[Disorders of microcirculation in diffuse toxic goiter and their correction in the early postoperative period].

A study was made of microcirculation in 45 patients suffering from diffuse toxic goiter at the early postoperative period with the aid of microphotography of the bulbar conjunctiva vessels, and investigation of the rate of the isotope elimination from the intradermal depot and the capillary permeability by V. P. Kaznacheev's method. The data obtained permitted to draw a conclusion that on admission to the clinic patients with toxic goiter were suffering from arteriolar spasm, had poor vascular pattern in the bulbar conjunctiva, the sledge phenomenon in small venules, an acceleration of the isotope elimination from the intradermal depot and an increase of the capillary permeability. Preoperative preparation improved the microcirculation indices, without completely normalizing them, however. Microcirculatory disturbances reached their maximum the first two days after the operation and declined by the 3rd-5th day. Recommendations on correction of the microcirculatory disturbances are given.

[The choice of the method for extracorporeal detoxication in diffuse suppurative peritonitis].

The authors have made the comparative assessment of effectiveness of application of plasmapheresis, hemosplenosorbtion and combination of discrete plasmapheresis with subsequent perfusion of the formed elements of blood through the xenospleen in patients with diffuse peritonitis in the toxic and terminal phases in early postoperative period. It is indicated: performing of the combined afferent therapy--discrete plasmapheresis with subsequent free perfusion of formed elements of blood through the xenospleen increases the effectiveness of surgical treatment of the diffuse peritonitis in toxic and terminal phases due to more prominent immunomodulating and detoxicative effects than isolated application of each of methods.

[Modified method of blood perfusion via xenospleen in patients with peritonitis].

The authors compare the therapeutic efficacies of blood perfusion via porcine spleen and of discrete plasmapheresis followed by perfusion of formed elements of the blood via the porcine spleen by their own accord, included in the postoperative multiple-modality treatment of patients with the toxic and terminal phases of diffuse peritonitis. Discrete plasmapheresis followed by perfusion of formed elements of the blood via the xenospleen by their own accord yielded a more marked detoxifying and immunomodulating effect than hemosplenosorption, was simple to perform and associated with no complications. Use of this method in patients with diffuse peritonitis improved the efficacy of surgical treatment at the expense of reduction in the incidence of the postoperative complications and decreased mortality rates.

[Use of dimephosphon in patients with obliterating diseases of the vessels of lower limbs during the postoperative period].

The authors compared the course of the postoperative period in patients with obliterating diseases of the vessels in the lower extremities and concomitant diabetes mellitus who were on conventional drugs and on a new home-made drug dimephosphon. It has been established that the use of dimephosphon in a complex postoperative therapy improves the results of surgical treatment of patients with obliterating atherosclerosis of the vessels in the lower extremities and concomitant diabetes mellitus. The results are due to dimephosphon-induced activation of fibrinolysis, platelet hemostasis normalization, arrest of latent disseminated intravascular coagulation syndrome, immunocorrecting drug effect and its positive impact on hormonal homeostasis.

[Changes in central hemodynamics and their correction during reconstructive surgery in Leriche's syndrome].

Study of the parameters of central hemodynamics in the course of aortofemoral shunting for Leriche's syndrome revealed the role of lipid peroxidation dysfunction in the genesis of cardiovascular disorders developing in the immediate postoperative period. The postoperative period is characterized by expressed hypotension developing in the presence of activation of lipid peroxidation and suppression of plasma antioxidative activity. The authors recommend alpha-tocopherol, cytochrome C, and dalargin to prevent these hemodynamic disorders.

[Pathogenesis of respiratory insufficiency in patients submitted to bilateral aortofemoral shunting in Leriche's syndrome].

The complementary system is activated in response to development of the reperfusion syndrome in patients subjected to bilateral aortofemoral shunting for Leriche's syndrome. These changes result in the development of respiratory disorders presenting as constriction of the small and medium-sized bronchi, increase of pulmonary shunting of the blood, and manifest hypoxia.

[Improvement of epidural anesthesia in reconstructive surgery of the aorta in Leriche's syndrome].

Fifty-six patients subjected to aortofemoral bilateral shunting under prolonged epidural anesthesia are examined. Swan-Gans catheter was used to evaluate central and pulmonary hemodynamics. Ganglionic blocking with pentamine for taxiphylaxis was carried out in 31 patients prior to catheterization of the epidural space. Epidural anesthesia with ganglional blocking is associated with reduced vasodilation and hypotension and attenuates the hemodynamic reactions during clamping and unclamping of the aorta, thus decreasing the incidence of intra- and postoperative complications.
