Oxygenation by superoxide ion of halogenofluorocarbons (Freons and haloforms) in aprotic solvents.

The reactivity of chloro- and bromofluoromethanes (Freons and others) with superoxide ion (O2.-) has been evaluated by cyclic voltammetry in dimethylformamide (DMF). Substitution of fluorine atoms into chloromethanes results in a substantial decrease in their reactivity with O2.- (relative rates: CCl4 much greater than CF4, much greater than CCl4 much greater than FCCl3, CCl4 much greater than F2CCl2, CCl4 much greater than F3CCl, H3CCl much greater than F3CCl, and H2CCl2 much greater than F2CCl2). The bromo derivatives react much faster with O2.- than the corresponding chloro compounds (F3CBr much greater than F3CCl, F2CBr2 much greater than F2CCl2, and FCBr3 much greater than FCCl3). The rates of reaction for HCCl3, HCFCl2, and HCF2Cl are approximately the same, and the reactions appear to have a common path via dehydrochlorination. Reaction stoichiometries, apparent second-order rate constants, and product profiles have been determined for these fluoromethanes and related chlorofluoroethanes.

Ocular involvement and visual sequelae in hemophagocytic lymphohistiocytosis in association with congenital dyfsfibrinogenemia.

We report a 7-year-old girl with hemophagocytic lymphohistiocytosis and an uncommon complication of vascular retinopathy and visual sequelae. She was also verified to be complicated with dysfibrinogenemia. The patient was treated with etoposide, dexamethasone, and cyclosporin A. During the fifth hospital week, a generalized tonic-clonic convulsion developed followed by deep coma. When she regained full consciousness, she complained of a visual disturbance. The ophthalmologic examination showed bilateral extensive retinal edema with numerous cotton-wool spots, indicative of vaso-occlusive retinopathy. This is inconsistent with the main finding of previous cases with ocular involvement, namely papilledema. Severely reduced visual acuity and visual field defects remained in both eyes even after systemic therapy. An awareness of this rare but serious complication is important because it may be preventable by early institution of chemotherapy. The pathogenesis of the retinopathy is also discussed.

Comparison of growth during continuous ambulatory peritoneal dialysis and renal transplantation using conventional immunosuppressive drugs in children.

OBJECTIVE: Growth during continuous ambulatory peritoneal dialysis (CAPD) and after renal transplantation (Tx) was assessed in children under the age of 12 years. DESIGN: Growth was assessed by calculating the standard deviation score for height of normal children of the same age and gender. PATIENTS: Twenty-one patients undergoing CAPD and 13 patients who had received renal Tx at Kitasato University Hospital. RESULTS: Overall growth in those children undergoing CAPD and after renal Tx was not good, but growth during CAPD seemed to be slightly better than after renal Tx using conventional immunosuppressive drugs. Only young children who had started CAPD or received renal Tx before the age of 5 years with functioning graft showed catch-up growth, but the final height of these children may still be smaller than normal. CONCLUSION: Growth in children with end-stage renal disease is poor but growth during CAPD seems to be slightly better than after renal Tx using conventional immunosuppressive drugs.
