[Prospects for interferon therapy in chronic hepatitis C with normal transaminase levels].

AIM: To study effectiveness of recombinant interferon-alpha (Ifna) in patients with chronic hepatitis C with normal transaminases. MATERIAL AND METHODS: 26 patients with positive tests for abHCV and RNA HCV with a normal level of alaninaminotransferase (AlAT) entered the study. 16 patients of the test group received recombinant Ifna three times a week in a dose 3 mln IU. Mean duration of interferon therapy was 7.44 +/- 4.27 months. Control group consisted of 10 patients with natural course of the disease. Biochemical, virusological and morphological examinations were conducted in all the patients. The patients were followed up for 1.5-2 years, on the average. RESULTS: Treatment with recombinant Ifna was well tolerated. A persistent positive virusological response after the treatment was achieved in 25% of the test patients. They exhibited improvement of histological structure of the liver. In control group the dynamics was opposite. CONCLUSION: In patients with chronic hepatitis C with normal transaminases level interferon therapy improves structure of hepatic tissue. Puncture biopsy of the liver is the main method which determines the validity of the treatment and its efficacy.

[Hemorrhagic vasculitis and the hepatitis B virus].

Twenty-one patients with hemorrhagic vasculitis (HV) were examined for HBsAg, nine for HBeAg, anti-HBe, anti-HBc, anti-HBs. HBV markers were identified in 6 out of 21 patients (29.6%): HBsAg in 5 cases (one of them also anti-HBc and anti-HBe), anti-HBs in 1 case. The history of HV ranged from 4 months to 8 years. Normal serum transaminase was seen in 5 out of 6 patients with HBV markers. It was somewhat elevated in 1 of the examinees due to cytostatic treatment in parallel growing of HBsAg titer. Cryoglobulins occurred in 3 out of 6 cases. Renal biopsy from 3 HBV markers carriers identified mesangioproliferative glomerulonephritis in all of them.

[Use of commercial highly sensitive tests for detecting HbsAg in healthy persons].

The study has revealed the possibility of the contamination of serum with HBsAg under laboratory conditions during its treatment and preparation for analysis, which may be the cause of false positive results of HBsAg detection by the enzyme immunoassay (EIA). Excluding the factors of contamination, the authors demonstrate the efficacy of commercial systems, such as the passive hemagglutination test system "Gorky" and the EIA systems "Gorky" and "DIAplus", for the detection of HBsAg in the blood of donors in Moscow and Tashkent.

[Effectiveness of using recombinant interferon alfa2 (reaferon) combined with antioxidants in children with acute hepatitis B].

The authors describe the results of the first experience gained with the use of recombinant alpha 2-interferon in children with acute viral hepatitis B. The drug was administered rectally in combination with antioxidants (tocopherol). The study was carried out by the double blind method with randomization and two control groups (given tocopherol alone or placebo alone). 73 children with acute viral hepatitis B were examined. The therapeutic combination reaferon plus tocopherol was established to favour more rapid elimination of dyspeptic and abdominal phenomena, to shorten the time of the liver and spleen size increase, duration of hyperfermentemia, to provide for an accelerated reduction of HBsAg titers, elimination of HBeAg and seroconversion, to stimulate alpha-interferon production by leukocytes, and to activate the system of mononuclear phagocytes.

[Perinatal infection with hepatitis B virus and the problem of its specific prevention].

The incidence of HBs- and HBe-antigens detection in future mothers was studied in Moscow, Uzbekistan, and Moldavia, and the incidence of hepatitis B (HB) virus transmission from mothers with persisting HBs-antigenemia to their offspring. In reverse passive hemagglutination test, HBsAg was detected in 1.1% of pregnancies in Moscow, in 6.3% of pregnancies in Uzbekistan, and in 5.4% of pregnancies in Moldavia. In these regions, immunofluorescence revealed HBe-antigen in HBsAg carriers in 5.2, 13.9, and 16.3%, respectively. Perinatal infection with HB virus was found in 26.1% of births to HBsAg carriers in Moscow and in 40% in Uzbekistan and Moldavia; in the latter two regions 16.0% and 13.3% of these births were found to become chronic carriers of HBsAg. In the presence of persistence of HBeAg in HSsAg carriers, from 89 to 100% births to these mothers showed the development of durable HBs-antigenemia, while in the presence of anti-HBe such outcome was noted in only 3%. Specific anti-HBs-immunoglobulin administered to infants born to HBsAg carriers exerted a protective effect by reducing the HB virus infection rate in the first 6 months of life, but failed to prevent completely the development of HB virus infection. The necessity of a wide-scale vaccination against HB in order to prevent HB virus infection of neonates is emphasized.

[The danger of hepatitis B virus infection from transfusions of plasma in which the HBs-antigen was detected retrospectively by highly sensitive methods].

The prospective dynamic laboratory and clinical study of premature children was carried out: 55 children who received plasma transfusion during the first weeks of their life and were retrospectively (i. e. after plasma transfusion) found to have HBsAg detected by the passive hemagglutination (PHA) test, the enzyme immunoassay (EIA) and the radioimmunoassay (RIA) and 127 children who received the transfusion of plasma found to be HBsAg-negative according to the results of EIA and RIA. As revealed in this study, the risk of hepatitis B virus infection in such children after the transfusion of plasma containing HBsAg at low concentrations, determined only in the PHA test or in EIA and RIA, was, respectively, 7.5 and 6.3 times greater than in children receiving plasma found to be HBsAg-negative according to the results of EIA and RIA. The results of this investigation showed the tendency towards a decrease not only in the total contamination of plasma recipients with hepatitis B virus, but also in morbidity rate in icteric forms of acute posttransfusion hepatitis B, depending on the concentration of HBsAg in plasma used for transfusion.

[The risk of infecting premature newborn infants with the HB virus in transfusions with plasma HBsAg-negative by counterimmunoelectrophoresis].

The prospective dynamic study of 229 premature infants with the use of highly sensitive methods, such as the passive hemagglutination test and the enzyme immunoassay, for the detection of HBsAg and anti-HBs antibodies. Of these, 182 infants received plasma (blood) transfusions in the first days of their life. Plasma (blood) used for transfusions was known to contain no HBsAg in accordance with the results of countercurrent immunoelectrophoresis. 47 infants had no plasma (blood) transfusions. In the group of 182 plasma recipients the presence of hepatitis B virus infection was detected in 17.0% of infants, the ratio of icteric, nonicteric and inapparent forms being 1:4.2:2. In the group of 47 infants no case of hepatitis B virus infection was observed.

[Patients with chronic viral hepatitis as sources of infection].

Dynamic observation on 126 foci of infection formed by patients with manifest forms of chronic hepatitis B, 41 foci of chronic hepatitis of unknown etiology, and 37 foci formed by chronic "healthy" carriers was made. In the foci of type 1 the epidemic process developed intensively and was manifested mainly by HBsAg carriership in persons having had contacts with the patients. During the period of observation 43.0% of new cases of infection were detected. In the foci of types 2 and 3 the frequency of contacting infection was not different from that in the control group of the population.

[The frequency of detection of markers of hepatitis virus B infection among the inhabitants of Rustavi, the Georgian SSR].

Blood serum samples from 1,087 patients with acute viral hepatitis were studied. HBsAg was detected in 36.6% of cases. The study of anti-HBc IgM made it possible to diagnose hepatitis B in 6.6% of cases. The study of blood serum samples from 362 donors, 2,356 pregnant women and 163 medical workers in Rustavi for the presence of the markers of hepatitis B infection revealed a wide spread of hepatitis B in Rustavi.

[Detection of hepatitis B virus infection markers at a collective].

The occurrence of serological markers of hepatitis B virus infection among the members of a newly formed community (370 persons) was determined. The markers were detected with the use of highly sensitive methods for the detection of HBsAg, anti-HBs, HBeAg, anti-HBc, IgM anti-HBc. At the time of the formation of this community HBsAg, anti-HBs and anti-HBc were detected, respectively, in 4%, 11% and 31.3% and 6 months later, in 8.4%, 9.5% and 46.4% of persons. The presence of a considerable number of inapparent forms of hepatitis B and differences in the degree of the involvement of individual groups in this community into the epidemic process have been shown.

[Clinical significance of the markers of HB viral infection in hepatitis B in patients of various age groups].

The paper is concerned with some data on the detection of markers of HBV infection (HBsAg, anti-HBs, HBeAg, anti-HBe, anti-HBc of IgM class) in 277 HB patients of different age groups using highly sensitive methods based on Soviet assays. A clinical course of HB in patients of different age groups was correlated with the regularities of the detection of markers of HBV infection. The diagnostic and prognostic importance of HBV antigens and antibodies to them was shown in patients of different age groups.

[Hepatitis B markers in children with oncological diseases].

High-sensitivity methods were employed to test blood serum for hepatitis B markers in 293 infant cancer patients. HBsAg was identified in 31.7 while anti-HBs--in 16.7% of cases. Hepatitis B markers most frequently occurred in patients suffering from lymphoproliferative diseases (60.1%). HBeAg was detected in 46.5% of patients with HBsAg. The results point to a high incidence of hepatitis B virus in cancer patients.

[Characteristics of the distribution of the markers of hepatitis B vir us infection among the healthy population of the Tadzhik SSR and Azerbaijan SSR].

A total of 708 healthy persons in Tajikistan and 576 healthy persons in Azerbaijan, these groups comprising persons of both sexes and different age groups, were examined by the method of double gel immunodiffusion (the gel precipitation test) and by the passive hemagglutination test for the presence of the markers of hepatitis B virus (HBV) infection (HBsAg and HBeAg) and antibodies to them. This investigation showed that, in accordance with the level of hepatitis B morbidity, HBsAg was significantly more often detected among the population in Tajikistan (7.2%) than in Azerbaijan (2.8%). In both republics HBV carriers occurred most frequently among children aged 1-4 years (4.0% in Azerbaijan and 13.9% in Tajikistan), and among men more frequently than among women. In accordance with different intensity of the spread of HBV infection in the territories under comparison, differences in the age structure of the immune population were noted: in Tajikistan the formation of the immune layer occurred most frequently among younger age groups and in Azerbaijan, among senior adult age groups. The presence of a considerable percentage of persons with HBe-antigenemia (14.3-14.9% as determined by the gel precipitation test) among HBV carriers, observed in Tajikistan and in Azerbaijan, indicates that some of them have undetected chronic hepatitis B.

[HBe antigen and its antibodies in HBsAg carriers in various regions of the USSR].

A total of 300 blood serum samples, containing HBsAg and obtained from donors in three regions of the USSR (the RSFSR, the Uzbek SSR and the Moldavian SSR) differing in the level of HBsAg carriership, were studied for the presence of HBeAg and antibodies to this antigen in the passive hemagglutination (PHA) test and in the enzyme immunoassay (EIA). The occurrence of HBeAg was found to depend on the level of HBsAg carriership in the region. Thus, according to the EIA results, in Gorky, Kishinev and Tashkent HBeAg was detected, respectively, in 5.5%, 12.3% and 13.3% of serum samples, the level of HBsAg carriership in these cities being, according to the results of the PHA test, 1.4%, 5.0% and 9.0%. As shown by the results of EIA, the occurrence of HBeAg increased with the rise of the titer of HBsAg, while regarding the occurrence of antibodies to HBeAg the reverse relationship was observed.
