seasonality remain highly speculative. We examined the seasons of birth of 472 patients carrying the diagnosis of schizophrenia. We also studied the gender and paranoid vs nonparanoid subtypes of this subject group. The total group does not show an excess of winter births. The same is true for total male (n = 193) and total female (n = 279) schizophrenic patients. The different subtypes of female patients show a homogenous distribution by seasons of birth; but the male paranoid schizophrenia group (n = 102) shows a significant increase of births during the first quarter of the year. The findings are interpreted according to the prevailing hypotheses of schizophrenia. The implications of these findings are discussed.

High risk suicide factors across cultures.

High risk suicide factors both in India and in the United States are examined. In India these are: humiliation, reputational and economic loss, poverty, examination failure, relationship disappointments, disputes with spouse and inlaws, property disputes, loss of loved one, and chronic medical illnesses. In the United States major depressions, schizophrenia, alcoholism, recent loss(es), retirement, limited social support system, age over 45 and high risk low rescue situations are significant. Most of high risk suicide factors in India apparently are related to interpersonal and socioeconomic causes, whereas in the United States these relate to individual and psychiatric causes. These interesting differences are discussed.

Analysis of genetic interrelationship among HLA-associated diseases.

We have developed a method to study the genetic relationship between any two HLA-associated diseases. We have considered the following hypotheses: (1) both diseases are caused by a common allele; (2) different alleles at the same locus predispose to the two diseases; (3) one disease is predisposed by two alleles, one of which can also lead to the second disease; and (4) different HLA-linked loci are involved in the etiology of each disease. For each hypothesis, we have derived the expected HLA haplotype-sharing distribution in sib pairs who are affected with two diseases. The comparison of the expectations indicate that, in many cases, the alternate hypotheses can be distinguished, if the sample size is appropriately large. The knowledge of the mode of inheritance of each disease is not usually necessary; however, it can greatly increase the power of the test. Analyses of data on pairwise combinations of rheumatoid arthritis (RA), autoimmune thyroid disease (ATD), and insulin-dependent (type I) diabetes mellitus (IDDM) suggest that (a) IDDM is predisposed by two HLA-linked alleles, one of which also predisposes to ATD, (b) one of the IDDM alleles also confers susceptibility to RA, and (c) although the HLA-linked susceptibilities to RA and ATD appear to be primarily due to distinct alleles, the ATD allele may also have a minor role in predisposition to RA.

Coexistence of Capgras and its related syndromes in a single patient.

A very unusual case of Capgras syndrome is described. It is unique because of the coexistence of three related syndromes, namely the syndrome of Fregoli, the syndrome of intermetamorphosis, and the syndrome of subjective doubles, in the same patient during the course of his illness. A brief description of these colourful syndromes is given and we speculate upon the possible role of psychodynamic and organic factors in the patho-physiology of these intriguing syndromes.

Suicide risk: evaluation and management.

Inadequate assessment of the suicidal patient can have tragic consequences. Reliance should be placed on a comprehensive evaluation, including social profile, mental and physical status, suicidal ideation, intent and behavior, and current social support systems. Heightened awareness, combined with a knowledge about risk factors for suicide and intervention techniques, will help prevent these tragedies.

Crisis Intervention

A controlled study of mezlocillin in uncomplicated acute gonorrhoea.

In a randomized trial, 149 patients with uncomplicated gonorrhoea were treated with a single intramuscular dose of 1.0 g mezlocillin, 150 with 2.0 g ampicillin plus 1.0 g probenecid orally and 150 with a single dose of 1.8 mega units of procaine penicillin. Cure rates at 3, 10 and 17 days after treatment were 97%, 99% and 96% for mezlocillin, 95%, 99% and 95% for ampicillin, and 97%, 98% and 98% for procaine penicillin, respectively. The incidence of post-gonococcal urethritis was 11.3% for ampicillin plus probenecid, 10.9% for procaine penicillin and 10.5% for mezlocillin. Side-effects after all three regimens were minimal.

Mezlocillin in gonorrhoea: a pilot study.

A pilot study was carried out in 50 patients with uncomplicated, untreated gonorrhoea to asses the effectiveness of mezlocillin, a new semi-synthetic penicillin. Patients received a single intravenous injection of 2 g mezlocillin and were followed-up for periods up to 101 days. Four patients defaulted immediately. Clinical and bacteriological findings showed that only 1 of the 46 patients followed-up had failed to respond to treatment, indicating a cure rate of 97.8%. Five (10.8%) male patients had developed post-gonococcal urethritis when seen 3 weeks or later in the follow-up period.
