[Using prostaglandin inhibitor dicloberl in patients with ureteral calculi].

75 patients have been given prostaglandin inhibitor dicloberl in a complex therapy. Comparative analysis of traditionally treated patients with renal stones and with dicloberl in a complex therapy is presented in the article. The combination of dicloberl with tramadol was found to be more effective drug treatment for patients with renal colic. This treatment has to be applied for patients with renal stones up to 6 mm. It is an effective pharmacotherapy for excreting renal stones.

[The incidence, features of the course, and treatment of arterial hypertension in patients with prostate hyperplasia].

In 144 middle-aged and elderly male subjects with benign prostate hyperplasia, a study was made into incidence and course features of concomitant arterial hypertension (AH). AH has been proved to be encountered in 144 percent of patients with benign hyperplasia of the prostate. It has been ascertained that in 84% of patients, irritative and obstructive symptoms related to benign hyperplasia of the prostate adversely affect the course of concomitant arterial hypertension. Striking reduction of manifestations of benign prostate hyperplasia-related irritative and obstructive symptoms using cardura, a selective alpha-adrenoblocker, has come to significantly enhance efficiency of treatment of concurrent AH.

[Drug therapy of spastic disorder of urine outflow in patients with spinal injury and spinal cord injury].

A study was made of efficiency of medicamentous correction of a spastic disorder of outflow of urine from the bladder in 42 patients with dull trauma of the spinal column and marrow. The use in the above patients of cardura alpha 1-adrenoblocker permits restoring a spastic disorder of urine outflow in a 10-day period after the trauma sustained in 82 percent of patients. Shortening the time of a spastic disorder of urine outflow in patients with trauma of the spinal column and cord we allow the frequency of development of the infectious inflammatory process in organs of the urogenitary system to be substantially reduced.

[Oxidation-reduction processes in patients with recurring nephrolithiasis complicated with chronic pyelonephritis and urostasis during combined treatment].

Results of study into the condition of reduction-oxidation processes, in particular of those associated with generation of the superoxide radical, are submitted together with those of determination of levels of inorganic phosphate which is known to be an important indicator of the tissue bioenergy potential, in 112 patients presenting with a recurring form of nephrolithiasis exposed to a multiple-modality treatment that included conventional antiinflammatory and antibacterial therapy plus "open" and endourological operations on kidneys and ureters. The multiple-modality treatment of the main group patients was supplemented with phlogenzyme, a drug of II generation systemic enzymotherapy. An enhanced generation of superoxide radical was recordable having developed in the wake of a steady increase in the activity of xanthinoxidase. The authors have come to the conclusion that the conducted therapeutic intervention, especially as part of systemic enzyme therapy, results in decline of activity of the xanthine oxidation processes and brings about a change in the blood:urine inorganic phosphate concentrations ratio.

[Acute pyelonephritis associated with intestinal dysbacteriosis: incidence and enhancement of efficiency of treatment].

A study was made on the incidence rate of acute pyelonephritis associated with intestinal dysbacteriosis in urological patients (n = 68). Prehospitalization and precombined-treatment duration of the illness came up to one to four days. It has been ascertained as a result of the conducted study that under present ecological conditions, acute pyelonephritis runs its course in the presence in patients of dysbacteriosis of the intestines even before the start of treatment in an urological clinic setting. Antibacterial treatment of patients with acute pyelonephritis without simultaneous action on the pathogenic intestinal microflora and normalization of colonizing normoflora was found to produce profound aggravation of dysbacteriosis and to result in the development of candidasis. Extermination with the aid of the intestinal antibiotic intetrix of the pathogenic microflora in the intestines together with achieving of normalization of intestinal normoflora by way of the enteral intake by patients with acute pyelonephritis of eubiotics permit the marked improvement to be achieved in results of combined treatment thereof.

[The effect of symptomatic therapy with the alpha 1-adrenoblocker Cardura in patients with benign prostatic hyperplasia on the course of cardiovascular diseases].

Forty-two patients with benign hyperplasia of the prostate were studied for the role of symptomatic therapy with a selective adrenoblocker Cardura in the prophylaxis of progression of such diseases of the cardiovascular system as arterial hypertension, angina pectoris. The obstructive and irritative symptomatology-related uresis has been shown to get normalized. Also recordable in the conducted investigations was an alleviation of arterial hypertension together with a noticeable decrease in the frequency and severity of attacks of angina.

[The use of the alpha 1-adrenoblocker doxazosin in the pharmacotherapy of disorders of urine outflow of spastic origin].

Our objective in this study was summing up of results of clinical studies on effectiveness of a selective alpha-adrenoblocker doxazosine in the pharmacotherapy of reflex and spastic disorders of outflow of urine. Doxazosine ("Cardura", Pfizer Inc.) was prescribed per os once per day in the morning after meal in a dose of 2-4 mg. The drug can be used not only as a hypotensive agent but also in conservative treatment of patients with prostate hyperplasia. It was found to be very effective in attempts to bring the disordered outflow of urine back to normal in patients presenting with calculi in the lower third of ureters as well as those with prior prostata resection; in affections of and injuries to the brain and spinal cord.

[The role of immunologic factors in the pathogenesis of urolithiasis].

The role of thrombin, fibrinogen, specific and unspecific immunological reactions in the pathogenesis of urolithiasis were evaluated. It was found in the urine of patients with urolithiasis and concomitant pyelonephritis that most bacteria, leucocytes and organic substances fixed on crystalloids contained on their surfaces immunoglobulins M, G and A. The mechanisms of the possible participation of immune reactions in the pathogenesis of urolithiasis are discussed.

[Postoperative endolymphatic antibiotic therapy in urology patients with suppurative-inflammatory diseases].

In 54 patients with pyoinflammatory processes in the urogenital system, the efficacy of endolymphatic antibiotic therapy was investigated after surgical interventions into these organs along with administration of antibiotics just into the inguinal lymph node. To administer antibiotics, separation and drainage of the inguinal lymph node were conducted concurrently with surgical interventions in 26 patients for pyoinflammatory processes in the epididymis, prostate, and kidneys. In the other group (n-28), the inguinal lymph node was drained on days 4 to 5 of postoperative interventions due to the progression of an acute infectious inflammation in the affected organs and wounds. The efficacy of endolymphatic antibiotic therapy was evaluated by using clinical, laboratory, cytochemical, and immunological criteria. The drainage of inguinal lymph nodes in combination with aminoglycoside and cephalosporin antibiotics given in the postoperative period proved to be a safe and effective mode of treatment of pyoinflammatory processes in urogenital organs and wounds.
