Immediate breast reconstruction revisited.

Immediate breast reconstruction was discussed from the psychological, oncological and surgical points of view. It has been found that at present no adverse effects of the procedure on the course of breast cancer are known, while immediate timing is connected with significant psychological and surgical benefits.

Stress urinary incontinence in women--an overuse syndrome.

The levator ani muscles in women are overloaded by constant supporting abdominal contents, and counteracting rises of abdominal pressure at the level of the urethra. The excessive load leads to a decrease in their type II fibers. This results in stress urinary incontinence due to loss of the fibers responsible for additional compression of the urethra at strain.

[Uroflowmetry in diagnosis of urinary stress incontinence in women].

The authors' aim was to evaluate the range of uroflowmetry in diagnostics of stress urinary incontinence (sui) in women. The examinations were carried out in 59 women with sui, aged from 35 to 45 years (group I). The control group consisted of 20 asymptomatic women (group II). To evaluate volumes of urine voided during micturition in a unit of time a urological flowmeter UF-1 (produced by COTM, Białystok) was used. Numeric data of flow were analysed, as well as registered curves of flow were ascribed to one of the following flow patterns: normal, multi-peak or interrupted. The conducted studies revealed statistically significant differences in flow values in group I, compared to group II. A more frequent incidence of multi-peak and interrupted flow patterns was found in women with sui, which could be related to a neurogenic component in etiology of the disease.

Cord blood triglyceridemia in cases of placental insufficiency.

Cord blood triglyceride concentration increased in cases of placental insufficiency. The significant relationship has been found between elevated triglyceride, decreased Apgar score, and low pH values. We suggest that the measurement of cord blood triglyceridemia might be used as a valuable indicator of chronic fetal distress.

[Degradation and consumption of one's own glycogen in placental tissue in pregnancy complicated by gestosis and anemia].

We estimated in vitro degradation and consumption of glycogen by placental tissue derived from pregnancies complicated by gestosis and anemia. Placental tissue was incubated in suitable medium. Glycogen concentration either before or after incubation was assayed. Glycogen consumption was expressed as micromoles of glucose per 1 g of wet tissue, the percentage of glycogen consumption has been shown in tables. It is concluded that in advanced gestosis and severe anemia of pregnancy the consumption of native glycogen is increased, consequently it may lead to metabolic insufficiency of human placenta.

[Effect of diseases in pregnancy on certain placental functions. Enzymatic systems degrading placental glycogen in pregnancy with gestosis].

Pathways of human placental glycogen degradation either in normal or gestotic pregnancy were examined. Determinations of glycogen phosphorylase served as an index of glycogen cleavage in the phosphorylitic pathway. The activity of hydrolytic route was measured by estimating placental glucoamylase. Samples of placental tissue were obtained after delivery (between the 36th and the 40th week of pregnancy). In placentas derived from gestotic cases elevated activity of phosphorylase A, the decrease in phosphorylase B activity and the rise in glucoamylase were found. It suggests that gestosis may evoke certain impairment of placental glycogen metabolism deteriorating chiefly the glycogen degradation as it may be observed in hypoxic experiments.

[Placental function in pregnant women with anemia].

The important finding in anemia of pregnancy is impairing hormonal placental function, certain complications of pregnancy and labor. Oxytocinase in the maternal serum was determined to evaluate placental function. Many authors pointed out the important role of the enzyme in biochemical monitoring of human pregnancy. 120 females at the III trimester of gestation were selected at random, but women with additional complications of pregnancy were excluded. Serum iron, total iron binding capacity, hemoglobin level, red blood cells count and hematocrit were assayed. Oxytocinase activity in different periods of gestation was compared to the earlier established normal values. It was found that iron deficit was associated with decreasing activity of the enzyme. It is concluded that there is impaired placental function in pregnancy anemia.

Umbilical cord gamma-glutamyl transpeptidase and placental dysfunction.

Placental dysfunction has been suspected if human placental lactogen level and/or cystine-aminopeptidase activity were lower than 10th centile. Significant rise in gamma-glutamyl transpeptidase (GGTP) has been found in samples of arterial cord blood of newborns born to mothers with placental insufficiency. We observed the relationship between GGTP activity and decrease in cord pH and lower Apgar score of corresponding infants. We suggest that chronic hypoxemia as a consequence of placental dysfunction may result in damage of fetal liver cells.

Acid-base status of the newborn in relation to cord blood serum creatine kinase isoenzymes' activities.

Creatine kinase (CK) isoenzymes are considered to be specific tissue injury markers. The purpose of this study was to evaluate CK isoenzymes (CK-MM, CK-MB, and CK-BB) in umbilical cord blood sera of newborns in relation to their acid-base status. Investigations were performed in a group of 75 newborns delivered after 37 weeks of pregnancy. We estimated CK-isoenzymes with the use of DEAE-Sepharose C1-6B column chromatography. Total CK activity was measured using kits supplied by Boehringer-Mannheim (Monotest-R CK-NAC aktiviert). Newborns were considered hypoxic if umbilical artery blood pH was under 7.2 and/or base excess less than -10 mmol/L. Of 75 newly born infants 26 had features of perinatal hypoxia. We were unable to demonstrate significant differences in total CK, CK-MM and CK-MB activities in examined groups of newborns. We found a significant rise of CK-BB activity in cord sera of hypoxic infants. Our results show that the fetal brain is the most vulnerable to a hypoxic state.
