Pseudotumoral appearance of cytomegalovirus esophagitis and gastritis in AIDS patients.

Cytomegalovirus (CMV) is frequently isolated from patients with acquired immunodeficiency syndrome (AIDS), and often causes disseminated disease. Gastrointestinal CMV involvement is recognized with increased frequency in AIDS patients. It can involve the entire alimentary tract and usually produces mucosal ulceration. Pseudotumoral lesions develop very infrequently. We report two unusual detailed cases of biopsy-proven CMV esophagitis and gastritis resembling carcinoma, in patients with human immunodeficiency virus infection. In AIDS patients, the gastrointestinal CMV involvement could show pseudotumoral appearance.

Effects of several calcium channels modulators on the [3H]noradrenaline release and 45Ca influx in the rat vas deferens.

1. The effects of nifedipine (1 microM), CdCl2 (0.1 mM) and the Bay K 8644 enantiomers (1 microM) on [3H]noradrenaline release and 45Ca uptake in epididymal and prostatic rat vas deferens were investigated. 2. Nifedipine, CdCl2 and Bay K 8644 optical isomers did not affect the basal tritium release. However, the [3H]noradrenaline release evoked by high potassium (50 mM) from both portions of rat vas deferens was markedly inhibited by CdCl2, scarcely affected by nifedipine and not modified by Bay K 8644 enantiomers. 3. (-)-Bay K 8644 increased the basal and potassium (50 mM) induced 45Ca uptake whereas (+)-Bay K 8644, nifedipine and CdCl2 did not alter the basal 45Ca uptake. However, they strongly inhibited the uptake induced by potassium in both portions of rat vas deferens. 4. These results suggest that the calcium channels (mainly L type) are involved on the contractions in rat vas deferens epididymal and prostatic halves; these channels differ from those present in sympathetic nerve terminals (likely of N Type) which modulates the NA release. 5. This study also shows that Bay K 8644 optical isomers possess opposite effects on the L channels of bisected rat vas deferens smooth muscle.

Statistical evaluation of the predictive power of fine needle aspiration (FNA) of salivary glands. Results and cytohistological correlation.

Fine needle aspiration (FNA) of the salivary glands was carried out on 97 patients. Diagnosis was confirmed by histological findings in 93 patients. There were 75 benign lesions (including 52 benign tumours) and 18 malignant lesions. In our series "positive predictive value" of FNA was 0.900 and the negative predictive value was 0.963. Thus, the probability of a false positive is 0.100 and of a false negative 0.037.

Reduction of cardiovascular side effects associated with ocular administration of metipranolol by inclusion in polymeric nanocapsules.

A new formulation of metipranolol base for ophthalmic administration was developed consisting of a colloidal suspension of polyepsiloncaprolactone nanocapsules with an oily core (Migliol 840) in which the drug is dissolved. Physicochemical properties of the nanocapsules show that the polymer coating around the oily droplets causes an important reduction of the droplet size, with no significant modification of the surface charge noted. When this formulation was administered to rabbits, a reduction of intraocular pressure similar to that seen with commercial eye drops was observed. Nevertheless, the evaluation of the cardiovascular side effects clearly showed lower conjunctival absorption of the encapsulated drug compared with the commercial drops. The direct (bradycardia) and the indirect evaluation (based on the study of the influence on the hypotensive and positive chronotropic effects of isoprenaline) showed that blockage of beta-adrenoreceptors was reduced greatly by the topical administration of the new formulation.

Comparative study of in situ hybridization and immunohistochemical techniques for the detection of human papillomavirus in lesions of the uterine cervix.

Among the techniques currently used for the detection of human papillomavirus (HPV) in genital lesions, only two correlate HPV with the histopathological findings of the lesion: immunohistochemistry and in situ hybridization. Consequently, we were prompted to carry out a comparative study on both techniques to check their utility and efficacy as routine diagnostic methods. 52 biopsy specimens of uterine cervix diagnosed histopathologically as condylomas and cervical intraepithelial neoplasia+koilocytosis were studied by immunohistochemical and in situ hybridization techniques using a polyclonal antibody against the common antigen of the HPV capsid and three biotinylated DNA probes specific to HPV types 6/11, 16/18 and 31/35/51. Immunohistochemistry detected 21 positive cases (40.38%), whereas in situ hybridization detected 40 positive cases (76.92%); of the latter, 30 were positive for HPV types 6/11, 3 for HPV types 16/18 and 11 for HPV types 31/35/51. The results suggest that in situ hybridization is a more sensitive technique than immunohistochemistry. However, we recommend the use of both techniques in the case of potentially malignant lesions since better prognostic information can be obtained from joint analysis of both results.

Predominant role for nitric oxide in the relaxation induced by acetylcholine in cat cerebral arteries.

The possible role of nitric oxide (NO) and other endothelial relaxant factors in the vasodilation induced by acetylcholine (ACh) in isolated segments of cat cerebral arteries was analyzed by using the following treatments: 1) the blockers of cyclo- and lipoxygenase, indomethacin and 5,8,11,14-eicosatetraynoic acid; 2) the NO inactivators, phenidone, hydroquinone and oxyhemoglobin; 3) the inhibitors of NO synthesis, NG-nitro-L-arginine methyl ester and NG-monomethyl-L-arginine; 4) the blockers of sodium pump activity, ouabain and K(+)-free medium; and 5) the antagonist of K+ channels, 4-aminopyridine (4-AP). A comparative study between the relaxant actions of exogenous NO and ACh was also performed. The most relevant results obtained were: 1) cat cerebral arteries are very sensitive to the endothelial effects of ACh, as well as to the exogenous NO; 2) ACh-induced endothelium-dependent dilatation is not affected by indomethacin and 4-AP, partially inhibited by 5,8,11,14-eicosatetraynoic acid, phenidone, hydroquinone, oxyhemoglobin and NG-nitro-L-arginine methyl ester and abolished by NG-monomethyl-L-arginine; 3) this latter effect is selectively antagonized by L-arginine, suggesting that the inhibition of NO synthase may be enough to abolish relaxation to ACh; and 4) sodium pump blockade abolished endothelial but not exogenous NO effects. From these results, we conclude that ACh-induced relaxation in these vessels can be entirely mediated by the release of endothelial NO. Although other endothelial factors cannot be discarded, their possible contribution to ACh-evoked relaxation is likely negligible.

Study of infection by human papillomavirus in severe dysplasias and carcinomas in situ of the uterine cervix using immunohistochemistry and in situ hybridization.

Diagnosis of human papillomavirus (HPV) infection in uterine cervical lesions is usually based on histopathological criteria and, in some cases, is confirmed by immunohistochemistry. The recent development of in situ hybridization techniques has facilitated the detection of HPV in these lesions. Consequently, we carried out a study on 18 uterine cervical biopsy specimens histopathologically diagnosed as severe dysplasias and carcinomas in situ, using an immunohistochemical method with a rabbit polyclonal antibody against the HPV common structural antigen and in situ hybridization techniques with three biotinylated DNA probes for HPV types 6/11, 16/18, and 31/35/51. By immunohistochemistry only one case (5.5%) proved to be positive, whereas by in situ hybridization 12 HPV-positive cases were obtained (66.6%), of which 7 were positive for HPV types 16/18 (38.8%) and 6 for HPV types 31/35/51 (33.3%). One case was positive with positive with both DNA probes. From our results it can be inferred that in situ hybridization is a more sensitive technique than immunohistochemistry for confirming the presence of HPV in severe dysplasias and carcinomas in situ of the uterine cervix. Furthermore, in situ hybridization provides much more information than immunohistochemistry since it permits the identification of the HPV types causing the lesion.

[Trophic activity of neurotensin in massive intestinal resection].

Regulatory peptides are among the main factors affecting the proliferative adaptive process of the small bowel; its mechanisms of action remains unknown. An 80% small bowel resection was done in Wistar rats (250-300 g) randomly assigned to two groups: control and neurotensin (300 mu/kg). Jejunal and ileal samples were taken on day 14 post-resection and histomorphometric (villous length) and proliferative (DNA content) measures were obtained. Results show an increase in villous length in the neurotensin group but no changes in DNA content were observed. We conclude that neurotensin increases intestinal mucosal growth after massive intestinal resection in the rat.

Axonal and non-axonal immunolocalization of cytosolic aspartate aminotransferase (cAATase), GABA and glutamic acid decarboxylase (GAD) in the rat cochlear nucleus.

The localization of both cAATase activity, by histoenzymological method, and the immunoreactivity against cAATase, were investigated in the cochlear nucleus of rats. The immunohistochemical determination of cAATase was carried out using the PAP method, with an antiserum obtained from rabbits immunized with porcine cAATase. It was also studied both the immunolocalization of GABA-like and GAD-like substances. Our observations, with light microscope, revealed weak cAATase activity in the small neurons, and a more intense one in the fibers surrounding neuronal bodies. The large neurons presented a very weakly activity within their neuronal bodies and dendrites, but it was strongly found in granulations that surround the perikaryon and dendrites. cAATase immunoreactivity presents the same distribution as the enzymological activity. In the same way, we have investigated the pattern of distribution of both GABA- and GAD-like substances. Immunolocalization of these substances was similar to that found for cAATase. In the control sections incubated with Gostatin (0.05 mM), cAATase activity was absent. The immunoreactivity was also negative in every immunohistochemical control sections. These facts suggest that aspartate could intervene as a co-neurotransmitter or neuromodulator in the rat cochlear nucleus, and that axonic endings could contain cAATase, GABA and GAD. It was also found immunoreactivity against cAATase, GABA and GAD, in neuronal bodies, dendrites and glial processes, in close association with capillary wall. These observations have led us to suggest the possible co-localization and co-release of both GABA and aspartate from synaptic and non-synaptic sites in the cochlear nucleus.

Presynaptic M2-muscarinic receptors on noradrenergic nerve endings and endothelium-derived M3 receptors in cat cerebral arteries.

The muscarinic (M) receptors involved in the vasodilation elicited by acetylcholine (ACh) and in the carbachol inhibition in electrically induced [3H]noradrenaline (NA) release in cat cerebral arteries was investigated. For this, atropine, pirenzepine, AF-DX 116, 4-DAMP, non-specific, M1, M2 and M3 receptor antagonists, respectively, were used. ACh elicited concentration-dependent relaxations up to 10(-6) M which were attenuated by these antagonists; the order of potency (pA2 values) to inhibit the ACh-induced relaxation was: atropine (10.1) 4-DAMP (8.9) greater than pirenzepine (7.6) greater than AF-DX 116 (5.9). The electrical stimulation (200 mA, 0.3 ms, 2 Hz, during 1 min) of these arteries preincubated with [3H]NA caused tritium release which was inhibited by carbachol (10(-6) M). The 4 antagonists attenuated the action of the M agonist; the order of potency (pIC50 values) was: atropine (8.7) greater than 4-DAMP (8.1) greater than AF-DX 116 (7.9) greater than pirenzepine (5.8). The action of McN-A-343, a putative M1 agonist, was also investigated. This agent produced small vasodilator responses and elevated concentrations (5 x 10(-5) M) inhibited the stimulated NA release, which was partially antagonized by atropine (10(-7) M) and pirenzepine (10(-8) and 10(-7) M). These results suggest the existence of M3 and M2 receptors mediating the relaxation induced by ACh and the NA release inhibition evoked by carbachol, respectively.

Improvement of ocular penetration of amikacin sulphate by association to poly(butylcyanoacrylate) nanoparticles.

The main objective of this paper was to investigate the ability of polycyanoacrylate nanoparticles to improve the corneal penetration of hydrophilic drugs. Three different nanoparticle formulations were prepared by changing the nature of the stabilizer agent (Dextran 70000, Synperonic F 68 and sodium lauryl sulphate). The significant influence of the stabilizer type on the particle size, electrophoretic mobility and on the drug loading efficiency was proved. Moreover, the ocular disposition of amikacin was affected by its association to nanoparticles, displaying the most interesting results when Dextran 70000 was employed for preparation of nanoparticles. The increase of the amikacin concentration in cornea and aqueous humour was statistically significant for this nanoparticle formulation with respect to the other formulations and the control solution. The in-vitro release profiles obtained using a dialysis system were similar for all the nanoparticle formulations and for the control solution, indicating that drug molecules are desorbed from the nanoparticles quickly enough to maintain the equilibrium concentration in the dialysis system.

M3-muscarinic receptor mediates prejunctional inhibition of noradrenaline release and the relaxation in cat femoral artery.

The aim of the present study was to analyse the muscarinic receptors involved in the vasodilation elicited by acetylcholine (ACh) and the carbachol inhibition of electrically-evoked [3H]noradrenaline (NA) release in cat femoral artery. For this purpose, the following receptor antagonists were used, atropine, pirenzepine (M1-antagonist), AF-DX 116 (M2-antagonist) and 4-diphenylacetoxy-N-methylpiperidine methobromide (4-DAMP; M3-antagonist). The order of potency (pA2 values) of these drugs at postjunctional level was: atropine (9.7) greater than or equal to 4-DAMP (9.6) greater than pirenzepine (7.2) greater than AF-DX 116 (6.0), and at prejunctional level (pIC50 values) was: 4-DAMP (9.3) greater than atropine (8.5) greater than AF-DX 116 (7.1) greater than pirenzepine (5.9). These findings indicate that the muscarinic receptors mediating the vasodilation induced by ACh and the carbachol inhibition of NA release are of the M3-subtype.

Action of adenosine and characterization of adenosine receptors in human placental vasculature.

1. Adenosine induced concentration-dependent relaxations in segments of human chorionic arteries and veins precontracted with 75 mM K+, which were attenuated by 1-methyl-3-isobutylxanthine (MIX). In the same conditions, L-phenylisopropyladenosine (L-PIA), N-ethylcarboxamideadenosine (NECA) and dibutyryl-cyclic AMP (Db-cAMP) also produced concentration-dependent vasodilations. 2. The order of potency with respect to EC50 was in arteries: NECA greater than or equal to L-PIA greater than Db-cAMP greater than or equal to adenosine, and in veins: L-PIA greater than or equal to NECA greater than adenosine greater than or equal to Db-cAMP. 3. In segments precontracted with 10(-6) M PGF2 alpha, adenosine, NECA and L-PIA induced similar relaxations, which were markedly larger than those found in K+ contracted segments. 4. Db-cAMP (15 min preincubation) inhibited more potently K+ (75 mM) contractions than adenosine. 5. Adenosine reduced Ca addition-induced contractions in depolarized segments exposed to Ca-free medium, as well as 45Ca uptake elicited by 75 mM K+. 6. It is concluded that there exists a similar population of adenosine A1 and A2 receptors in chorionic vessels, which mediate adenosine relaxation. This effect appears to be mainly produced by inhibition of Ca influx, but not by a cAMP-dependent mechanism.

An experimental model of chronic osteomyelitis caused by Escherichia coli treated with cefotaxime.

An experimental model in Wistar rats, of osteomyelitis caused by Escherichia coli, was used to evaluate the efficacy of cefotaxime in two treatment regimens of different durations. Four groups of rats were set up: a group of rats receiving short-term treatment (14 days) with subcutaneous cefotaxime (100 mg bd), killed after 56 days; a control group receiving no treatment, killed after 56 days; a group of rats undergoing long-term treatment (28 days) with subcutaneous cefotaxime as above, killed after 70 days and a control group of rats receiving no treatment, killed after 70 days. Analysis of histopathological and microbiological findings revealed significantly better results in the long-term treatment group. No side-effects were observed during treatment or afterwards.

Pharmacological dissection of Ca2+ channels in the rat aorta by Ca2+ entry modulators.

Ca2+ agonists, Bay K 8644 and CGP 28392, induced concentration-dependent contractions in segments of rat aorta partially depolarized with 15 mmol/l K+. The responses caused by Bay K 8644 were greater than those elicited by CGP 28392. The actions of these Ca2+ agonists were blocked by nifedipine. CGP 28392 and mainly Bay K 8644 increased the contractions induced by different K+ concentrations but did not alter those caused by noradrenaline (NA) and higher K+ concentrations. Bay K 8644 enhanced and nifedipine reduced the contractions elicited by Ca2+ addition in a Ca2(+)-free solution containing K+ (25, 50 or 75 mmol/l). This enhancement was blocked by nifedipine. Nifedipine and verapamil reduced more effectively the responses induced by K+ than those caused by NA; they did not modify contractions evoked by Ca2+ ionophore A23187. They also reduced the increases in K+ contractions elicited by CGP 28392 and Bay K 8644. Bay K 8644 increased 45Ca2+ uptake induced by K+ and did not affect that caused by NA. Nifedipine blocked this increase as well as 45Ca2+ uptake elicited by NA and K+. Basal 45Ca2+ efflux was not modified by verapamil and Bay K 8644. NA and K+ significantly increased this efflux in normal solution, but not in Ca2(+)-free medium. Bay K 8644 enhanced 45Ca2+ efflux induced by K+ in normal medium, whereas it did not affect those elicited by NA and K+ in a solution without Ca2+. This Ca2+ agonist had scant ability to induce Ca2+ efflux in a Ca2(+)-free medium. These results indicate that in this vascular preparation: (1) Ca2+ agonists (Bay K 8644 particularly) have the ability to produce Ca2+ influx (and subsequent contraction) and Ca2+ efflux by activation of voltage-operated Ca2+ channels (VOCs), only when they are preactivated with K+; (2) VOCs and receptor-operated Ca2+ channels (ROCs) appear to constitute two different populations, and (3) Ca2+ antagonists possess, in comparison with Ca2+ agonists, a small range of selectivity to block only VOCs; the latter drugs are better to pharmacologically discriminate between VOCs and ROCs in the rat aorta.

Cutis marmorata telangiectatica congenita and glaucoma.

A case of cutis marmorata telangiectatica congenita (CMTC) is described in association with congenital glaucoma. Approximately 68 cases of CMTC and only four cases associated with glaucoma have been reported. The type of glaucoma seems to be related to an abnormality of the filtration in the anterior angle, due to increased pressure in the episcleral vessels. The surgical treatment is trabeculectomy, and opening an alternative drainage pathway.

Vasoconstrictor effect of the Ca2+ agonist Bay K 8644 on human cerebral arteries.

The effects of Bay K 8644 on the reactivity and 45Ca2+ uptake in segments from human cerebral arteries were studied. Bay K 8644 induced concentration-dependent contractions up to 10(-6) M; 10(-5) M produced a reduction of the maximal response. The Ca2+ agonist elicited these contractions by itself, and no previous depolarization was needed. The response to Bay K 8644 was antagonized competitively by nifedipine (5 x 10(-8) and 10(-7) M, pA2 value of 8.17) and non-competitively by verapamil (10(-6), 5 x 10(-6) and 10(-5) M). The contraction induced by 10(-7) M Bay K 8644 was inhibited by a Ca2+-free medium containing 1 mM EGTA. The subsequent cumulative Ca2+ addition, caused concentration-dependent contractions up to 2.5 mM Ca2+, which were reduced by nifedipine (10(-8) and 10(-7) M) or verapamil (5 x 10(-6) and 10(-5) M). When the EGTA concentration in the Ca2+-free solution was reduced to 0.1 mM, contractions induced by Ca2+ up to 5 mM, including 0 Ca2+, were increased with respect to those obtained in the presence of 1 mM EGTA. Basal 45Ca2+ uptake was not modified with Bay K 8644 (10(-6) M) or nifedipine (10(-6) M). K+ (25 and 50 mM) produced an increase on 45Ca2+ uptake, which was potentiated by Bay K 8644 (10(-6) M) and antagonized by nifedipine (10(-6) M); this latter agent reduced the potentiation elicited by the Ca2+ agonist.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of the Ca agonists Bay K 8644 and CGP 28392 on vascular smooth muscle tone.

1. Nifedipine (5 x 10(-9) M) reduced and the Ca agonists CGP 28392 (10(-6) M) and Bay K 8644 (10(-7) M) increased the contractions elicited by K+ in segments of rat aorta, whereas those caused by noradrenaline (NA) were unaffected. 2. CGP 28392 and Bay K 8644 evoked concentration-dependent contractions in segments moderately depolarized with 15 mM K+; these responses were reduced by nifedipine. 3. 45Ca uptake induced by 50 mM K+ or NA (10(-7) M) was reduced by nifedipine (10(-6) M). Bay K 8644 (10(-6) M) only increased the 45Ca uptake induced by K+, which was inhibited by nifedipine (10(-6) M). 4. These results indicate that in this vascular preparation: (a) Ca agonists only activate voltage-dependent Ca channels (VDCs), the potency of Bay K 8644 being higher than of CGP 28392, and (b) VDCs and receptor-operated Ca channels have different pharmacological properties.
