Co-amplification of MYCN and a DEAD box gene (DDX1) in primary neuroblastoma.

DEAD box proteins are putative RNA helicases that have been implicated in cellular processes involving alteration of RNA secondary structure, such as translation initiation and splicing. These proteins share eight conserved amino acid motifs, including Asp(D)-Glu-(E)-Ala(A)-Asp(D) which is part of a more extended motif. Recently, we have shown that the novel DDX1 gene containing a DEAD box motif maps to the same chromosome band as MYCN at 2p24 and is co-amplified with MYCN in retinoblastoma cell lines. Here, we show that the DDX1 gene is co-amplified with the MYCN gene in 2 of three neuroblastoma cell lines and that DDX1 RNA levels correlate with DDX1 gene copy number. Since amplification of MYCN is an indicator of poor prognosis in neuroblastoma, it was of interest to determine whether co-amplification with DDX1 occurred in clinical samples of neuroblastoma and whether such a finding carried any additional prognostic significance. We determined the gene copy number of DDX1 in 32 neuroblastoma patient samples (representative of all stages): 13 were MYCN amplified and 19 had normal copy numbers of the MYCN gene. Of the 13 neuroblastomas that were MYCN amplified, seven were also DDX1 amplified. Of the 19 that were not MYCN amplified, none were DDX1 amplified. This is the first example of a gene that is co-amplified with MYCN at a high frequency in neuroblastoma. While there was a trend towards a worse clinical outcome with co-amplification, the numbers were too small to reach significance.

A trial to assess the effectiveness of bowel preparation prior to intravenous urography.

A randomized prospective trial was undertaken to compare the relative effectiveness of two commonly used bowel preparations (senna tablets and sodium picosulphate powder) administered to patients before they underwent out-patient intravenous urography. Their 'control' films were compared with plain 'kidneys, ureters, and bladder' (KUB) radiographs of patients who had had no bowel preparation. The results show no significant difference in the degree of faecal shadowing between those receiving a bowel preparation and the unprepared patients. Nor is there any difference between the two laxatives. We conclude that the routine administration of a bowel preparation is unlikely to improve the diagnostic quality of out-patient intravenous urograms. In addition, 40% of the urogram patients found the effects of the laxatives to be unpleasant or very unpleasant.

Depression and chronic fatigue. Indications for psychiatric consultation.

Symptoms of fatigue and the fatigue syndrome itself are familiar complaints heard by nonpsychiatric physicians in the general hospital. The nature of these complaints is often diffuse and frequently conceals atypical psychiatric disorders. In the general hospital setting, consultation psychiatry, which functions as the interface between psychiatry and these medical practitioners, is ideally situated to assist in the assessment and management of these patients. This article reviews the methods of approach that psychiatric consultants can use to assist their nonpsychiatric colleagues in identification and referral for problems involving fatigue. In addition, a brief review of diagnosis, management, and treatment of such patients is given.

Medical clearance: fact or fiction in the hospital emergency room.

Prior medical clearance is an important variable in the psychiatric assessment of the emergency room patient. An analysis of documentation of prior physical examinations on a consecutive series of 137 patients seen in a general hospital emergency room and referred for psychiatric consultation revealed wide variance in physical examinations. The authors discuss the implications of incomplete physical examinations for psychiatry and offer proposals for standardization of this important aspect of patient evaluation.

A database review in C-L psychiatry: characteristics of hospitalized suicide attempters.

A computerized database was used to study the characteristics of psychiatric consultations in patients admitted to nonpsychiatric services. The authors reviewed a consultation practice with suicide attempters over a four-year period and compared the patients with a patient sample matched for gender and age also seen by the service. For suicide attempters, the time elapsed between admission to the service and the consultation, as well as the lengths of stay, were significantly shorter. Suicide attempters were significantly more likely to have mood disorders and Axis II disorders. The authors review the role of consultation psychiatrists with these patients and suggest that a triage model fits the services provided and the needs of nonpsychiatric physicians.

Pathological effects of pellagra on the esophagus.

This study determined the pathological effects of niacin/tryptophan deficiency on the esophagus. Nine patients with severe clinical pellagra and 31 age- and sex-matched controls were assessed. All pellagrins had an esophagitis varying from severe to mild. The esophagitis improved in five patients following six to seven days of vitamin therapy. The relationship between vitamin deficiencies and esophageal cancer is discussed.

Diagnostic characteristics and dispositions in suicidal hospitalized medical and surgical patients.

Suicidal ideation is frequently identified in psychiatric consultation patients, but the outcomes of patients with suicidal ideation are infrequently discussed. We reviewed 4 years of consultations and identified two groups of suicidal patients, those admitted following suicide attempts and those with medical illnesses. These groups and the remaining nonsuicidal patients were analyzed for lengths of stay, diagnoses, and outcomes. Suicidal patients were younger (p less than 0.001), and had earlier consultations (p less than 0.001) and shorter stays (p less than 0.001). There were more affective disorders (p less than 0.001) and Axis II disorders (p less than 0.001) in suicidal patients, but more adjustment disorders (p less than 0.005) in nonsuicidal patients.

The prolonged course of gastrointestinal tuberculosis.

Review of biopsy specimens showed that a patient incorrectly diagnosed 7 years ago as having Crohn's disease actually had histologically proven gastrointestinal tuberculosis of 7 years' duration. It is significant that gastrointestinal tuberculosis and Crohn's disease can mimic each other not only in their clinical, radiologic, and histologic manifestations but also temporally. We discuss the interrelationship between AIDS, tuberculosis, and inflammatory bowel disease.

Rectal manifestations of pellagra.

Twenty (44%) of 45 patients with cutaneous pellagra had diarrhoea. Inflammation was seen on sigmoidoscopy in 42 and was severe in 5 (11%), moderate in 4 (9%) and mild in 33 (73%). Six patients (13%) had perianal excoriation. Rectal biopsies taken in all cases showed extensive ulceration in 1 case, moderate to severe inflammation in 16 (36%) and mild inflammation in 18 (40%). Minimal oedema of the lamina propria was seen in 11 (24%) patients. At 5 to 7 days after starting vitamin treatment in nine cases histological changes had returned to normal in 4, improved in 3 and had not changed in 2. Diarrhoea stopped within a week in all cases.

Establishment of the hematopoietic microenvironment in the marrow of matrix-induced endochondral bone.

Implantation of demineralized diaphyseal bone matrix (DBM) into subcutaneous sites in allogeneic Long-Evans rats results in new endochondral bone formation accompanied by hematopoietic bone marrow differentiation in the newly formed ossicles. In the present study, we investigated the relationship between the time of appearance of hematopoietic stem cells (CFU-S) and those of certain postulated elements of the hematopoietic microenvironment, i.e., fibroblast colony-forming cells (CFC-F), colony-stimulating factors (CSF), and collagen types. CFU-S were first detected at 16 days' postimplantation in the developing ossicle. Their numbers increased exponentially until day 24, decreased slightly between days 27 and 33, and then slowly increased in number again until day 36. CFC-F were present at day 10, and their numbers increased exponentially until day 15, decreased dramatically until day 24, and then remained constant till day 35. Thus, the transient growth of CFC-F preceded the appearance and growth of CFU-S in the ossicle by two or three days.
