The "temporocele".

This article reports a case of spontaneous extracranial pneumatocele and introduces a new simple terminology for this clinical entity. A 10-year-old Saudi girl presented with left tympanoparietomastoid swelling. It was found to be fluctuant and full of air. On exploration, bony septae were found arising from the skull aponeurosis, the cranial boundary was elevated, and an ivory, cancellous bony swelling was over the root of the zygoma. No naked eye anatomic communication to the mastoid air cell was found. The mastoid pneumatization was within normal limits. All preoperative, operative, and postoperative investigations are presented. The authors introduce temporocele as a new term applied to this extracranial spontaneous pneumatocele.

Local acriflavine in laryngoscleromatous subglottic stenosis.

Ten patients with laryngoscleromatous subglottic stenosis have been treated by local two per cent acriflavine solution by a newly designed technique. Two patients with the fibrotic form of this disease were not cured, but the other eight patients (with the granulomatous form) gained a reasonable airway which enabled them to discard their tracheostomy tubes two months after the start of therapy. Up to the present time, for as long as five months after detubation, no single patient has needed a revision tracheostomy. The overall results of this work encourage a trial of local two per cent acriflavine in cases with granulomatous laryngoscleroma.

Common features and surgical interference in a prevalent oral cancer in Saudi Arabia (a preliminary report).

Oral carcinoma is the third most common malignant neoplasm in the Western region of Saudi Arabia. Its specific pathological features and possible aetiological factors contributing to its high incidence are discussed. The role of 'Shamma', which is a quid used by many local inhabitants, is also discussed. Our experience with the first four cases treated by surgery is presented. Recommendations are made for the early diagnosis and detection of oral carcinoma in this region.

Histopathological cochlear changes induced by antimonial antibilharzial drugs.

Sixteen normal guinea-pigs were injected by two different antimonial antibilharzial drugs (Stibophen NF and Bilharcid EP). Therapeutic doses of either drug produced hydropic degeneration in the hair and supporting cells of the organ of Corti. The pillar cells and the neurones of the spiral ganglion were not affected. Doubling the dosage produced atrophy of the organ of corti and its replacement by granular epithelioid cells. These antibilharzials carry a definite risk to the cochlear auditory elements aggravated by exceeding the therapeutic dosage.

Local acriflavine: a new therapy for rhinoscleroma.

Fifty patients with rhinoscleroma have been studied clinically, histopathologically and bacteriologically before, during and after the local application of acriflavin solution, in different concentrations, over an eight-week period. The results have been encouraging. Local acriflavine solution, in a concentration of 2 per cent, has been shown to be an effective and safe remedy for rhinoscleroma.

Histopathological changes in experimental free thyroid transplants.

Free thyroid autotransplants into new receptor sites have been tried in 28 dogs, by two different techniques. Good but irregular histopathological survival of thyroid acini has been found after free autotransplantation of bisected and everted thyroid lobes into the external genitalia of dogs. The results have been very encouraging and the functional assessment of these implants is essential; such work is already in progress in our laboratory. Our findings and their significance have been discussed.

Effects of tympanic neurectomy on experimentally induced parotid fistulae.

The present work was designed to determine whether or not tympanic neurectomy alone could cure parotid fistulae. In six adult dogs parotid fistulae were induced and the tympanic nerve was transected on the right, while the nerve on the left side was left intact as a control. Complete healing of the fistulae on the right side was noticed, while those on the left side persisted. The findings and their significance are discussed.

Reversible neuropraxic visual loss induced by allergic Aspergillus flavus sinomycosis.

This work reports a patient with visual loss treated successfully with surgical removal of the Aspergillus flavus sinomycosis. Vision was partially reversed within hours after surgery before starting planned corticosteroid therapy. The patient's visual acuity continued to improve steadily until it became equal to that of the other eye. The immediate gain in vision and continued improvement without corticosteroid therapy suggest a new hypothesis for visual loss induced by allergic sinonasal Aspergillosis. Simple mechanical pressure alone of the aspergillus mass over the nerve can produce visual loss, and this loss is reversed by removing the mass without corticosteroid therapy.
