[Method of cover of "difficult" duodenal stump in resection of the stomach].

The experience with 233 stomach resections in "difficult" ulcers is analyzed. The main cause of complications and lethality after these surgeries is duodenal stump sutures insufficiency seen in 17 (7.3%) patients, and in 10 (4.3%) patients it was the cause of death. Original duodenal stump cover was performed in 86 patients. There were no cases of suture insufficiency, two patients died of pneumonia and hepatic failure.

[Diagnosis and treatment of iatrogenic and criminal injuries of the esophagus].

An experience with treatment of iatrogenic and criminal traumas of the esophagus included 57 patients. There were 36 patients with iatrogenic traumas (perforation of the esophagus during diagnostic and curative manipulations and operations on the lungs and esophagus). Different kinds of operations were made depending on the localization of the injury. Three patients of this group died. Criminal traumas of the esophagus (wounds with cold steel) were treated in 21 patients, sutures on the esophagus were put in 19 of them, drainage--in 2 patients. All the patients recovered. Better results of treatment of the penetrating iatrogenic and criminal injuries of the esophagus can be achieved by timely diagnosis and operation using the proper volume and method.

[The diagnosis and surgical treatment of neck wounds].

The authors share their experiences with treatment of wounds of the neck in 210 patients. Major vessels were injured in 85 of them (40.5%), arterial vessels being injured in 19 patients, venous vessels in 23 patients. 43 patients had wounds of the hollow, parenchymatous organs and nerves. All the patients with wounds of the hollow and parenchymatous organs recovered. Lethality among the patients with wounds of the major vessels was 11.9% and was due to a combination of injuries of the vessels and blood loss incompatible with life. Active surgical treatment of penetrating wounds of the neck with careful revision of the wound, use of emergency esophagogastroduodenoscopy, laryngotracheobronchoscopy, X-ray examination allow the amount of diagnostic errors and lethality to be reduced.

[The treatment of iatrogenic esophageal trauma].

An experience with treatment of 35 patients with iatrogenic trauma of the esophagus is presented. Perforation of the cervical part took place in 14 patients and 21 patients had perforation of the thoracic part. The method of choice for operation of the injured thoracic part of the esophagus must be primary suture, at later stages it must be the drainage of the upper mediastinum. The main type of intervention for perforation of the cicatrized thoracic part of the esophagus is thought to be operation of Torek-Dobromyslov. Three patients died. One patient died from esophageal veins hemorrhage and hepatic insufficiency and two from progressing purulent complications.

[Treatment of isolated and combined burns of the stomach].

Results of the surgical treatment of 31 patients with isolated and associated burn injuries of the stomach are presented. Resection of the stomach was performed on 24 patients (15 patients--by Billroth-1 and 9 patients--by Billroth-2 method). A necessary procedure is the intraoperative forced bougieurage under narcosis of the scarry esophagus. Two patients died: one of them after gastric resection, the second--after making collateral gastroenteroanastomosis from incompetent sutures of anastomosis, peritonitis. Successful treatment is dependent on the timely diagnosis and rational curative tactics.

[Use of novosept in surgical operations].

For 15 years now the participants of surgical operations have been using novasept for the treatment of hand and operation fields. Novasept contains synthetic cationactive substances and superficially active substances which moisten the skin and penetrate the pores. The drug possesses high bactericidal action and does not irritate the skin.

[Cancer development in the cicatricially altered esophagus].

The development of cancer in the cicatricially changed esophagus was observed by the authors in 6 patients. Only one patient proved to be operable. The authors make an objection to prophylactic exclusion of the jugular portion of the esophagus or its transection in the region concerned. The follow-up and careful examination of these patients is believed to be necessary in increased pain sensations, the use of esophagoscopy and aimed biopsy being mandatory.

[Surgical treatment of cardial and gastroesophageal neoplasms].

The experience in surgical treatment of cancer of the proximal portion of the stomach in 1003 patients is described. The transpleural access was used in 452 operations. The authors believe that long-term results of the surgery are mostly dependent on the spread of tumour and the type of tumour growth.
