[4 cases of Guillain-Barré syndrome with axonal lesions].

We report the neurological features observed in 4 patients who developed a clinically typical Guillain-Barré syndrome. Electrophysiological and histological data displayed an unusual number of axonal lesions, although these were always associated with demyelination. These cases must be distinguished from the rare cases of Guillain-Barré syndrome with pure axonal lesions. Three patients had a poor functional recovery. This confirms the more severe prognosis of Guillain-Barré syndrome with major axonal degenerative changes.

Kainic acid induces early and delayed degenerative neuronal changes in rat spinal cord.

Intrathecal injections of kainic acid and sodium chloride were performed in rats to study the cellular modifications observed in spinal cord. Early neuronal changes (2 and 24 h) associated dark and shrunken or swollen and vacuolated cytoplasms. Delayed (3, 6, 14 days) changes, mainly consisted in degenerative aspect of motoneurons with eccentric and indented nucleus, swollen cytoplasms with proximal neurite enlargements, the presence of 'lipofuscin-like' pigments, disorganized intracytoplasmic organelles and filament accumulations.

[Toxoplasma gondii: ultrastructural study of cystic formations observed in human fibroblast culture].

A strain of Toxoplasma gondii, non pathogenic to mice (Prugniaud strain) produced cyst-like stages when inoculated in human fibroblast culture. An ultrastructural study was performed in order to compare these cyst-like forms to brain cysts of the same strain and to intracellular clusters of tachyzoites observed in mice TG 180 sarcoma cells. Electron microscopy confirmed the similarities between brain cysts and cysts obtained in vitro.

Effects of glycerol injection into rat sciatic nerve.

In recent years, injection of pure glycerol into the trigeminal cistern has been used for the treatment of trigeminal neuralgia. The mechanism of action of this therapy remains unclear. Using both light and electron microscopy, we investigated the effects of microinjections of sterile, pure glycerol into the endoneurium of the sciatic nerve of the rat. We observed total destruction of both myelinated and unmyelinated fibers. In nearly all animals, signs of automutilation were observed in the paralyzed limb. Histological evidence of nerve degeneration appeared soon after injection, with intense proliferation of perineurial cells that eventually divided the endoneurium into numerous microcompartments.

[Extent of idiopathic necrosis of the femoral head. Assessment using computerized tomography. Anatomo-radiologic correlation].

The indications for treatment of idiopathic necrosis of the hip depend on the stage of development of the lesion, its extent and the site of the necrotic area. Digital computerised tomography with horizontal sections at five millimetre intervals from the upper to the lower pole of the femoral head is of value in the determination of these three features. For this purpose, fifteen femoral heads removed in the course of total hip arthroplasty were subjected to histological sections in parallel to those of the CT scan. This study showed that digital computerised tomography indicates the extent and topography of the necrosis and provides true histo-radiological sections.

Cytotoxic properties of glutamate and aspartate in rat peripheral nerves: histological findings.

Two excitatory neurotransmitters, L-glutamate and L-aspartate, known for their neurotoxic properties in the central nervous system were injected in rat sciatic nerve. The morphological results are compared with sodium chloride injections in sciatic nerve on the other side. Rats were sacrificed 24 h to 1 month after the administration of compounds. Marked endoneural edema with global axonal loss of myelinated fibers were observed in animals injected either with glutamate or aspartate. A demyelinating process was also associated with axonal damages. Sodium chloride did not induce similar morphological abnormalities. This model of toxic neuropathy may be mediated by the excitotoxic properties of L-aspartate and L-glutamate.

Congenital hypo- and hypermyelination neuropathy. Two cases.

Two young patients were referred recently to the authors for investigation of a peroneal atrophy syndrome. Since the first symptoms were observed in infancy, a congenital hypomyelination neuropathy was suspected, and superficial peroneal nerve biopsies were taken. Signs of severe and widespread demyelination/remyelination were observed. These features appeared morphologically similar to those observed in the globular or tomaculous neuropathies. The mechanism of the hypermyelination is discussed.

Analysis of unsaturated fatty acids of endoneurium and perineurium from normal and degenerating rat sciatic nerve. Morphological correlations.

The major portion of the endoneurial lipids is found in myelin. Since perineurial cells differ morphologically from endoneurial cell components, we attempted to determine whether these morphological differences also extended to a difference in fatty acid (FA) composition. Under normal circumstances, unsaturated FAs are more abundant than saturated ones (55-60% of total FAs) in endoneurium and perineurium. A characteristic biochemical difference between these two structures lies in the distribution of linoleic acid (C18:2(n-6)) which represents 20% of total FAs in perineurium and only 2% in endoneurium. Wallerian degeneration takes place after injection of pure glycerol into the endoneurium. This is followed by regeneration characterized by a proliferation of perineurial cells infiltrating the center of the nerve fascicule forming microcompartments. The changes in linoleic acid content reflect these morphological changes. A marked increase in linoleic acid is detected in the endoneurial fraction in parallel with the observed infiltration of perineurial cells into the nerve fascicule.

Tick-bite meningoradiculoneuritis: clinical, electrophysiologic, and histologic findings in 10 cases.

We studied 10 adults with meningoradiculitis several weeks after a tick bite (Ixodes). EMG evidence of denervation was associated with normal motor conduction velocities, prolongation of distal latencies, and low sensory amplitudes, suggesting axonal neuropathy. Sural nerve biopsies confirmed the axonal involvement. Infiltrations of lymphocytes and plasma cells, sometimes forming thick pericapillary cuffs with no accompanying necrosis of vessel walls, were numerous. Many capillaries of the endoneurium, perineurium, and epineurium were affected in this way.

Acute pure sensory paraneoplastic neuropathy with perivascular endoneurial inflammation: ultrastructural study of capillary walls.

We studied a patient with epidermoid carcinoma of the lung (treated surgically 1 year earlier) and an acute symmetric pure sensory neuropathy that regressed almost completely within 1 month. Superficial peroneal nerve biopsy 15 days after onset showed evidence of demyelination with perivascular endoneurial inflammation. On ultrastructural examination, lymphocytes were seen passing through endothelial cells of endoneurial capillaries.

[Surgical excision of meningioma of the posterior fossa in a dog after detection by x-ray computed tomography].

Following the progressive occurrence of a central vestibular syndrome in a 9 year old female dog, the diagnosis of a compressive lesion was confirmed by cerebrospinal fluid analysis (C.S.F.) and by computerized axial tomography (C.T.). The C.T.Scan revealed a large tumor occupying two thirds of the posterior fossa on the right side. A direct surgical approach to the right side of the posterior fossa revealed a large, friable mass, consistent with a meningioma, underneath the cerebellum, over the brain stem. Microscopic surgical dissection permitted the removal of the major part of the tumor. The nature of the intracranial neoplasm was confirmed histopathologically as a meningioma. No post operative complication arose and the dog returned to normal. Three months later, clinical signs reappeared and a control C.T.Scan showed recurrence of the tumor. The dog was euthanized four months postsurgically. Intracranial tumors are common in the dog, but rarely diagnosed and operated. However, a direct surgical approach to the posterior fossa for a tentative curative removal of an intracranial tumor has never been described in a dog. The localization and description of the lesions demonstrate the utility of C.T.Scans in the canine species. Recurrence of the tumor following surgery confirms the importance of complete excision of intracranial meningiomas when possible.

Case of dementia and myoclonia in an adult associated with anomalies in polyunsaturated fatty acids in leukocytes and peripheral nerve. An ultrastructural study of peripheral nerve.

We report a case of a 66-year-old patient presenting with abnormal movements and associated dementia. Death occurred 4 years after the onset of symptoms. In spite of the lack of autopsy results, the picture was one of late onset neuronal ceroid lipofuscinosis (Kufs' disease). Ultrastructural study of a peripheral nerve biopsy sample indicated a process of demyelination associated with unusual inclusions in Schwann cell cytoplasm. Biochemical analysis of the same sample and leukocytes showed considerable alterations in polyunsaturated fatty acid levels. These findings are discussed in the light of work on cases of infantile neuronal ceroid lipofuscinosis.

[Evaluation of a new latex test for detecting human rotaviruses in feces].

We have sought the presence of rotavirus in 237 fecal samples. Three techniques were applied to each sample: ELISA, latex agglutination (LTX) with two reagents (Rotalex and SlidexRotaKit), and electron microscopy after negative staining. The specificity of ELISA and LTX is very good (more than 93.4 per cent), the sensitivity is decreasing from ELISA to SlidexRotaKit and Rotalex (respectively 83.6-74.5 and 67.3 per cent). The advantages of LTX include rapidity, simplicity, low cost price.

IgM demyelinative neuropathy with amyloidosis and biclonal gammopathy.

A 59-year-old man developed a sensorimotor neuropathy of the upper and lower limbs, associated with a biclonal gammopathy, within the space of a few months. Each of two paraproteins was coupled with a distinct IgM kappa IgG lambda light chain. Examination of a nerve biopsy specimen by electron microscopy revealed a demyelinative process with a widening of the interlaminar space in the myelin sheath, as well as deposits of amyloid substance between nerve fibers. Direct immunofluorescence revealed the presence of IgM and of the kappa light chain in certain Schwann cells, while the lambda IgG was fixed to the amyloid deposits. Immunoperoxidase histochemistry showed a positive reaction in normal human nerve tissue to the immune serum IgM and kappa light chain. The findings suggest that the widening of the interlaminar space of the diseased myelin corresponds to an active fixation of immunoglobulin on the sheath of the Schwann cell. The presence of two light chains in this patient's gammopathy caused a dual pathology: the kappa chain, a demyelinative neuropathy, and the lambda chain, a primary amyloidosis, with deposits in the peripheral nerve and in the kidney.

Chronic demyelinating neuropathy with IgM-producing lymphocytes in peripheral nerve and delayed appearance of "benign" monoclonal gammopathy.

A chronic demyelinating neuropathy with "benign" IgM gammopathy was followed for 6 years in a 63-year-old man. The clinical, biologic, and EMG aspects were similar to those already reported, but a lymphoplasmocytic infiltrate in the nerve connective tissue of this patient has only rarely been observed in benign IgM gammopathy. The paraprotein was not evident in the serum until 5 years after symptoms of the neuropathy started.
