Multiple transrectal ultrasound-guided prostatic biopsies--true morbidity and patient acceptance.

Transrectal ultrasound (TRUS) is becoming more widely used as a method of investigating prostatic disease. This study investigated the acceptance of this technique in 89 patients undergoing evaluation for suspected malignant disease. The true morbidity associated with TRUS and TRUS-guided biopsy was evaluated. Serious complications were rare, but minor complications were frequent. Careful counselling is recommended prior to the procedure in order to minimise the patients' anxiety and ensure that if complications do occur they are dealt with swiftly and appropriately.

Impact of previously unrecognized benign prostatic hyperplasia on the daily activities of middle-aged and elderly men.

To assess the importance of benign prostatic hyperplasia on activities of daily living, a cross-sectional survey of 1627 men aged 40-79 years (representing a 65% response rate) registered with two health centres in central Scotland was carried out, using a urinary symptom questionnaire and uroflowmetry to identify men more likely to have benign prostatic hyperplasia. The condition was defined as a prostate gland of more than 20 g in the presence of symptoms of urinary dysfunction and/or a peak flow rate of less than 15 ml s-1, without evidence of malignancy. Transrectal ultrasonography was used to measure the volume (and by inference weight) of prostate glands. A total of 410 men satisfied the criteria for benign prostatic hyperplasia. Overall, 51% of men with benign prostatic hyperplasia reported interference with at least one of a number of selected activities of daily living as a result of urinary dysfunction, compared with 28% of men who did not have this condition. In 17% of men of working age (40-64 years) with benign prostatic hyperplasia, this interference occurred most or all of the time for at least one activity of daily living compared with only 3% of men in the same age group who did not have this condition. If the criteria of unmet need for treatment of benign prostatic hyperplasia constitutes interference by urinary dysfunction most or all of the time in at least one activity of daily living, then the findings of this survey suggest that a substantial number of middle aged and elderly men living in the United Kingdom may be in need of assessment and treatment for this condition.

A prostatectomy audit: phase I--insights and questions.

A local prostatectomy audit is being undertaken to gain some insight into the pre- and peri-operative factors that influence post-operative outcome and length of post-operative stay. The aims of the first phase of the audit, involving 150 prostatectomy patients, have been to attempt to define the minimum data set required to meet the study's objectives, and to devise a data-collecting tool that can be completed by the medical and nursing staff as part of the routine work. A tentative model has been devised to forecast post-operative stay. The audit has raised a number of questions which will form the basis of further studies.

Transitional cell carcinoma of upper urinary tract: some prognostic indicators.

Thirty cases of transitional cell carcinoma were reviewed over a twenty-year period. The incidence of bladder tumor recurrence was no different in those who had complete or incomplete nephroureterectomy. Grade and stage correlated well with survival. Intravenous urogram findings at presentation were important prognostic indicators.

Endoballoon rupture and stenting for pelviureteric junction obstruction: technique and early results.

Endoballoon rupture and stenting was performed on 31 patients with pelviureteric junction obstruction. A percutaneous antegrade approach was used in 20 patients, a retrograde technique in 11. Clinical and renographic improvement was seen in 21 patients. Four patients later underwent nephrectomy, all of whom had a GFR of less than 20% in the affected kidney. Endoballoon rupture is technically undemanding, easier and less invasive than endopyelotomy. In patients with adequate renal function it provides a suitable alternative to pyeloplasty.

Malacoplakia: a case involving epididymis and a case involving a bladder complicated by calculi.

We report the sixth case of malacoplakia of the epididymis in the world literature and the first to occur in a patient who had undergone a vasectomy previously. The patient was treated successfully by epididymectomy. Another case of malacoplakia of the bladder is reported in which multiple adherent stones developed on the surface of the lesion. Long-term antibiotic therapy led to stabilization of bilateral obstruction at the ureterovesical junction.

Teflon injection in stress incontinence.

Forty-six patients, 28 females and 18 males, underwent periurethral or perurethral Teflon injection. The results in females were satisfactory, especially where resting anatomy was normal. Male results were disappointing, no patients with post-prostatectomy stress incontinence being cured.

Oxybutinin and the prevention of urinary incontinence in spina bifida.

A controlled study was performed to determine the effect of oxybutinin on incontinence in spina bifida patients on clean, intermittent self catheterisation. Cure or significant improvement was achieved in 66.6% of the patients. Bladder capacity was increased and filling pressure reduced during treatment.

Gunshot injuries of the urinary tract.

Five cases of gunshot injuries to the kidney, ureter, bladder, urethra and male external genitalia are reported. One case required nephrectomy. The others regained normal genitourinary function after surgical repair and/or adequate drainage and splintage.

Renal carcinoma with staghorn calculus, perinephric abscess, and xanthogranulomatous pyelonephritis in same kidney. Subcutaneous abscess of thigh as initial presentation.

A case is reported of the simultaneous occurrence of renal carcinoma with staghorn calculus, perinephric abscess, and xanthogranulomatous pyelonephritis in the same kidney, with subcutaneous abscess of the thigh as the initial presentation. The coexistence of these diseases in the same kidney would seem to be important since they are frequently confused with each other.

The effect on epidermal DNA synthesis of a combination of topical steroid with either dithranol or tar as used for psoriasis.

The hairless mouse was used to investigate the effects of a combination of glucocorticosteroid with either dithranol or tar on epidermal DNA synthesis, in order to determine whether such combinations reduce epidermal DNA synthesis more effectively than the single agents. Dithranol alone produced a significant local inhibition of DNA synthesis at concentrations of 0.1% and 0.05% but not at lower concentrations. Dose-response data for dilutions of clobetasol propionate and betamethasone 17-valerate showed progressive diminution of both local and systemic effects with decreasing concentrations. An additive effect was found from combining clobetasol propionate with dithranol and from combining betamethasone 17-valerate with liquor picis carbonis. These combined preparations were tested again after storage for 6 months and 2 months respectively and showed no loss of efficacy. These results lend justification to the use of these combined preparations in the treatment of psoriasis.

Applications of glucocorticosteroids. The effects of twice-daily vs once-every-other-day applications on mouse epidermal cell DNA synthesis.

To determine a schedule for continuous suppression of epidermal cell DNA synthesis, 0.05% fluocinonide ointment was applied either twice daily or once on alternate days to hairless mouse skin for 174 hours. Suppression occurred similarly for both regimens for 78 hours followed by an increase in DNA synthesis despite continued application of fluocinonide. The systemic effect was less marked with the alternate-day schedule. The effect of a single application of the fluocinonide ointment was also studied over 174 hours. Substantial inhibition of DNA synthesis occurred but for a shorter period followed by an increase and then return to normal values. Experiments were performed where second applications were made at various time intervals after the first. These did not appear to affect events until 192 hours had elapsed, when a second application caused profound inhibition of DNA synthesis once more.

The management of priapism.

This is a study of 10 men presenting with priapism and a further 25 men with impotence following priapism. Recurrent episodes of prolonged erection were a common finding and the priapism was present in most men on waking. Abnormal nocturnal tumescence was observed in one patient. The role of medical and surgical treatment was assessed.

Cholangio carcinoma treated by liver split and resection.

A case of cholangiocarcinoma at the liver hilum presenting with jaundice is described. Pre-operative transhepatic cholangiography, hepatic arteriography and splenoportography led to liver split, with successful resection of tumour and triple hepatico-jejunostomy. Despite tumour recurrence eight months of palliation were achieved. Current methods of treating the condition are discussed.
