[Acutely progressive pulmonary tuberculosis: morphological and bacteriological features].

The data of autopsies (n = 431) and morphological (n = 52) and bacteriological (n = 47) studies of the operating material obtained from the lungs of patients with tuberculosis during 1980-1989 and 1990-1999 were analyzed at the Research Institute of Pulmonology and Phthisiology, Republic of Belarus. The analysis revealed an increase in the incidence of acutely progressive forms of pulmonary tuberculosis. Autoptic data indicated that in 83.7% of cases, acute progressive tuberculosis was associated with the development of caseous pneumonia as an independent tuberculous process (31.7%) and as a phase of ensuing development of other forms (52.0%). The distinctive morphological features of today's caseous pneumonia were as follows: a great extent of lesion, an unrestrained progression of a process, a predominance of an alterative-and-exudative component of tuberculous inflammation, a rapid decay and suppuration of caseous masses, no differentiation of caseous and pneumonic foci, and suppressed cellular reactions of tissue immunity. The morphological and bacteriological studies of the operating lung material from patients with pulmonary tuberculosis suggested that there was a predominance of a progressive disseminating process at the moment of surgery.

[Contribution of immune complex pathological reactions to immunogenesis of pulmonary tuberculosis].

The association of long-term persistence of circulating immune complexes in blood flow with their deposits in the lung in 54.4% of cases and with morphological reactions of cellular and tissue structures, with the functional status of the immune system and with the progression of a tuberculous process, which was found immunohistochemical, immunological, and light microscopic studies in 68 patients with pulmonary tuberculosis, and the high reproducibility of lung-damaging effects of immune complexes in the experiment suggest that the immune complex pathological mechanism is involved into the pathogenesis of progressive pulmonary tuberculosis.

[Errors in the diagnosis of tuberculosis in general hospitals].

Ninety-six lethal cases of tuberculosis overlooked in general hospitals of Minsk in 1983-1989 have been analyzed clinicopathologically. The examination in general hospitals provided intravital diagnosis of tuberculosis in 18% of the cases only. The largest proportion of the overlooked cases related to disseminated tuberculosis (69.0 +/- 4.7%). Extrapulmonary tuberculosis and caseous pneumonia are also often missed in general institutions. Low detection of tuberculosis is attributed to incompetence of the internists in reading roentgenograms, inadequate phthisiatric alarm, incomplete x-ray and bacteriological examinations, especially of patients at risk for tuberculosis.
