[Human diploid cell (HDC) rabies vaccine immunoprophylaxis in persons at risk (author's transl)].

Twenty persons working in the institute testing rabies vaccines and sera were vaccinated three times at monthly intervals with HDC rabies vaccine. In 9 persons in whom the basic immunisation was completed more than two years previously a booster dose was given. The antibodies formed after vaccination were measured in international units (IU). Serum antibody levels rose constantly during the basic immunisation period. Four weeks after the second vaccination definite sero-conversion could be demonstrated in all persons, the mean antibody level was 6.05 IU/ml. The antibody level rose after the third vaccination to a mean of 10.7 IU/ml and sank over the following six months to 1.5 IU/ml. Two years later the mean level was 0.5 IU/ml. After boostering the antibody level rose to 29.6 IU/ml within a week and after two weeks reached one hundred fold the original level. In view of these results which agree with other studies reexamination of the present vaccination scheme for prophylactic rabies vaccination is recommended.

[Randomised trial of acupuncture compared with conventional massage and "sham" laser acupuncture for treatment of chronic neck pain - range of motion analysis].

AIM: The aim of this study was to compare the effects of acupuncture on active motion of the cervical spine in patients with chronic neck pain with those of "sham" laser acupuncture and massage. MATERIAL AND METHODS: 177 patients with chronic neck pain were included in this prospective, randomized, placebo-controlled study. The patients were allocated by external randomization to five treatments over three weeks with acupuncture, massage and "sham" laser acupuncture. The range of active motion was measured by means of a 3D ultrasound real time motion analyzer. RESULTS: The analysis of cervical motion in three directions showed the largest increase in range of motion 14 days after acupuncture. Compared to massage, a significant improvement in total range of motion was seen in those patients treated by acupuncture immediately (p = 0,03) and one week (p = 0,03) weeks after therapy. There was no significant difference in those patients treated by sham laser acupuncture. CONCLUSION: The results of the study indicate that acupuncture is superior to conventional massage for improving active range of motion in patients with chronic neck pain. Because of its positive effects, its acceptance among patients and the lack of severe side effects, acupuncture can be recommended for the treatment of chronic neck pain, although there was no significant difference in results between "sham" laser acupuncture and acupuncture.

[Is use of the Knee Society Roentgenographic Evaluation and Scoring System for radiologic control of knee prostheses reliable?].

PURPOSE: To follow the radiographic parameters of prosthetic loosening of the Knee Society Roentgenographic Evaluation and Scoring System (KSRESS) and to test their clinical relevance. METHODS: This prospective study documents the use of the KSRESS and clinical rating system in 333 primary posterior cruciate retaining PFC total knee arthroplasties preoperatively, at 3 month and at yearly intervals postoperatively. The average follow-up period was 3.2 years (range 2-5.5 y). RESULTS: The average component position and alignment angles remained unchanged at biomechanically correct angles during the follow-up period. Radiolucent lines were observed regularly at the edges of the tibial and femoral components. The average total radiolucent lines score decreased for the tibial component. Patellar and femoral lucencies remained unchanged. The total radiolucent lines scores did not correlate with postoperative pain, body-mass-index, alignment, stability, knee and function score. Four revisions, one with relevant radiolucent lines, had to be performed. CONCLUSIONS: The KSRESS is a useful tool. Standardisation of roentgenograms is mandatory. The PFC arthroplasty, the operative technique and the instruments ensure a biomechanically correct implantation.

New model for the induction of osteoblastic bone metastases in rat.

BACKGROUND: We have investigated different models for osteoblastic lesions. Currently, there are two models using MatLyLu R-3327prostate cancer cells: tumor cell application in the left heart ventricle and intravenous application with concomitant transient surgical clamping of the lower caval vein. MATERIALS AND METHODS: Thirty male Copenhagen rats (age 9+/-2 months, mean weight 323+/-21 g) were each injected with 200,000 R-3327 prostate cancer cells. In 10 rats the left ventricle route was used (group 1), in an other 10 rats the intravenous route (group 2), while in the third group of 10 rats a new model of a direct intra-osseous route was applied (group 3). Additionally, a control group of 5 rats underwent the same procedure as in group 3, but only saline without tumor cells was administered. A 99mTc-HMDP bone scan and histological examination of bone and lung were performed for follow-up. RESULTS: In the bone scan, bone lesions could be not visualized in groups 1 and 2, but in group 3 osteoblastic lesions were observed in both femora in 9 out of 10 rats. Upon histological examination, there were lung metastases in animals from groups 1 and 2, but not in group 3. Clinical signs for bone metastases in the lumbar spine (motor disablement of the hind legs) were found in groups 1 and 2. CONCLUSION: The intra-osseous administration of MatLyLu R-3327 prostate cancer cells represents a useful and effective model for osteoblastic bone lesion, and allows further autoradiographic evaluation of bone uptake using bone-seeking radiopharmaceuticals.

[Contribution of immunoglobulin assays to the diagnosis of human African trypanosomiasis in patients immunologically suspect (immunofluorescence and ELISA). A preliminary report (author's transl)].

Can the diagnosis of sleeping sickness in patients immunologically suspect, without seeing the parasite, be obtained by dosing IgG and IgM? The authors demonstrate that two groups of people with positive immunodiagnosis, one parasitologically confirmed and the other not confirmed, do not show significant difference when dosing immunoglobulins. These two groups respectively show a significant difference when compared with a parasitologically and immunologicaly negative group. The simultaneous increase in IgG and IgM might confirm the immunodiagnosis.

[Previous experience with subcapital femoral neck osteotomy in severe cases of epiphysiolysis capitis femoris].

In severe cases of epiphyseolysis capitis femoris the high-grade dislocation of the epiphysis near the deformation is corrected by subcapital femoral neck osteotomy. In the period from 1976-1986 the subcapital femoral neck osteotomy was carried out ten times at the Department for Orthopaedic Surgery of Martin-Luther-University Halle. Our own operative procedures as well as the results achieved are represented.
