[Bolus of cyclophosphamide and methylprednisolone in uveitis in Behçet's disease. Preliminary results with the use of new criteria of evaluation].

In a prospective open trial, the effect of pulse cyclophosphamide or methylprednisolone in the treatment of Behçet's disease uveitis was studied. The therapeutic effect was evaluated based on two scores: one evolutive, reflecting ocular inflammation and calculated for each eye before and after therapy; and the other, a measure of disease severity, which took into consideration the irreversible damage. Patients who received cyclophosphamide pulse therapy (n = 8) had significantly lower evolutive scores (16.12 +/- 3.95, vs 10.37 +/- 8.53, p < 0.01) than the patients receiving classical methylprednisolone pulse therapy (n = 20), who had no significant changes in their evolutive scores (12.95 +/- 8.55, vs 10.05 +/- 8.56, p > 0.05).

[Hereditary chondrocalcinosis in Tunisia. Apropos of 3 families].

Hereditary chondrocalcinosis was reported in three Tunisian families. The clinical manifestations appeared early in life and extensive radiologic involvement was apparent. The mode of inheritance seemed to be autosomal with incomplete penetrance. In one family HLA typing revealed that all affected subjects had the haplotype A1 B12 DR3.

Cervex-Brush and Cytobrush. Comparison of their ability to sample abnormal cells for cervical smears.

A study was designed to test specimen adequacy and the detection of disease using two new cervical cytologic sampling devices, the Cervex-Brush and Cytobrush plus a modified Ayre spatula. Two clinicians used each device at random. The laboratory was blind to the sampling method, and the results were compared with those from control samples (Ayre spatula with cotton swab or alone). The 567 test samples were compared with 569 age-matched control samples. The Cervex-Brush and Cytobrush plus spatula were comparable in detecting disease, and both were improvements over the control method. Both the Cervex-Brush and Cytobrush plus spatula produced more consistent good smear quality across the age range studied than did the control method, which resulted in a lower percentage of good-quality smears in older women. Use of the Cytobrush plus spatula and the control method produced smear results the quality of which varied considerably with each clinician.

[Budd-Chiari syndrome in children. Apropos of 7 cases].

Seven cases of Budd-Chiari syndrome are reported in children. The mode of onset was fulminant in one case with rapidly lethal liver failure, acute in 5 cases with rapid appearance of hepatomegaly and ascites and insidious in one case, with isolated hepatomegaly. Hepatomegaly, which is a constant sign, was present in the 7 patients. Ascites and collateral venous circulation were present in 6, splenomegaly in 2 and moderate jaundice in one only. Liver function tests, deeply abnormal in the patient with fulminant liver failure, was only slightly abnormal in the 6 others. Diagnosis was corroborated by ultrasonography, cavography, hepatic veins angiography and liver biopsy in 6 patients and by post mortem examination in the 7th. Etiologic investigations did not allow finding the cause of Budd-Chiari syndrome. However, this series can be distinguished by associated total villous atrophy in 3 cases, psoriasis in one, hepatitis B in one, hepatitis A and intestinal giardiasis in one. Portasystemic shunts were performed in 3 patients. One died in the immediate postoperative period, the 2 others are presently in good health with a 5 and 6 1/2 year-follow-up. One patient died rapidly from fulminant liver failure. Another, untreated, died 16 years after the onset of the disease, from an unknown cause. Two patients are lost to follow-up.

Budd-Chiari Syndrome

Hyperphosphatasemia related to three intestinal alkaline phosphatase isoforms: biochemical study.

An unexplained hyperphosphatasemia was noted in a healthy 47-year-old woman. The electrophoretic pattern on agarose gel with and without wheat germ lectin of the serum of this patient showed the presence of three isoforms which have been characterised as being of intestinal origin: their biochemical (neuraminidase, phenylalanine) and thermodenaturation properties are similar to those of intestinal tissue extract. The pI and pH optima of these isoforms agree with an intestinal origin. Our results suggest the existence of different allelozymes of intestinal alkaline phosphatase.

Natural killer cell activity, interferon-gamma and antibodies to herpes viruses in patients with Behçet's disease.

Interferon-gamma (IFN-gamma) titres in 20 patients with active Behçet's disease were examined and compared with those of 20 normal donors. Sera from Behçet's disease patients revealed an IFN-gamma increase but no correlation between IFN level and natural killer (NK) activity. The analysis of lymphocyte subsets by monoclonal antibodies registered an increase of CD8+ T subpopulation and cells co-expressing CD8(+)-Leu7a+ markers. Moreover, a high number of cells expressing CD25+ and HLA-DR+ phenotype has been noted in patients with active Behçet's disease. Serological analysis showed a high level of IgG antibodies to HSV-1. The increase of IFN-gamma titre, the high number of activated T cells and the increasing level of IgG antibodies to HSV-1 are important manifestations during the active stage of Behçet's disease. These findings are discussed in relation to the immunopathogenesis of Behçet's disease.

Cytotoxic T cells against herpes simplex virus in Behçet's disease.

Lymphocytes from 36 patients with Behçet's disease (20 in remission and 16 in active phase) were stimulated in vitro with herpes simplex virus and then tested for their ability to generate cytotoxic T cell responses to the virus. Significant cytotoxic responses were found. CD4+ and CD8+ subpopulations from the patients in remission generated specific cytotoxic activity against autologous target cells. These observations suggested that CD4+ and CD8+ cytotoxic T cells may have an important host response in herpes virus infection in Behçet's disease.

Quantitative morphometric analysis of brain growth using magnetic resonance imaging.

Using a simple image analysis system, we have established normative data obtained from magnetic resonance imaging (MRI) scans for the relative size and growth of the major structural domains of the brain. Midsagittal scans of the head were analyzed in 95 subjects aged 0 to 20 years. Only scans with no demonstrable structural abnormalities of the brain were utilized. Age-related changes in the relative growth of specific structures were calculated by dividing patients into 5-year age increments. No age-related changes were found in the size of the posterior fossa, supratentorial intracranial space, or cisterna magna relative to the total intracranial vault space. A significant (P less than .01) increase in the size of the corpus callosum relative to that of the supratentorial structures and relative to the total intracranial vault space was noted to occur between the 6- to 10-year-old and 11- to 15-year-old age groups. This may reflect an increase in myelination of the corpus callosum that occurs during this period. A small reduction in the ratio of the posterior fossa to the supratentorial space was found between the 0- to 5-year-old and the 6- to 10-year-old age groups, but the biological significance of this difference is uncertain. It appears from these data that quantitative analysis of MRI scans can be used to draw objective conclusions about the relative sizes and growth of the major brain structures. The development of normative data for these structures and the simplicity of the methodology should have many clinical applications in the assessment of aberrant brain development.
