[Methodological aspects of observations of rapid tympanic membrane movements due to changes in static pressure (Valsalva maneuver) using digital high speech video recordings].

BACKGROUND: CCD-high speed camera systems are used to record high frequency videoclips. Introducing this technology in the analysis of eardrum motions, fast motions of the eardrum can be taped using an endoscope. METHODS: Records of the eardrum were made under specific static pressure conditions during the Valsalva maneuver. Initial experience with the help of a Kodak image (I 3) Ektapro 1000 Motion Analyzer allowed only conventional video signal processing and storage. A new data interface makes it possible to save and process the complete videoclip digitally. RESULTS: Using an image processing workstation and special mathematical algorithms, three-dimensional computer animations of rapid eardrum motions on static pressure can be performed. The present study describes different animations of fast motion eardrum movements. CONCLUSIONS: Digital high-speed video imaging is a suitable method to describe rapid eardrum motions on static pressure (Valsalva maneuver). It is possible to visualize the motion of the eardrum over time on the fast Valsalva movement at high resolution.

Nursing's renaissance. An innovative continuum of care takes nurses back to their roots.

Nurses in the Tucson area are not only serving persons in hospitals but also caring for them in their homes and neighborhoods and teaching them how to maintain their health. Three nursing programs--community nursing centers, home health services, and nurse case management--are part of the nursing continuum of care within Carondelet Health Care. Promoting clients' optimal wellness level, helping them maintain their highest level of functioning, preserving their dignity and independence, and enhancing their self-care are the goals of the Carondelet Community Nursing Centers. Nurse practitioners monitor clients' blood pressure and cholesterol and blood sugar levels and can test for episodic problems. Carondelet Home Health nurses teach clients about disease processes, symptom management, and medications; assess or monitor a patient's condition; care for wounds, and coordinate services such as physical therapy, occupational therapy, and home-delivered meals. Persons are usually referred to home health as they are discharged from the hospital. In addition to helping clients with psychosocial problems, nurse case managers perform traditional nursing functions like monitoring and teaching about medications. Nurse case management clients include the frail elderly or persons who have at least one of the following: a chronic disease that is causing steadily declining health; a terminal illness; an acute episode that requires monitoring and support; care-giver stress; or an inability to cope, as evidenced by anxiety, depression or substance abuse. Services for such clients will gain prominence in a reformed healthcare system.

'Horoscope effect' not only for seasonal but also for non-seasonal allergens.

We report on the relation between the month of birth and the chance of developing an IgE antibody response as found in a study sample of 150,000 subjects. Our results confirm that for the three seasonal allergens birch pollen, grass pollen and house dust mite, an increased relative risk was found for subjects born up to 3 months before the main season for that allergen in The Netherlands. For cat and dog allergy an increased relative risk was found from November to January, perhaps reflecting increased exposure to these pets during the winter. Surprisingly, however, also for egg white and cow's milk a clearly increased relative risk was found from November to January and a decreased relative risk in May. These data support the hypothesis of a 'sensitive' period in the first months of life during which allergen exposure is more likely to prime for an allergy later in life. The results with the non-seasonal allergens suggest that another seasonal factor exists which early in life assists (or prevents) priming by allergen.
