[Relation of the static urethral pressure profile and symptoms of urinary urgency in the female].

In a retrospective study of 463 stationary urethral-pressure profiles the coherence between clinical relevant changes of the urethral pressure and bladder instability is discussed. It is shown, that clinical important pressure changes in the stationary urethral-pressure profile appear significantly more frequently in female patients with signs of bladder instability. The results and the definition of urethral-pressure changes are compared with the literature.

[Urogynecologic findings in patients following abdominal radical operation for cervix cancer].

Two collectives of patients of the I. Gynecological-obstetrical clinic Praha/CSSR suffering from cervical cancer are examined after abdominal radical hysterectomy Wertheim-Meigs. Group 1 contained 125 women treated in the years 1957 to 1966, group 2 included 89 women undergoing the operation in the years 1981 to 1986. In group 2 we found a lower rate of perioperative injuries and early and late disturbances of the urinary tract following radical hysterectomy. This may be a result of more careful preparation of the ureter and intensive postoperative care. We examined 56 women of group 2 by urodynamics. There are statistical significant changes of the maximum urethral pressure and bladder compliance after the operation. This values returned to normal limits with increasing distance between operation and follow-up.

[Urethral injury in surgery for urinary incontinence].

In this paper we demonstrate a case history of a 26 year old woman with an urethral injury. Primarily she suffered from urinary incontinence. Colporrhaphia anterior and vesicopexia sec. BALL were performed. Urethral injury occurred after this operation while postoperative catheterization was performed. Diagnostic and therapeutic management of this complication are reported. Pre- and postoperative urodynamical and urethrocystographical details are described, too.

[Treatment of urinary urgency symptoms in the female with the beta-2-sympathomimetic clenbuterol].

Aside from parasympatheticolytis, calcium antagonists, musculotropic relaxants and tricyclic antidepressants, beta 2-sympatheticomimetics are also used in the treatment of urgency. The beta 2-sympatheticomimetic Clenbuterol, which has relatively slight cardiological side effects, was administered per os to 30 women. Prior to and six weeks after the commencement of daily medication with 2 x 20 micrograms of Clenbuterol, the therapeutic effect of the drug was tested by means of a urogynecological examination (history, gynecological findings, cystometrography, urethrocystometrography, uroflowmetry and pretherapeutic urethrocystometrography). An improvement rate of 65% was found. The parameters of urethrocystometrography and uroflowmetry were not altered under medication with Clenbuterol. Cystometrographic parameters, on the other hand, improved (minimal desire to urinate, maximal desire to urinate, compliance). The results of this study are discussed with reference to the literature. For the beta 2-sympatheticomimetic Clenbuterol, a good level of efficacy can be established in women patients on whom parasympatheticolytics alone failed to have a satisfactory effect.

[Personal experiences with bladder training in the treatment of urge incontinence in the female].

Anamnestic and urodynamical results are evaluated before and after bladder retraining drill (BRD) in women suffering from urge incontinence. A continence rate of 74% was determined 6 to 24 months after BRD. This agrees with dates reported by literature. Urodynamically we obtained an improvement of cystometric parameters (even in parts statistically significant). The values of the urethrocystographic angles and the urinary flow rate have been not changed. We could state that the BRD is a good possibility to realize multistep-therapy of female incontinence.

[Primary vaginal carcinoma - analysis of 100 cases (author's transl)].

The importance of prevention to vaginal carcinoma was underlined by results obtained from a retrospective analysis of oncological patients who had been hospitalised at the First Gynaecological Hospital of Prague, between 1946 and 1970. While five-year healing of 41 per cent compared well to reports published elsewhere, the authors feel that the prognosis of this rare tumour might be further improved by earlier detection and treatment.
