[The use of SM-4300 against severe infections complicated with hematological diseases].

The intravenous human gamma-globulin (SM-4300) was used with antibiotics for 9 patients who had severe bacterial infections resisted to the antibiotics. All of these patients had underlying hematological diseases, acute leukemia, aplastic anemia, malignant lymphoma and pure red cell aplasia. In many of the cases the leukocyte counts were extremely decreased. In 11 episodes of the infections, 4 cases were successfully treated by SM-4300 and 4 cases were not. No adverse reaction was found. It seems that the use of SM-4300 is effective for the treatment of these severe infections.

Detection of adult T-cell leukemia-associated antigen in T-cell malignancies in the Nagasaki district of Japan.

Adult T-cell leukemia-associated antigen (ATLA), a human retrovirus-associated antigen, and anti-ATLA antibodies were examined in 52 cases of T-cell malignancies in the Nagasaki district in southwestern Japan, which is known to be an ATL endemic area. These T-cell malignancies included 27 cases of ATL, 2 cases of T-cell chronic lymphocytic leukemia (T-CLL), 18 cases of T-cell lymphoma, and 5 cases of mycosis fungoides. Using the immunofluorescence method, ATLA-positive cells were detected in short-term mass culture of mononuclear cells from 22 of the 27 ATL patients, both T-CLL patients, 17 of 18 T-cell lymphoma patients, and 3 of the 5 mycosis fungoides patients. In an ATL patient in whom a high percentage of ATLA-positive cells were detected, many type C virus particles were observed in the extracellular space of the cultured mononuclear cells with an electron microscope. Sera from all 27 of the ATL patients, the 2 T-CLL patients, 15 of the 18 T-cell lymphoma patients, and 4 of the 5 mycosis fungoides patients were anti-ATLA antibody positive. These results indicate the possible participation of the retrovirus, ATL virus (ATLV), in these T-cell malignancies in ATL-endemic areas.

Erythrocyte glutathione reductase activity and acquired trisomy #8 in various hematologic disorders.

The gene for erythrocyte glutathione reductase (E-GR) activity has been assigned to chromosome #8. In the present series, we examined the E-GR activity in 14 cases with chronic myelodysplastic syndrome (CMS, preleukemia), atypical acute myelogenous leukemia (AML), or chronic myelogenous leukemia (CML), with and without acquired trisomy #8. No difference in the incidence of high levels of this enzyme was found between two groups, i.e., those with and without trisomy #8 suggesting the existence of a complex regulatory system in addition to chromosome #8.

Reduced helper (OKT4+): suppressor (OKT8+) T ratios in aplastic anaemia: relation to immunosuppressive therapy.

T cell subset composition of peripheral blood and bone marrow from 22 patients with aplastic anaemia (AA) was studied by monoclonal antibodies (OKT3, OKT4 and OKT8). In the peripheral blood, OKT3 (pan-T), OKT4 (helper/inducer-T) and OKT8 (suppressor-T) cells varied widely in number. The ratios of OKT4:OKT8 exhibited the same tendency. However, a subgroup of AA with a reduced ratio of lower than 1.0 was present both in cases with or without prior prednisolone therapy. Of the eight patients treated with immunosuppressants, four with reduced ratios responded, whereas the other four with normal or higher ratios did not. The ratios of two of four responders gradually reached the normal level. These results suggest that the reduced OKT4:OKT8 ratio may be related to the cell-mediated immunosuppressive mechanism postulated as a cause of stem cell inhibition in a subgroup of AA, and indicate prospectively the effectiveness of immunosuppressive therapy.

Sequential karyotypic evolutions and bone marrow aplasia preceding acute myelomonocytic transformation from myelodysplastic syndrome.

Serial haematopathological and cytogenetic studies disclosed three distinct clinical phase in a case of refractory anaemia (RA), a subtype of myelodysplastic syndrome (MDS; FAB group, 1982): first, chronic MDS phase (1 year 10 months) with karyotypic abnormality (45, XY, --7) (Clone I); second, hypo-aplastic phase concurrent with first clonal evolution (45, XY, --7, 12p--) (Clone II); third, acute myelomonocytic leukaemia phase (6 months) with second clonal evolution (45, XY, --7,t (1q --; Bq+), Bq --, 12p --) (Clone III). In the second phase the bone marrow became almost aplastic as Clone II expanded progressively, indicating simultaneous occurrence in Clone II stem cells of growth advantage for self-renewal function over Clone I and normal stem cells, and arrest of differentiation. These observations support the hypothesis that leukaemic change in MDS, at least in RA, occurs by stepwise clonal evolution(s), not by progressive arrest of differentiation in original MDS clone.

Alkaline phosphatase-positive leukemic monocytes in a child with acute monocytic leukemia.

A subpopulation with alkaline phosphatase activity and neutrophilic granules was found in leukemic monocytes from a child with acute monocytic leukemia (M5B). Almost all leukemic cells were strongly positive for nonspecific esterase and phagocytized opsonized zymosans. These findings suggest that the subpopulation are chimeric cells with characteristics of both monocytic and neutrophilic cell lines. The leukemogenesis in this patient might involve the simultaneous expression of differentiation genes otherwise restricted to either cell lineage.

[Effects of gamma-oryzanol on gastric lesions and small intestinal propulsive activity in mice].

Effects of gamma-oryzanol were studied on gastric lesions induced by both conditioned emotional stimuli (CES) and REM sleep deprivations and on the facilitation of small intestinal propulsive activity by CES in mice. The CES were given by the communication box method, and the REM sleep deprivations were performed by a modified flower pot method. The incidence of gastric lesions in responder mice induced by CES was reduced by twice p.o. administrations at 6 hr interval of gamma-oryzanol at 200 and 500 mg/kg, oxazolam at 2 mg/kg and atropine at 1-10 mg/kg. The incidence in sender mice was also reduced by gamma-oryzanol at 200 and 500 mg/kg. In addition, the incidence of gastric lesions induced by REM sleep deprivation was also reduced by single administration of gamma-oryzanol at 100 and 200 mg/kg and oxazolam at 5 mg/kg. The facilitation of small intestinal propulsive activity in responder mice induced by CES was suppressed by gamma-oryzanol at 100 and 200 mg/kg and atropine at 10 mg/kg. These results indicate that gamma-oryzanol has an antiulcerative action on gastric lesions induced by CES and REM sleep deprivation, and it has a suppressive action on the facilitation of intestinal propulsion induced by CES.

Monoclonal gammopathy in adult T-cell leukemia/lymphoma: a report of three cases.

Adult T-cell leukemia/lymphoma (ATL/L) is clustered in southwestern Japan, especially in the Nagasaki and Kagoshima areas. It was shown that this clustering correlated with the presence of antibodies to antigens of a new C-type RNA leukemia virus. Surface marker analysis of neoplastic T-cells of ATL/L patients shows the helper/inducer phenotype (Leu-1+, Leu-2a- and Leu-3a+). On the other hand, the association between T-cell malignancy including ATL/L and monoclonal gammopathy is very rare. Three unique cases of ATL/L with monoclonal gammopathy are reported. Yet the meaning of an M-component in T-lymphocytic proliferation remains uncertain. The relationship between the leukemia virus, ATL/L-cells and monoclonal gammopathy is discussed.

Temporal analysis of a dose-response relationship: leukemia mortality in atomic bomb survivors.

A data analysis that incorporates time dependencies is demonstrated for the dose response of leukemia mortality in the atomic bomb survivors. The time dependencies are initially left unspecified and the data on leukemia mortality--up to the end of 1978--are used to infer them. Several findings based on T65 revised doses (T65DR) are obtained. First, it is shown that the fits to the data of time-dependent L (linear in gamma dose)-Q (quadratic in gamma dose)-L (linear in neutron dose), L-L, and Q-L dose-response models are significantly improved (P less than 0.001) by using the corresponding time-dependent dose-response models. Second, it is shown that the increased risk of leukemia mortality due to gamma irradiation decreases in time while the increased risk due to neutron exposure decreases more slowly, if at all, in time. Consequently, relative biological effectiveness (RBE) of neutrons is shown to increase in time (P = 0.002) and the current definition of RBE as a time-independent quantity is therefore challenged. It is demonstrated with time-dependent models that the L-L model has a poor fit (P = 0.01) to the data for the first 7 years of study, but has an adequate fit for the remaining 21 years. In contrast the Q-L model has an adequate fit for the entire follow-up period (P greater than 0.30).

Evaluation of cefoperazone in the treatment of bacterial infections in patients with hematological diseases.

This study was conducted as a multicenter trial in 12 institutions in the Kyushu region of Japan. The clinical effects of cefoperazone were evaluated in 101 patients with 112 infections complicated by hematological disease. In about half of the patients, the baseline neutrophil count was below 1,000/mm3. Results were excellent in 23 of 101 cases and good in 48, with an efficacy rate of 70.3%. Cefoperazone alone or in combination with other antibiotics was effective in 60.7% of patients who had not responded to previous antibiotic therapy. No serious side effects attributable to cefoperazone were noted. The results indicate that cefoperazone is a useful antibiotic for the treatment of severe infections in patients with hematological diseases.
