Vulval reconstruction using rectus abdominis musculocutaneous flap transfer with secondary liposuction in extramammary Paget's disease.

Two female patients with vulval skin defects after extramammary Paget's disease were reconstructed using a pedicled, distally based rectus abdominis musculocutaneous flap. After the operation, the bulkiness of the flap, which might be a major problem in the application for a vulval skin defect, was improved successfully by flap volume reduction with secondary liposuction. We conclude that rectus abdominis musculocutaneous flap transfer with secondary liposuction is one of the acceptable methods for reconstructing vulval skin defects.

Induction of the low-molecular-weight stress protein HSP27 in organ-cultured normal human skin.

To examine the inducibility of 27-kD-molecular-weight heat shock protein (HSP27) in human skin, an indirect immunofluorescence (IF) study was performed on organ-cultured normal human skin by using a monoclonal antibody specific for HSP27. After heat treatment at 45 degrees C for 1 h, nuclear IF was observed in the epidermal cells. When the organ-cultured skin explants were exposed to 10.0 micrograms/ml or 100.0 micrograms/ml 8-methoxypsoralen (8-MOP) for 1 h and then irradiated with UVA (320-400 nm), positive nuclear IF was also observed 6 h after UVA irradiation. Considering these results with the previous reports about HSP72, it appears that, in human skin, HSP27 as well as HSP72 plays an important role in resisting various environmental stresses.

Expression of E-cadherin in skin carcinomas.

In this study, we investigated the expression of E-cadherin in 31 cases of human skin carcinoma including basal cell carcinoma (BCC), squamous cell carcinoma (SCC), Paget's disease, Bowen's disease (invasive type), and trichilemmal carcinoma, by immunohistochemical staining using a monoclonal antibody specific for E-cadherin. Similar to the E-cadherin expression in normal epidermis, E-cadherin was strongly expressed in all samples of BCC on the cell borders, whereas marked decrease or loss of E-cadherin expression was found in the tumor cells of SCC, Paget's disease, and Bowen's disease (invasive type). On the other hand, E-cadherin expression of trichilemmal carcinoma was slightly reduced. Considering the clinical and histological features of these skin carcinoma, the reduction of E-cadherin expression is considered to be associated with the invasion and metastasis of human skin carcinoma.

Bullous pemphigoid associated with internal malignancies: identification of 180-kDa antigen by western immunoblotting.

We report three cases of bullous pemphigoid associated with internal malignancies. Two cases were associated with gastric cancer and one with rectal cancer. Immunoblot analysis, using epidermal extract of normal human skin, revealed that these patients' sera reacted only with bullous pemphigoid antigen with a molecular weight of 180 kDa (BP180). The relationship of BP180 to malignant tumours is discussed.

Age-related decrease in the inductability of heat shock protein 72 in normal human skin.

In order to know the effect of chronological ageing on the induction of heat shock protein (HSP) in normal human skin, the 72-kDa HSP (HSP72) was examined in organ-cultured samples of normal human skin which were obtained from 30 individuals (age range 17-86 years). The skin explants were first incubated at 37 degrees C for 24 h, and then heat-treated at 45 degrees C for 1 h. After heat treatment, the skin explants were incubated at 37 degrees C for 1 or 3 h. Immunohistological analysis, using a monoclonal antibody specific for the HSP72, revealed that although the time course of the heat-induced HSP72 expression was similar in the young and aged groups, a lower level of induction of HSP72 was observed in the aged group. This result indicates that there is an age-related dysfunction of the heat shock response in normal human skin.

Trichilemmal carcinoma developing in a burn scar: a report of two cases.

Two cases of trichilemmal carcinoma (TLC) developing in burn scars are reported. In Case 1, a 73-year-old man developed a TLC on his left lower leg five years after a burn. In Case 2, a 43-year-old man developed a cauliflower-like mass on his head 42 years after a burn. Histologically, tumor cells showed a lobular proliferation in continuity with the epidermis. Tumor nests were mostly composed of large atypical cells with clear cytoplasms containing PAS-positive, diastase sensitive materials. Some of the nests showed trichilemmal-type keratinization. These cases were treated only with surgical excision, and there has been no evidence since of local recurrence or metastasis.

Usefulness of hard palate mucosa graft as nasal lining in alar reconstruction.

A hard palate mucosa graft was used as nasal lining for alar reconstruction in six patients. In five of the six patients, the "take" of the graft was complete. The nasal cavity was reconstructed satisfactorily, and no nasal obstruction was observed and no collapse of the nasal cavity was found in breathing because of the stiffness and thinness of the grafted mucosa. None of the patients suffered from erosion or irritation of the graft during the postoperative period. The palate donor site healed in 3 to 5 weeks, and no irritation or ulceration was seen after the healing. The hard palate mucosa can be considered as a lining material in alar reconstruction.

Induction of the 72-kD heat shock protein in human skin melanoma and squamous cell carcinoma cell lines.

Heat shock proteins (HSPs) or stress proteins comprise a characteristic group of proteins synthesized in cells exposed to heat or other environmental stimuli. Of the many HSPs, the 72-kD heat shock protein (HSP72) is the most stress-inducible one. In the present study, we examined the effects of heat, chemicals (azetidine and sodium arsenite), ultraviolet (UV) light, and gamma-ray irradiation on the induction of HSP72 in cultured human skin melanoma cell lines (P-39 and G-361), a human skin squamous cell carcinoma cell line (HSC-1), and an SV40-transformed human lung fibroblast cell line (WI38VA13) as a control. In these cell lines, heat treatment induced HSP72 more rapidly and intensely than did chemical exposure. Compared with the SCC cell line, the two melanoma cell lines produced less HSP72 with heat treatment. UVC irradiation (20 J/m2) induced HSP72 only in the WI38VA13 cells. After gamma-ray irradiation, no HSP72 induction was detected in any of the cell lines examined. These observations suggest that, in cultured cells, inducibility of HSP72 depends not only on the inducer but also on the origin of each cell line.

Dermis-fat graft after parotidectomy to prevent Frey's syndrome and the concave deformity.

Dermis-fat grafts were performed on 7 patients at the time of parotidectomy to prevent Frey's syndrome and the characteristic concave deformity. Only 1 patient demonstrated postoperative Frey's syndrome, and 1 demonstrated a slightly concave deformity due to fat absorption. Although other barrier materials have been reported to achieve success in preventing gustatory sweating, our method simultaneously corrects the associated deformity of facial contour as well.

Carcinosarcoma of the skin: immunohistochemical and electron microscopic observations.

A rapidly growing, hemorrhagic, exophytic tumor on the upper back of a 44-year-old male patient was investigated. Histological, immunohistochemical, and electron microscopic studies revealed both basal cell carcinoma-like and spindle cell sarcoma-like structures intermingled in the same tumor. Clinical consequences to this patient were mainly dependent on the sarcomatous element.

Reconstruction of two separated electrical burn ulcers.

In this study, we describe the reconstruction of composite tissue defects in two separated regions (left elbow and right thigh) using only one-sided rectus abdominis myocutaneous flap. We report here the results of our procedure, as it increases the usefulness of the rectus abdominis myocutaneous flap and can be applied to other clinical situations.

Reconstruction of palate with radial forearm flap; a report of 3 cases.

A prosthesis is the preferred method for the closure of a palatal defect following maxillectomy, with fairly satisfactory results. However, some patients suffer from symptoms due to mismatch of the prosthesis. We have reconstructed palatal defects using radial forearm flaps in 3 cases. The forearm flap is utilised for palatal reconstruction in the double-folded form. The flap is thin and pliable, there are no problems in regard to bulk and drooping of the flap, and the patients can wear the denture with minimal discomfort. We believe that reconstruction of the palate with a forearm flap is a useful procedure, especially for patients suffering from symptoms due to mismatch of the prosthesis.

Reconstruction of penis with free deltoid flap.

Two cases of reconstruction of the penis with a free deltoid flap are described. The urethra was reconstructed with part of the flap and external coverage of the penis was provided with the remainder. In one case an autogenous rib cartilage graft was inserted in the reconstructed penis as a strut. Postoperatively both patients can urinate standing up, and for the one in which a rib cartilage graft was used sexual intercourse is possible. Both cases have good recovery of tactile and protective sensation in their reconstructed penis.

Simultaneous reconstruction of cervical esophagus and oral cavity with one free jejunal graft.

A case of simultaneous reconstruction of the cervical esophagus and oral cavity with one free jejunal graft is described. A 36-year-old patient had suffered from stricture of the cervical esophagus after reconstruction with a deltopectoral flap and, at the same time, had developed cancer of the anterior floor of the mouth. Both defects were simultaneously reconstructed with one free jejunal graft pedicled on one pair of jejunal vessels. A segment of jejunum was divided into three portions leaving the mesenterium intact. The middle portion was discarded; one portion was used for cervical-esophageal reconstruction and the rest was split longitudinally and used for resurfacing the oral cavity.

Systemic sclerosis-like lesions during long-term penicillamine therapy for Wilson's disease.

Systemic sclerosis-like lesions developed in a 14-year-old boy with Wilson's disease who had been treated with D-penicillamine for 11 years. Clinical and laboratory manifestations included proximal scleroderma, pulmonary restrictive defects, positive antinuclear antibodies, and the deposition of C3 at the dermal-epidermal junction of the lesional skin. This is the first case reported in which long-term administration of penicillamine was followed by the development of systemic sclerosis-like lesions.

The pedicled extended serratus anterior myocutaneous flap for head and neck reconstruction.

A new "extended" myocutaneous flap has been designed by fasciocutaneous extension anteriorly and inferiorly from the 6th to 8th slips of the serratus anterior muscle. This flap has been used as a pedicled flap for various types of head and neck reconstructions and satisfactory results obtained. This article describes the anatomy of the flap, surgical technique and typical cases.
