Androgen-producing bilateral large cortical adrenal adenomas associated with polycystic ovaries in a young female.

An association of bilateral large adrenocortical androgen-producing adenomas with polycystic ovaries in a young female is presented. She developed mild hirsutism and secondary amenorrhoea at the age of 17, and was treated for 3 years with an anti-androgen (cyproterone acetate) and ethinyloestradiol. Routine follow-up at the age of 21 showed bilateral large adrenal tumours and polycystic ovaries, together with high serum testosterone and dehydroepiandrosterone sulphate values. Bilateral adrenalectomy was performed, which resulted in lowering of the elevated androgens, and large bilateral adrenocortical adenomas were confirmed histologically. Contrary to expectations, the polycystic appearance of the ovaries persisted after adrenalectomy. This case supports the possible role of adrenal androgens in the pathogenesis of polycystic ovaries as well as the possibility of the persistence of polycystic ovaries without adrenal androgens once they have developed.

Adenoma

[Transfusion of leukocytes and platelets in the treatment of acute leukemia].

Supportive therapy of concentrated leukocytes and platelets has no obvious significance. In the paper authors presented the methods of separation of concentrated leukocytes and platelets, investigation of number and functional abilities of preserved leukocytes and platelets as well as the monitoring of the transfusion effect in the patients with acute myeloblastic leukemia. Special attention was paid to the separation of blood cells by the cell centrifuge with continuous flow. Side effects after leukocytes and platelet transfusion was monitored -- occurence of the sensibilization to the HL-A antigens. For the detection of HL-A antibodies two-stage microlymphocytotoxic test was used for the investigation of patient's serum; for the application of leukocyte and platelet transfusion we used cross-match test between donor's lymphocytes and patient's serum in progressive dilution. In the paper authors discussed the correlation between the appearance of HL-A antibodies and the number of given transfusions.

Erythropoietin level in patients with acute leukaemia.

Erythropoietin level in the serum and urine of adult patients with acute leukaemia (AML, ALL, MML) was estimated by polycythaemic mouse bioassay in order to obtain more information about the associated anaemia. In AML and ALL patients the serum erythropoietin level as found to be increased and in a negative correlation with the blood haemoglobin concentration. In ALL patients erythropoietin in urine was increased regularly while in AML patients it was not. No correlation between the serum level and the urinary excretion of ESF, or between the blood Hb and the serum ESF, was found in MML patients. The results show that anaemia in leukaemia is not due to the low ESF level.
