A study was made of changes in functional status of complementary system in 42 patients with chronic renal failure on programmed hemodialysis. Early stages of hemodialysis were accompanied by short-term and reversible activation of complementary system. 100 sessions were followed by complement depression. Complement system was found to be involved in two pathogenetic mechanisms of respiratory insufficiency: block of pulmonary microcirculation by leukocytic aggregates and DIC syndrome associated with microthrombosis of the lesser circulation vessels. To manage the above complications, plasmapheresis is advocated.
Efferent therapy (hemosplenosorption, plasmapheresis, autologous blood radiation) was used in multimodality treatment of 45 patients with sepsis. The therapy exerted a beneficial effect on the disease, favoured the elimination of or minimized intoxication, normalized hemostasis and immunity. Indications for use of hemosplenosorption, plasmapheresis and autologous blood radiation are provided.

A study was made of adrenal function in 2 groups of patients with chronic renal insufficiency on programmed hemodialytic therapy. Insufficiency of gluco- and mineralo-corticoid function of the adrenal glands and hyperfunction of their medulla developed in patients with normal BP and controlled hypertension. In patients with uncontrolled hypertension, insufficiency of adrenal glucocorticoid function was attended by hypercatecholaminemia and aldosteronism. Genesis of the revealed disorders was associated with the loss of hormones into the dialysing solution through the membrane, depletion of adrenocortical functional reserves and disturbance of pulmonary regulation of the level of biologically active substances as a result of microthromboembolization of the vessels of the lesser circulation attending hemodialysis.

The examination of 88 patients with chronic gastric ulcers in the early postoperative period has revealed different changes of central hemodynamics, functional capacity of the myocardium and microcirculation in cases with bleeding gastric ulcer and associated ischemic heart disease. When correcting the disturbances of the cardiovascular system the main mechanism of functional disturbances of the circulation system should be taken into consideration (hypovolemia, hyperdynamics or hypodynamics of the myocardium, disturbance of microcirculation).

Blood plasma renin activity, blood adrenaline and noradrenaline concentrations, and the kallikrein-kinin system were studied in 48 patients with chronic renal failure (CRF). Thirty-five patients with normal blood pressure and controlled hypertension (group I) showed a decrease in blood plasma renin and kallikrein-kinin system deficiency. In 13 patients with uncontrollable hypertension (group II), the renin-angiotensin and sympathoadrenal systems were found to be activated in the presence of the kallikrein-kinin system deficiency. Disorders of normal interrelations between the renin-angiotensin, sympathoadrenal and kallikrein-kinin systems in patients with CRF are determined to a significant degree by upset regulation of the metabolism of biologically active substances by the lungs in connection with the occurrence of microemboli in the pulmonary circulation during hemodialysis.

The clinical course of repeated peridural anesthesia was studied in 41 patients. Against the background of the repeated peridural anesthesia sensometry by A. K. Sangajlo method and investigation of hemodynamics were performed. It was established that for the conducting of the repeated peridural anesthesia patients should be carefully selected with the objective evaluation of the sequellae after the first anesthesia taken into account. Great technical problems can accompany the procedure resulting from availability of adhesive processes in the peridural space. The resorbtive effect of the anesthetic drug is found to be reduced.
