The relationship between the psychological and immunological state in patients with atopic dermatitis.

The relationship between the psychological and immunological state in patients with atopic dermatitis was investigated. It was found that patients with atopic dermatitis are significantly more depressive and scored higher for state anxiety (P < 0.05) than the normal control group. They also tended to be more psychosomatic symptom-prone and to score higher for trait anxiety. NK activity in patients with atopic dermatitis was significantly lower than in normal controls (P < 0.01), while serum IFN-gamma levels tended to be slightly higher and serum IL-4 levels somewhat lower. No significant correlation was detected between any pair of psychological test/immunological examination for either patients or normal controls. However, in the examination of path analyses, psychological variables affect serum levels of IFN-gamma and IL-4 more in the patients than the normal controls. Only STAI(T) and (S) affect NK activity in atopic dermatitis patients. By a discriminant analysis, NK cell activity and serum IFN-gamma appeared statistically significant. We conclude that the psychological state is to some extent related to the immunological state.

Malignant melanoma developing from an intradermal nevus.

A 58-year-old man presented a black nodule with satellites located in the left upper region of the umbilicus. Clinical history and microscopic findings led us to diagnose it as a malignant melanoma developing from an intradermal nevus. Histopathologically, melanoma cells developed upward from the nest of the intradermal nevus and no junctional activity was shown. Cases of malignant melanoma arising from an intradermal nevus have rarely been reported. Moreover, previously reported cases showed melanoma cells beneath the intradermal nevus.

Anxiety, depression and psychosomatic symptoms in patients with atopic dermatitis: comparison with normal controls and among groups of different degrees of severity.

Atopic dermatitis is a common skin disorder. The age distribution is mainly located from infancy to adolescence, which period is the most important for forming character or effecting their psychological condition. We used three types of psychological tests to investigate anxiety, depression and psychosomatic symptoms in 45 patients with atopic dermatitis and 34 normal controls. These tests consisted of the manifest anxiety scale (MAS), self-rating depression scale (SDS) and Cornell Medical Index (CMI). On the MAS, the atopic dermatitis group did not show any statistical difference from normal controls. However, the SDS and the CMI produced statistically significant differences (P < 0.05 and P < 0.01, respectively). Furthermore, we classified their symptom in three degrees of severity (mild: 15; moderate: 14; severe: 16). Patients with mild symptoms did not show any statistical difference from normal controls on the three tests. Patients with moderate symptoms showed a statistically significant difference from normal controls on the SDS (P < 0.01) and the CMI (P < 0.05), but not on the MAS. Finally, patients with severe symptoms (in-patients) produced somewhat lower test scores than those with moderate symptoms. We conclude that the patients with atopic dermatitis were more depressive and psychosomatic symptom-prone than normal controls, and suggest that some patients with atopic dermatitis should be treated both dermatologically and psychiatrically.

A combination therapy of psychotropic drugs and antihistaminics or antiallergics in patients with chronic urticaria.

The efficacy of psychotropic drugs in combination with antihistaminics or antiallergics in patients with chronic urticaria was investigated. Forty patients, who had had urticaria for at least one month and showed insufficient suppression of symptoms for more than one month while being treated with antihistaminics or antiallergics, were divided into two groups on the basis of their score on three kinds of psychological tests: a manifest anxiety test, a self-rating depression scale and the Cornell medical index. One group had a high score, the other a low score. We investigated both groups with or without psychotropics in combination with antihistaminics or antiallergics. Incidence of urticaria and degree of improvement were examined after 2 and 6 months. There was no statistical difference between the groups after 2 months. After 6 months, the members of the low-score group treated with psychotropics showed a little better prognosis than those not treated. However, the high-score group treated with psychotropics showed a statistically significant decrease of incidence and increase in degree of improvement (P < 0.01). In conclusion, it is suggested that a long-term combination therapy with psychotropics is effective in suppressing urticaria for patients who are anxious, depressive and psychosomatic symptom-prone, and moderately effective for those with a low score in these areas.

Psychosomatic treatment of a case of chronic urticaria.

A 35-year-old Japanese woman presented with urticaria in January of 1992. As her symptoms gradually became worse, she came to our hospital in March of that year. I treated her with various combinations of antihistaminics and antiallergics. However, her symptoms did not respond and continued to deteriorate. Although her blood was analyzed in an attempt to identify antigenic or physical factors, no positive data were obtained. Because three kinds of psychological tests showed that the patient was highly anxious and depressive, I additionally treated her with psychotropics and psychotherapy in May of 1994. After a month, the symptoms began to disappear. She has since been free from the symptoms while taking medicine only twice a week. Our group recently presented the efficacy of psychotropics in patients with chronic urticaria. This case suggests that highly anxious or depressive cases with chronic urticaria should be treated not only dermatologically, but also psychologically.

Giant acquired fibrokeratoma of the nail bed.

A 41-year-old Japanese woman presented with a red, elastically hard, hook-shaped hyperkeratotic large tumor on her right third toe, which originated from the nail bed. The final diagnosis was acquired fibrokeratoma. Our case is remarkable, because acquired fibrokeratoma is usually relatively small and does not involve the nails.

A case of dermatofibrosarcoma protuberans on the right first toe.

Reports of dermatofibrosarcoma protuberans on a toe are extremely rare. It frequently occurs on the trunk and extremities. A Japanese woman presented with a dark-brownish hyperkeratotic plaque on the dorsal skin of her first toe. The initial clinical diagnosis of verruca vulgaris prompted treatment with cryotherapy. After that a glossy milky-white tumor appeared. Only the results of the histopathologic examination resulted in a diagnosis of dermatofibrosarcoma protuberans. The unusual macroscopic finding was considered to be due to repetitive stimulation by foot movement.

Anxiety, depression, psychosomatic symptoms and autonomic nervous function in patients with chronic urticaria.

Most reports on the psychological analysis of chronic urticaria have concentrated on psychodynamic theories of causation of the disease. For the present study, we used three kinds of psychological tests as well as electrocardiography to estimate anxiety, depressiveness, psychosomatic symptoms and autonomic nervous functions in 30 outpatients with chronic urticaria and 39 normal controls. For evaluation we used the manifest anxiety scale (MAS), self-rating depression scale (SDS), Cornell medical index (CMI) and convergence of variance of R-R interval (CVR-R). Psychologically positive responses to any one of the tests were seen in 70.0% of the chronic urticaria patients, but in only 25.6% of the controls. These differences all showed statistical significance (P < 0.01). The In(CVR-R) (Y) and age (X) suggested a linear regression, but although the regression slope was steeper for the urticaria group (Y = 2.924-0.027X) than for the controls (Y = 2.702-0.023X), the difference was not statistically significant. These data indicate that patients with chronic urticaria are more anxious, depressive and psychosomatic symptom-prone than normal controls. In conclusion, we suggest that chronic urticaria patients should be diagnosed and treated both dermatologically and psychologically.

Disseminated cutaneous nodules revealing gastric carcinoma.

A 39-year-old Japanese male presented with multiple erythematous nodules on the head, face and trunk. Further investigations revealed a Borrmann type II gastric carcinoma and identified disseminated bone metastases. No other metastases of internal organs, including the liver and lungs, were clinically detected in this stage. This metastatic pattern is quite unusual.

Growth inhibition of human keratinocytes by antisense c-myc oligomer is not coupled to induction of differentiation.

To study the relationship between cell growth and differentiation in human keratinocytes, we examined the effect of the antisense oligomer of c-myc mRNA. This oligomer is stable in culture medium. A 24 h incubation of cells with 5 microM antisense c-myc oligomer resulted in a 48.2% decrease in c-myc protein and inhibited cell growth by 80.7% compared to the sense c-myc oligomer. In contrast, antisense c-myc oligomer had no effect on differentiation when the population of involucrin-positive cells and cornified envelope formation were used as differentiation markers. These results show that antisense c-myc oligomer inhibits cell growth but does not induce differentiation in normal human keratinocytes. Therefore, cell growth and differentiation are not necessarily coupled in these cells.

Stimulation of fibronectin secretion in cultured human keratinocytes by transforming growth factor-beta not by other growth inhibitory substances.

We investigated the effects of various growth inhibitory substances on fibronectin secretion in cultured normal human keratinocytes. Fibronectin secretion in keratinocytes was expressed by two methods: immunofluorescent staining of cells with anti-human fibronectin antibody, and sodium dodecyl-sulphate-polyacrylamide gel electrophoresis of [35S]-labeled proteins secreted by the cells followed by autoradiography. With immunofluorescent staining, the extracellular fibronectin was observed as coarse fibrils only in some cells of the control culture, while cells treated with 20 ng/ml transforming growth factor-beta 1 (TGF-beta 1) showed intense fluorescences in a radiating pattern around most of the cells, indicating that TGF-beta 1 markedly increased fibronectin secretion in keratinocytes. Analysis of [35S]-methionine labeled proteins also revealed that TGF-beta 1 increased fibronectin secretion eight fold in culture medium. TGF-beta 2 has also induced weak increase of fibronectin secretion 1.8 fold in keratinocytes. In contrast, other growth inhibitory substances, such as tumor necrosis factor-alpha, 1,25-dihydroxyvitamin D3 and cyclosporin A, did not show any significant effects on fibronectin secretion. These results indicated that increase of fibronectin secretion is not associated with growth inhibitory effect, but rather with a specific effect of TGF-beta.

Growth-inhibitory effects of 1,25-dihydroxyvitamin D3 on normal human keratinocytes cultured in serum-free medium.

The effect of 1,25-dihydroxyvitamin D3 [1,25(OH)2D3] on the growth of normal human keratinocytes cultured in serum-free medium was investigated. 1,25(OH)2D3 inhibited the cell growth at 10(-7) M by 75.3% and at 10(-6) M almost completely. The growth inhibition was accompanied by changes related to proliferation: (1) remarkable inhibition of DNA synthesis, (2) the decrease in the number of high-affinity receptors for epidermal growth factor, with almost no change in total receptor number, (3) the rapid decrease in c-myc mRNA level. The inhibition of DNA synthesis and the decrease of c-myc mRNA expression occurred at 3 h after the addition of 1,25(OH)2D3. These results suggest that decrease of c-myc mRNA expression is one of the primary effects of 1,25(OH)2D3 in the growth inhibition of human keratinocytes.

Modulation of growth and differentiation in normal human keratinocytes by transforming growth factor-beta.

The effect of transforming growth factor-type beta 1(TGF-beta) on the growth and differentiation of normal human skin keratinocytes cultured in serum-free medium was investigated. TGF-beta markedly inhibited the growth of keratinocytes at the concentrations greater than 2 ng/ml under low Ca2+ conditions (0.1 mM). Growth inhibition was accompanied by changes in cell functions related to proliferation. Remarkable inhibition of DNA synthesis was demonstrated by the decrease of [3H]thymidine incorporation. The decrease of [3H]thymidine incorporation was observed as early as 3 hr after addition of TGF-beta. TGF-beta also decreased c-myc messenger RNA (mRNA) expression 30 min after addition of TGF-beta. This rapid reduction of c-myc mRNA expression by TGF-beta treatment is possibly one of the main factors in the process of TGF-beta-induced growth inhibition of human keratinocytes. Since growth inhibition and induction of differentiation are closely related in human keratinocytes, the growth-inhibitory effect of TGF-beta under high Ca2+ conditions (1.8 mM Ca2+, differentiation-promoting culture environment) was examined. TGF-beta inhibited the growth of keratinocytes under high Ca2+ conditions in the same manner as under low Ca2+ conditions, suggesting that it is a strong growth inhibitor in both low and high Ca2+ environments. The induction of keratinocyte differentiation was evaluated by measuring involucrin expression and cornified envelope formation: TGF-beta at 20 ng/ml increased involucrin expression from 9.3% to 18.8% under high Ca2+ conditions, while it decreased involucrin expression from 7.0% to 3.3% under low Ca2+ conditions. Cornified envelope formation was modulated in a similar way by addition of TGF-beta: TGF-beta at 20 ng/ml decreased cornified envelope formation by 53% under low Ca2+ conditions, while it enhanced cornified envelope formation by 30.7% under high Ca2+ conditions. Thus, the effect of TGF-beta on keratinocyte differentiation is Ca2+ dependent. It enhances differentiation of human keratinocytes under high Ca2+ conditions, but inhibits differentiation under low Ca2+ conditions. Taken together, there is a clear discrepancy between TGF-beta effects on growth inhibition and induction of differentiation in human keratinocytes. These data indicate that growth inhibition of human keratinocytes by TGF-beta is direct and not induced by differentiation.

A giant calcifying pseudocyst seen in a patient with overlap syndrome.

A case of female with a large cystic tumor in her infrascapular region is reported. The tumor was 8 cm x 7 cm in size. Histologically, it was a pseudocyst with calcification. The patient showed no abnormal renal functions other than proteinuria. Because she had suffered from overlap syndromes of systemic lupus erythematosus, progressive systemic sclerosis, and suspected dermatomyositis, we diagnosed this a unique case of dystrophic calcinosis. There have been several reports of cases with connective tissue diseases showing subcutaneous calcification, but to our knowledge no such giant calcifying pseudocyst has ever been reported.
