Epikeratophakia for the treatment of hyperopia.

Nordan epikeratophakia technique (NET) is described for the treatment of hyperopia and the results in eight eyes are reported (mean = +5.09 +/- 0.85 diopters preoperatively and -2.09 +/- 1.22 diopters postoperatively). The Nordan epikeratophakia technique is a modification of Kaufman-McDonald epikeratophakia with advantages that are described. Data are analyzed according to traditional means in addition to newer indices that consider both the uncorrected visual acuity and the refractive error.

Depression of phagocytosis by plasmin degradation products of plasma fibronectin.

Previous research has shown that disseminated intravascular coagulation causes a depression in RE function. Fibronectin, a high-molecular-weight surface-binding glycoprotein, is known to modulate RE function by facilitating opsonic activity and is sensitive to proteolytic cleavage by plasmin, yielding FNDP. The present investigation suggests that isolated FNDP, generated in vitro by incubation with plasmin, can depress phagocytosis in vivo as well as in vitro. Phagocytosis in rats was determined by a clearance technique employing CR51-RBCs and in vitro by employing a monolayer of peritoneal exudate macrophages. The in vivo studies demonstrated significantly reduced hepatic phagocytosis after the injection of FNDP an delayed clearance of injected test particles. Macrophage uptake in vitro, supported by either normal rat serum or purified fibronectin, was significantly reduced when incubated with FNDP. These results suggest that depression of the RE system and phagocytosis during intravascular coagulation may be mediated in part by the formation of plasmin degradation products of plasma fibronectin.

Adolescents in transition: a look at a transitional treatment center.

This article describes the structure, philosophy, and goals of a high expectations halfway house program for adolescents. The Transitional Treatment Center (TTC) is a residential treatment program for emotionally disturbed adolescents making the transition from institutionalization to independent community life. Topics covered are specification of the TTC program objectives, methods for monitoring residents' progress towards resocialization and plans for future programming.

Preschool vision screening for amblyopia and strabismus. Programs, methods, guidelines, 1983.

Amblyopia and strabismus, which afflict at least 5% of children, require treatment early in life for best visual results. At present, many such children are treated late or not at all. Mass screening at preschool age, and perhaps ultimately of infants, appears the only viable solution to this problem. To ascertain the present status of preschool screening in the United States, on-site visits, mail questionnaires and telephone interviews were used to study existing preschool vision screening programs at the federal, state and private organization levels. We estimate that, at most, 21% of preschool children receive any form of vision screening. Only two states, Michigan and Minnesota, have legislated requirements for such screening. Several organizations have attempted to establish screening guidelines, with suggestions of specific test and referral criteria. These guidelines are reviewed. The guidelines are of particular interest because screening programs following them typically indicate far lower prevalence rates than most studies indicate actually exist, suggesting that the guidelines result in underreferrals. In order to assess this matter, vision screening methods appropriate for preschoolers or infants, based on current evidence, are reviewed. Stereoscopic testing, utilizing a random dot stereogram format, appears the best instrument available for amblyopia and strabismus screening, but large scale comparative studies of the different test methods are needed to arrive at a final determination. Suggestions are made for the physician interested in initiating preschool vision screening programs.
