[Vasculitis complicating treatment with streptokinase: delayed allergic reaction].

The authors report a case of vascular purpura occurring eleven days after the injection of streptokinase for a myocardial infarction. This adverse event falls within a context of hypersensitivity vasculitis (type III). In this particular case, imputation to the thrombolytic agent was established by the official method for the imputation of adverse or toxic effects of drugs. Routine renal function studies were normal. The outcome was satisfactory following rest only but the existence of a mixed cryoglobulinemia, responsible secondarily for cold-related distal ischemic symptomatology of the lower limbs led to the successful prescription of a short course of corticosteroids.

[Mediterranean boutonneuse fever. Apropos of 154 recent cases].

The Mediterranean spotted fever is always present in the south of France. The actual incidence is unknown. The disease appears in summer. The diagnostic is based on the association of fever, "black spot" and exanthema and/or a seroconversion. Indirect immunofluorescence is the most used technique. Some patients have severe complications: neurologic, cardiovascular, renal, thrombopenia. These cases look like Rocky Mountain Spotted Fever. The treatment is based on tetracycline.

[Cutaneous side-effects in drug assessment].

Drug-induced cutaneous side-effects are very frequent. All dermatitis may be induced by drugs. It is difficult to identify the drug responsible because patients take many drugs (prescriptions, self medication). For each pathology, physicians may use assessment methods based on specific chronological criteria (challenge, dechallenge, rechallenge) and specific semeiological criteria (promoting factors, alternate non-drug related explications, specific laboratory tests) to identify the drug responsible.
