[Hand injuries due to agricultural accidents (author's transl)].

Agricultural injuries are common in childhood. About a quarter of the cases shows injuries of the hand. These are sometimes very extensive and, particularly in younger children, open fractures or amputation of one or more fingers are often found. Machines for peeling maize-cobs, cutting and mixing fodder, and tractors are those implements which cause the commonest serious injuries to the hand. The high percentage of children under eight years of age among our cases clearly indicates the small account of safety measures held among country people. For successful accident-prevention it is therefore necessary to increase the consciousness of danger to children among the farming population by means of detailed information, as well as to carry out improvements on the machines themselves.

[Tetanus antibody determinations in children (author's transl)].

Tetanus antibody determinations in 109 children aged 1 1/2 to 15 1/2 years showed that 98.2% had a sufficiently protective tetanus antitoxin titre. There were no significant differences between completely and incompletely first immunised children. Increased titres of up to 25 IU/ml serum were found shortly after multiple boostering. According to the observed titres boostering after trauma is at the earliest necessary after 5 years in complete or incomplete primary immunisation or after the last boostering. Passive immunisation is only indicated in the traumatised non-immunised and in patients with wounds of larger standing who have not been immunised for more than 5 years.

[Anorectal manometry in childhood (author's transl)].

Anorectal manometric studies with open-ended and water-filled tubes are useful in the diagnostic of many anorectal diseases. The electro-manometric investigation is together with X-ray and histologic examinations an important method in the diagnosis of Morbus Hirschsprung, especially in cases with short segment. Moreover a manometric differentiation of other forms of constipation is possible.

[Bicycle spoke injuries in children (author's transl)].

140 typical bicycle spoke injuries in children were seen during two years in the Pediatric Surgical Hospital of the University of Graz (Austria). 15% of the patients had bone fractures. In order to prevent such accidents satisfactory child seats, special foot supports and protective canvas over the upper half of the wheel are necessary.

[Persistence of the oviduct (pseudohermaphroditismus masculinus internus)].

Internal male pseudohermaphroditism is a rare malformation. Its reason is the lack of the Müllerian inhibiting substance produced by the fetal Sertoli cells. Therefore, additional female genital organs exist in a normal male. Genetic determination seems to be possible. Early correction of cryptorch testes is the therapy of choice and promises fertility for some patients.

[Winter sport injuries in childhood (author's transl)].

3374 accidents occurring on the field of sport during the years 1975--1977 accounted for 19% of all accidents dealt with at the Institute of Kinderchirurgie in Graz. 51% of the accidents were caused by the typical winter sports: skiing, tobogganing, ice-skating and ski-jumping with skiing accounting for 75% of the accidents. The fracture localization typical of the different kinds of winter sport is dealt with in detail. The correct size and safety of the equipment were found to be particularly important in the prevention of such accidents in childhood.

[Bone scan of acute hematogenous osteomyelitis in childhood (author's transl)].

A variety of 99m-Tc-phosphate compounds are sensitive and reliable for scintigraphic diagnosis of bone diseases. Therefore bone scan has become a safe method for evaluating patients with suspected acute hematogenous osteomyelitis. So we were able to perform early and adequate treatment without evidence of bony changes in the radiograph. Quantitative regional measurements with a relative uptake ratio was found to be a clinically useful parameter in the follow-up of osteomyelitis.

[Cinefluorographic studies in children with previous esophageal atresia repair (author's transl)].

Seven children with previous esophageal Atresia repair underwent cinefluorographic studies of the swalling act. They all showed disordered motor function in the esophageal segment below the Anastomosis with segmental contractions and lack of primary Peristalsis. Nevertheless we found organized propulsive waves below the Anastomosis too, which leaded to an esophageal emptying also in the supine position.

[Conservative therapy of post-traumatic splenic rupture].

Our experiences with conservative treatment of traumatic rupture of the spleen in children are described. Laparoscopy is an important diagnostic aid, which also influences the indication for the conservative procedure. An overwhelming, mostly lethal sepsis arises in a small percentage of splenectomized patients. In children, therefore, we favor preserving the traumatized spleen, even if ruptured.

[Systemic and biochemical manifestations of hepatoblastomas (author's transl)].

Among 220 children with malignant solid tumors diagnosed at the University Children Hospital and Department of Pediatric Surgery in Graz six (1,8%) hepatoblastomas were found. All patients were boys. Systemic manifestations may precede the discovery of the abdominal mass and may be important for diagnosis and follow-up.

[Traumatic abdominal-wall hernia in childhood].

In recent years an increased occurrence of posttraumatic abdominal-wall hernias, i. e., subcutaneous abdominal ruptures, has been observed in children. The causal trauma is a fall on a scooter or bicycle handlebar. The predilectional place of injury lies near the linea alba or lateral of the rectus sheath. After several days of observation in order to exclude the possibility of an additional intra-abdominal injury, the hernial gap is operatively closed. An important safety measure would be remodelling bicycle bars.

[Infusion and nutrition problems in pediatric surgery].

The considerable differences in the body composition of infants, children and adults is responsible for special problems of infusion therapy in various age groups. Because of the narrow range of tolerance in the neonatal period special knowledge and skill is required to manage postoperative fluid therapy. Subsequently the difficulties of feeding in pediatric surgery are discussed with special reference to gastroschisis, partial resection of the small intestine, short bowel syndrome and Hirschsprung's disease. Nowadays new balanced diets and progress in parenteral therapy have improved the outcome of this seriously endangered group of patients.

[Gastrostomy in paediatric surgery (author's transl)].

Gastrostomy has the same field of application as the feeding tube which is introduced either orally or nasally. With careful nursing and surgical technique, only few and slight disturbances are to be expected. This method, however, ist not suited for routine application by the premature baby because of its possible septic complications.
