[Hemostasis disorders after resection of liver and approaches to their prevention and correction].

Second- or third-degree dysfunction of hepatocytes was the most frequent (47.9%) disorder in the hemostasis system on the first-second days after extensive resection of the liver, whereas after economic resection this disorder was observed in only 5.5% patients. Intraoperative blood loss of more than 3 liters is fraught with hemorrhages and hepatorenal insufficiency in the postoperative period. Such blood loss is complicated by coagulopathic states presenting as 1) hepatocyte dysfunction of the second-third degree, 2) hypoxia and decreased production of blood clotting factors, and 3) variants of the DIC syndrome. Study of the genesis of postoperative coagulopathies after resection of the liver and development of pathogenetically-based methods of drug correction, together with introduction of novel instrumental methods affecting the intensity of local and total hemostatic reaction, helped reduce 2.5 to 3 times the total blood loss.

[Clinical significance of the determination of protein C dependent disorders of hemostasis in surgical patients].

Diagnostic value of protein C measurements was investigated in 159 surgical patients with diseases of the liver, great vessels, and wound infection. Protein C antigen was detected by ELISA with specific antibodies to protein C. The changes of protein C antigenic activity correlated with severity of hepatic disorders. Inhibition of protein C synthetic activity went on in parallel with a fall in pseudocholine esterase concentrations. This suggests feasibility of protein C measurements for examination of protein production by the liver. Characteristics and location of the lesions in great vessels were also related to protein C antigen levels. The fall in this antigen quantity was observed in patients with purulent wound infection. The greater the fall, the more serious the patient's condition became.

Poly(2-hydroxyethyl methacrylate) particles for preoperative endovascular occlusion of extensive angiodysplasias.

The application of superselective endovascular occlusion in vascular radiology has broadened the scope of possible radical surgery of giant angiodysplasias by reducing considerably the risk of severe haemorrhage during the surgery. Occlusion was performed on 229 patients using spherical and cylindrical particles of porous poly(2-hydroxyethyl methacrylate). The treatment of a patient suffering from angiodysplasia of the pelvis is described as an example. The first day following endovascular occlusion a hypercoagulation reaction developed, with a decrease of both the level of fibrinolysis and of anticoagulant factors. This was mainly due to a decrease of the level of antithrombin III and activation of the factor XIII indicating a strong tendency of intravascular thrombosis. During the third day, the hypercoagulation reaction increased. However, on the fifteenth day, parameters of the haemostasis system were close to normal values. Only the activity of the factor XIII remained high suggesting the stabilization and the organization of fibrin clot in the embolized blood vessel territory.

Angiodysplasia

[Characteristics of vascular changes and hemostatic reaction of the body in nonspecific aorto-arteritis].

Laboratory criteria to differentiate diagnostically nonspecific aorto-arteritis with other forms of obliterating arterial diseases have been established by hemostatic, morphological and cytological investigations. Factors of plasmic hemostasis and platelet functional activity are able to indicate etiological cause of aortal lesion in noninvasive clinical examination. Identification of platelet--forming reaction type shows that nonspecific aorto--arteritis is characterized by "red" platelet formation and fibrinization of the body. Atherosclerotic stenosing proceeds by "white" thrombus formation associated with enhanced activity of platelet adhesion and aggregation.

[Effect of controlled bacteria-free medium on the course of the wound process in patients with chronic venous insufficiency of the legs complicated by trophic ulcer].

Treatment in a controlled abacterial medium was applied to 102 patients with venous ulcer of the lower limbs. The course of the wound process was studied on the basis of objective examination methods (microbiological, cytological, morphological, study of the activity of tissue hemostasis factors). The results showed that a controlled abacterial medium causes favourable changes in the course of the wound process and allows the trophic ulcer to be prepared for operation in a short period of time (9-12 days).
