Pulmonary fungal infections in patients with hematological malignancies--diagnostic approaches.

In a retrospective study of 56 patients with hematological malignancies and fungal pneumonia we have analyzed the value of different diagnostic procedures. In all patients (Candida n = 29, Aspergillus n = 23, mixed fungal infection n = 4) bronchoscopy and/or high-resolution computed tomography of the lungs was performed. Cultural detection of fungi in bronchoalveolar lavage was successful in 23/32 Candida and 11/23 Aspergillus pneumonias. Other relevant pathogens were identified by bronchoscopy in 21 cases. Thorax CT scans showed diagnostic evidence of fungal pneumonia in 10/13 Candida and in 16/18 Aspergillus infections. Blood cultures were positive in 9/33 Candida pneumonias and in none of aspergillosis cases. Serological testing and surveillance cultures had only limited value for the early diagnosis of pulmonary mycosis. Our data suggest that bronchoscopy and high resolution CT scans are mutually complementary diagnostic tools with high sensitivity in patients with hematological malignancies and new pulmonary infiltrates. These procedures facilitate the early and reliable recognition of invasive fungal disease which may have a bearing on the initiation, length, and differential therapy of antimycotic drugs.

Adolescent

Prevalence of periodontal breakdown in adolescents and presence of Actinobacillus actinomycetemcomitans in subjects with attachment loss.

The present investigation was carried out to study the prevalence and clinical appearance of destructive periodontal disease in a school population of 15- to 16-year old adolescents in Amsterdam. In addition the prevalence of Actinobacillus actinomycetemcomitans was studied in those subjects showing attachment loss. Attachment loss was diagnosed in 230 of the 4565 subjects participating which is about 5% of the population studied. Within this population males were more frequently affected than females (P = 0.008). Extensive periodontal destruction was found in 16 subjects (0.3%). In this group a female/male ratio was found of 1.3:1. As a result of the epidemiological survey, 105 subjects with attachment loss volunteered for further investigation. The results showed that presence of plaque, redness and swelling of the gingiva, and bleeding on probing were general phenomena. In addition, the bleeding/plaque ratio, as determined for each subject at sites without attachment loss, increased with the severity of periodontal disease as expressed by the number of sites with attachment loss (P = 0.0038) as well as by the amount of destruction at these sites (P = 0.002). A. actinomycetemcomitans could be identified in 18 of the 105 subjects with attachment loss. Analysis showed that A. actinomycetemcomitans was more frequently isolated in subjects with moderate to severe periodontal breakdown than in subjects with mild breakdown (P less than 0.02). When estimating the percentage of juvenile periodontitis patients on the basis of the classically accepted criteria, it seems likely that between 0.1% to 0.2% of the population in Amsterdam is suffering from this disease entity.

Actinobacillus

Applications of hand-arm models in the investigation of the interaction between man and machine.

The mode of vibration of hand-held tools cannot be considered without knowledge of the influence of the operator's hand-arm system. Therefore some technical applications of hand-arm models were realized for drill hammers by the University of Dortmund. These applications are a software program to simulate the motion of machine components, a horizontal drilling jig, and a chucking device in a drilling rig.

Studies on intra-arterial cushions. III. The cushions at the origin of the rat carotid body artery (CBA).

The orifice of the rat carotid body artery is surrounded by intra-cranial cushions which form a boat-like structure. The cushions are covered by normally looking endothelium, ramified smooth muscle cells are seen in its stroma. The internal elastic lamella remifies where the cushions originate, and delicate elastic fibres extend into the cushions. Axon profiles are confined to the medioadventitial border. They were never seen to penetrate into the tunica media or even to reach the cushions. It is suggested that the cushions are passively flattened during pulsatile dilatation of the external carotid artery.

[Doubling of the arteria testicularis sinistra with variations of origin and course--an embryological consideration (author's transl)].

During dissection of a human body anomalies in the origin and the course of the testicular artery have been observed. Besides a left testicular artery arching over the left renal vein there existed an additional testicular artery on the same side originating from the distal part of the left renal artery. The development of those anomalies is discussed under embryological aspects.

Characterization of synchronized cultures of Bumilleriopsis filiformis. Changes in cytochrome-f photooxidation and fluorescence induction kinetics.

Activity of the photosynthetic apparatus of synchronized cultures was studied with the xanthophycean alga Bumilleriopsis filiformis, following the kinetics of fluorescence induction and photooxidation of cytochrome f (= cytochrome c-553) of intact cells. During the beginning of the cell-division phase, minimum cellular photosynthetic activity is observed and a maximum after its completion, which is accompanied by corresponding changes in Hill reaction activity and re-reduction of cytochrome f by photosystem II light. At minimum activity, the level of steady state fluorescence was higher than at the maximum. This is due, at least in part, to the diminished electron flow between the two photosystems seemingly caused by decreased photosystem I activity. This explanation was suported by the kinetics of cytochrome-f photooxidation. Thus, electron transport activity of both photosystems appears to vary during the cell cycle.
