Temporomandibular joint sounds in asymptomatic volunteers.

Abnormalities within the temporomandibular joints often produce audible sounds. Electronic recording of joint sounds has been used as a method of staging internal derangement. This study had three objectives: first, to determine whether the characterization of temporomandibular sounds can provide a sensitive and accurate measure for the presence or absence of joint abnormalities in asymptomatic volunteers; second, to evaluate the reproducibility of the sounds, when present, with successive recordings; and third, to evaluate the sound characteristics to determine their predictability for the types of internal derangement. Fifty asymptomatic volunteers (100 joints) were evaluated with an electronic device for the presence of joint sounds. Of the patients, 24% (N = 50, five men and seven women) (16% of all joints) had one or two abnormal joints as diagnosed by magnetic resonance imaging. Forty-four percent of all joints had identifiable sounds; 50% of the sounds were produced when the condyle was located at the apex of the articular eminence. Sounds often occurred in the early opening phase of jaw movement in joints diagnosed as normal by magnetic resonance imaging. Finally, the characteristics of these events did not produce adequate separation to stage the internal derangement.

Prevalence of temporomandibular joint internal derangement in patients with craniomandibular disorders.

To determine the prevalence of temporomandibular joint internal derangement in patients with signs and symptoms of craniomandibular disorders, bilateral imaging was performed in a consecutive series of 115 patients with signs and symptoms of craniomandibular disorders. Ninety patients (78%) had different stages of unilateral or bilateral internal derangement, and 25 patients (22%) had normal temporomandibular joints bilaterally. Out of 230 joints, 60 showed disk displacement with reduction, 8 showed disk displacement without reduction, and 29 showed disk displacement without reduction associated with anthrosis. The study indicates that almost 80% of patients with signs and symptoms of craniomandibular disorders have different forms of internal derangement.

Temporomandibular joint: improved MR image quality with decreased section thickness.

To determine if the quality of spin-echo magnetic resonance (MR) images of the temporomandibular joint (TMJ) could be improved by reducing section thickness, coronal and sagittal 3.0- and 1.5-mm MR images of the same joints were evaluated. Depiction of the disk, trabecular pattern, and cortex of the condyle was better on coronal 1.5-mm images than on 3.0-mm images (P less than .01), and 1.5-mm sagittal images were better for depiction of the trabecular pattern of the condyle than were 3.0-mm images (P less than .05). The ability of MR imaging with thinner sections to reveal more anatomic details should result in improved diagnostic accuracy.

[Unasyn in severe hospital infections].

A total 16 patients with moderate and serious surgical and urogenital infections were treated intravenously with the Unasyn IM/IV inj., combination of ampicillin and sulbactam, which is a beta-lactamase inhibitor. In this combination, sulbactam saves the ampicillin against the effects of beta-lactamases and extends the susceptibility to ampicillin of previously ampicillin-resistant strains. Clinical cure was observed in 69% of the patients, improvement in 19%, and failure in 12%. Microbiological elimination was proved in 50% of the patients, persistence in 6%, a in the rest of the patients (44%) were microbiological response unevaluable. The evaluation of subjective and objective tolerance of Unasyn IM/IV inj. was excellent. It follows from this study that the combination of ampicillin with sulbactam can be considered an effective and safe treatment of nosocomial infections, especially in the departments with an increasing rate of ampicillin-resistant strains due to production of the beta-lactamases.

Klion in the prevention of early complications following appendectomy.

The authors observations with prophylactic administration of metronidazole manufactured by the Gedeon Richter Company, Hungary (trade mark Klion) in children with phlegmonous appendicitis are reported. The drug was administered in three doses to 51 children. Results of bacteriology could be assessed in 42 children. The results of prophylactic administration of Klion in phlegmonous appendicitis were excellent. In our group there were no inflammatory wounds and/or intraperitoneal complications. For comparison, in a cohort of 1.008 children with phlegmonous appendicitis, without prophylaxis the rate of inflammatory complications was 3.62%. Klion has minimal side effects and is well tolerated and seems to be a superb drug for prophylaxis.

[Trimethoprim VUFB in the therapy of urinary infections].

Twenty women with asymptomatic significant bacteriuria took Trimethoprim (TMP) VUFB, 2 X 200 mg, for eight days. Twelve had morphological changes of the urinary pathways confirmed by intravenous pyelography. The test for the presence of bacteria with an antibody film in the urinary sediment suggested that the infection was in the upper urinary pathways in eight of the women. Immediately after termination of treatment the urine was negative in 16 women (80%), after another three weeks the negative finding persisted in the urine of nine women (45%). In one patient treatment was discontinued after five days on account of dyspeptic complaints, the remaining patients tolerated the drug well.

[Long-term effect of the administration of repeated single doses of netilmicin in urinary tract infections].

In 26 patients with different clinical forms of infections of the urinary pathways a 300 mg single dose of netilmicin was administered by the i.m. route and the bacteriological urinary finding was checked for as long as 10 weeks after administration of netilmicin. During this period the urine was bacteriologically negative in 42% of the subjects. In 8 subjects with relapses of the infection a second dose of 300 mg netilmicin was administered and the urine was bacteriologically negative for 10 weeks in four of the patients. On the whole it proved possible to eliminate the urinary infection for 42-112 days in 17 subjects (65%), even in so-called complicated infections. It may be stated that single dose treatment with netilmicin used once or repeatedly leads to prolonged elimination of the infection without affecting the sensitivity of the organisms, without side-effects and with a substantial reduction of the cost of treatment.

Comparison of three dosage regimens of ciprofloxacin in urinary tract infections.

Fifty-four patients with complicated UTI were administered ciprofloxacin in doses of 500 mg (30 subjects) and 250 mg (24 subjects) at 12-hour intervals. While a positive effect was noted in 96-100% upon termination of therapy, the effect was still present 3 weeks later in 90% of the high-dose, but only in 71% of the low-dose group. In 23 patients with uncomplicated UTI, a positive effect of the three-day therapy with 100 mg of ciprofloxacin at 12-hour intervals was observed in 91% of subjects. Intolerance to the agent was found in one case only. Development of resistance to ciprofloxacin was not observed.

[Prevention of recurrent urinary infections using Solco Urovac, a polymicrobial vaccine].

The administration of Solco Urovac polymicrobial vaccine to 26 patients with recurrent episodes of urinary tract infection significantly reduced the number of episodes, in one third of the persons even prevented them for a period of one year. In case of persistent bacteriuria, accompanying, for example, nephrolithiasis, the vaccine had no effect. A better effect can be achieved in cases of uncomplicated infections of the lower urinary tract than in infections associated with structural abnormalities of the kidneys and urinary tract. In some patients, the administration of the vaccine is accompanied by mild side effects which, but only exceptionally, can lead to the discontinuation of the therapy.
