Tip surface changes in endocardial stimulation electrode, visualised by scanning electron microscopy.

The authors have been probably the first investigators who applied scanning electron microscopy to studies of the changes occurring in the surface of the metalic tip of an endocardial stimulating electrode. They found a lowered conductivity for secondary electron emission, and describe the surface changes in a platiniridium-tipped electrode which had been used for almost four years, in comparison with an unused electrode.

[Phonomechanocardiography of 141 healthy patients].

When studying 141 normal persons of both sexes we checked acoustic phenomena and measured the sistolic intervals. We discuss here the presence of blowing anorganic phenomena and sounds III and IV among the general public. We develop equations of regression to correct the electromechanic sistole (QIIA) and the left ventricular ejection time (LVET) the registered values being different from those published by other researchers. Last of all we analyse semiology and the interpretation of the sistolic phases of the cardiac cycle using them in a routinary way and stressing its value and limitations, specially when used to get a better knowledge of the state of the myocardial functions.

[Phonomechanocardiographic parameters of the ventricular function in the healthy adult].

There are studied several phonomechanocardiographic parameters of ventricular function and the obtained values in a normal patients' lot. It is analyzed the semiology of those data which offer information about pre-charge (apexcardiogramme's "a" index) of the myocardiac contractile state (true isosvstolic phase, ventricular pression's elevation middle velocity, integrated isovolumetric pression, and contracility index) and of its "pump" function (expulsion fraction); in the same way it is discussed the measurement's potential utility of diastolic intervals. It is emphasized the value that these parameters' measurement can have to know cardiac capacity and the affectation this can have by pharmacologic influence or by the ilnes.

[Pulmonary diffusion capacity in patients with previous myocardial infarct (with respect to the influence of smoking and pulmonary congestion)].

Pulmonary diffusive capacity by the carbon monoxide method was evaluated in 43 patients two years after myocardial infarction, and without evidence of other types of heart disease. The patients did not have primary lung disease or clinic bronchitis. Special interest was given to the effects of cigarrette smoking and moderate pulmonary congestion. A significant decrease in diffusion capacity was observed in smokers and former smokers compared to none smokers. In moderate pulmonary congestive the oposite effects was registered. pO2 was decreased in half the patients with old myocardial infarction but there was no significant statistical correlations with D1co values. Mean values for pO2, pCO2, pH, EB did not show statistically valid differences among the subgroups under study. Different factors which may influence the evaluation of diffusion capacity are discussed.

[Static and dynamic volumes after myocardial infarction/clinical and radiological correlation].

A study was made of pulmonary function tests in patients averaging 2 years post MI, who did not present evidence of chronic bronchitis or other types of primary pneumopathy. The influence of smoking and moderate pulmonary congestion due to heart failure were studied. We found a significant inverse relation between dyspnea and the forced one second expiratory volume, as an expression of bronchial obstruction. There was a significant increase in the respiratory volume in patients after MI. This alteration was particularly evident in patients who smoked. On the contrary, in pulmonary congestion the tendency was towards a decrease in respiratory volume. We confirmed the importance of pulmonary function tests for the evaluation of subjective symptoms, this was confirmed by the finding of a decrease in the dynamic pulmonary volumes, specially in smokers, which demonstrates the unfavorable influence of smoking. Heart failure did not significantly influence static or dynamic parameters, although the residual functional capacity showed a tendency to decrease with the degree of congestion.
