Prospective double-blind clinical trial of bovine, human, and rabbit thromboplastins in monitoring long-term oral anticoagulation.

A prospective double-blind trial was performed to investigate the safety of long-term oral anticoagulant control when dosaged adjustment depends on the prothrombin times assessed with one type of thromboplastin being calculated as if assayed by another type. The three main types of thromboplastin, i.e., bovine (Thrombotest), human (British Comparative Thromboplastin), and rabbit (Simplastin Automated) were tested. Before entering the actual trial, patients had been checked with the bovine type of thromboplastin. During the actual comparison study, prothrombin times obtained with the human and the rabbit types of thromboplastin were translated, using the appropriate calibration data, into terms of the bovine type of thromboplastin, which were used for dosage prescription. After six months, all patients returned to bovine-type thromboplastin control. No substantial difference among the three groups was observed in the prothrombin times, the dosage of the anticoagulant, the bleeding complications, and the dropouts. It was concluded that the safety of monitoring patients receiving long-term oral anticoagulation was independent of the type of thromboplastin used.

Coagulation changes during management of fetal death with 15(s)-15-methylprostaglandin F2 alpha.

Changes in the coagulation mechanism were studied during and after 15(S)-15-methylprostaglandin F2 alpha (15-me-PGF2 alpha) administration for termination of pregnancy because of intrauterine fetal death after 20 wk pregnancy. 12 patients, of whom 2 were under and 6 over 28 wk, were studied. 2-Hourly intramuscular administration of 250 microgram 15-me-PGF2 alpha resulted in expulsion in a median time of 9 h (range: 2-24.3 h). Although the drug may have some inhibitory effect on platelet aggregation, its influence on coagulation and the hemostatic mechanism was negligible.

Successful prevention of coumarin-induced hemorrhagic skin necrosis by timely administration of vitamin k1.

Successful prevention of the progression of incipient hemorrhagic skin necrosis by timely administration of vitamin K1 in a woman treated with phenprocoumon is presented. From a critical review of the literature strong evidence emerges that coumarin necrosis does only occur in cases with severe initial drug induced hypocoagulability. Non- recognition thusfar of its importance is due to insufficient knowledge of the biological activities of thromboplastin preparations presently used in the laboratory control of oral anticoagulation. All well documented cases with apparently adequate Quick values were monitored with Faktor VIII insensitive thromboplastin. Therefore, such preparations should no longer be used in anticoagulant control.
