[Intrapartum fetal heart rate patterns and fetal outcome: prediction of newborn depression (author's transl)].

A statistically significant correlation exists between patterns of fetal heart rate (FHR) and neonatal state. However, the prognostic value of alterations of FHR for the individual has not been established. In 812 deliveries, we analyzed FHR tracings of the 30 minutes preceding delivery and correlated 6 characteristics of baseline, 10 of floating line and 6 of oscillation type with the incidence of severe acidosis (pH of umbilical cord artery less than or equal to 7.10) and low Apgar scores (one minute Apgar score less than or equal to 6). The exact risk for neonatal depression was calculated for each pattern of FHR. A high rate of depressed infants could be expected when the following alterations of FHR were seen during the last 30 minutes before delivery:--mild tachycardia, severe bradycardia,--severe variable decelerations, uniform late decelerations,--oscillation frequency less than 2 per minute independent of oscillation amplitude.

Healed untreated anterior cervical spine dislocation. A case report.

Fracture and fracture-dislocations of the cervical spine are serious injuries that rarely go undiagnosed. A case report of a bilateral facet dislocation with anterior displacement is presented that went undiagnosed and untreated, and healed without neurologic sequelae. No previous similar reports are known.

Significance of oscillation frequency in intrapartum fetal monitoring.

Until now, no statistical data have been available on the significance of the frequency of FHR oscillations. In order to evaluate the relation between oscillation frequency and neonatal outcome, the last 30 minutes of 812 FHR records were analyzed with respect to the number of 812 FHR records were analyzed with respect to the number of oscillations per minute only. Close correlations were found between slow oscillations below 2 per minute and the arterial cord pH as well as the Apgar scores after 1 and 5 minutes, respectively. No relation could be established between any oscillation frequency above 2 per minute and the measures of the neonatal outcome. In discussing the results, slow oscillations are suggested to be identical with the sinusoidal FHR. Emphasis is made that for an appropriate pattern recognition, true beat-to-beat recording is required, avoiding any averaging procedure.

Soft tissue injuries of the neck.

Soft tissue injuries of the neck are a frequent and important cause of disability and as such deserve close attention and concern. Patient management following this injury should not be left to the therapist but instead should be controlled closely by the physician to promote rapid recovery and avoid psychosomatic problems. Despite all methods of treatment there are patients who continue to complain of neck symptoms and others who develop degenerative disk changes in the years after injury.

Treatment methods in tibial condylar fractures.

A series of 915 tibial condylar fractures has been analyzed for the type of treatment used and some of the pitfalls encountered with emphasis on preservation of fracture position. The most commonly used treatment methods are discussed in depth, emphasizing the advantages and disadvantages of each and their indications in the various types of tibial condylar fractures. It is concluded that the treatment must be carefully fitted to the type of fracture and to the patient in order to achieve a strong movable knee with good alignment and stability.

Tibial condylar fractures. Evaluation of treatment and outcome.

One hundred ten tibial condylar fractures incurred during the period from 1972 to 1978 were reviewed retrospectively. Fifty-two knees were reviewed more than one year after initial injury and were subjected to critical evaluation using a point rating system. Overall, the results were acceptable in 84% of patients. The use of a cast of traction generally produced very good results. Case bracing was used in 31% of the series. Open reduction was reserved for 17 severe and challenging fractures; six of the 17 had unsatisfactory functional results.
