[Study of psychopathologic-biorhythm aspects of alcoholic patients in the withdrawal phase--initial results of a pilot study].

There are some works on the psychopathological-biorhythmical situation of alcoholics in their stages of abstinence. Especially the subjectively experience sleeping behaviour was only seldom assessed. The aim of the present examination was to differentiate different types of the course of the postacute abstinence phase. 30 hospitalized alcoholics whose acute withdrawal symptoms were eased took part in prospective study. By means of a test battery the state of mental abilities and sleeping behaviour were registered. The first results point out to a typing of the postacute abstinence phase whose clinical relevance still need further investigations.

Multiple system atrophy: natural history, MRI morphology, and dopamine receptor imaging with 123IBZM-SPECT.

Sixteen patients with a clinical diagnosis of probable multiple system atrophy (MSA) were examined clinically by MRI and by 123I-iodobenzamide single photon emission computed tomography (IBZM-SPECT). The clinical records of another 16 patients were also analysed retrospectively. On the basis of their clinical presentation, patients were subdivided into those with prominent parkinsonism (MSA-P, n = 11) and those with prominent cerebellar ataxia (MSA-C, n = 21). Autonomic symptoms were present in all patients and preceded the onset of motor symptoms in 63% of patients. Calculated median lifetime and the median time to become wheelchair bound after onset of disease were significantly shorter for MSA-P than for MSA-C (lifetime: 4.0 v 9.1 years; wheelchair: 3.1 vs 5.0 years) suggesting a better prognosis for cerebellar patients. A significant loss of striatal dopamine receptors (below 2 SD threshold) was detected by IBZM-SPECT in 63% of the patients (56% below 2.5 SD threshold). There was no difference between patients with MSA-C and those with MSA-P in the proportion with significant receptor loss and the extent of dopamine receptor loss. Planimetric MRI evaluation showed cerebellar and brainstem atrophy in both groups. Atrophy was more pronounced in patients with MSA-C than in those with MSA-P. Pontocerebellar hyperintensities and putaminal hypointensities on T2 weighted MRI were found in both groups. Pontocerebellar signal abnormalities were more pronounced in MSA-C than in MSA-P, whereas the rating scores for area but not for intensity of putaminal abnormalities were higher in MSA-P. MRI and IBZM-SPECT provide in vivo evidence for combined basal ganglia and pontocerebellar involvement in almost all patients in this series.

Another visual illusion involving orientation.

The orientation of a grating may be misperceived when that grating appears superimposed within one-dimensional visual noise of a different orientation. An adjustment procedure was employed to measure the size of this tilt illusion and its dependence on noise orientation. The illusion is maximum when the noise and test differ by 20 to 40 deg, depending on the spatial frequency of the test grating. Noise composed of spatial frequencies several octaves removed from the test spatial frequency can still cause illusory tilt, although the degree of tilt under these conditions depends on test spatial frequency. This orientation illusion, like the tilt aftereffect, simultaneous tilt illusion and angle expansion illusion, may reflect inhibitory interactions between orientation selective neurons in human vision.

A simple superimposition method for anatomical adjustments of CT and SPECT images.

A technique for superimposition of CT and single photon emission CT (SPECT) images is described. Regions of interest can be placed on SPECT images and projected to CT images. In this way, anatomical registration of regions of increased or decreased activity is possible. The equipment is easy to handle and available at low cost.

[Fluorine-18]fluorodeoxyglucose positron emission tomography, DNA ploidy and growth fraction in squamous-cell carcinomas of the head and neck.

BACKGROUND: Positron emission tomography (PET) offers an opportunity to examine noninvasively cellular functions with different tracers. [(18)F]Fluorodeoxyglucose (FDG) is most commonly used in identifying malignant tumors. Several tumor biologic characteristics (tumor cell viability, growth faction, treatment response to radiation, cell membrane dysfunction, recurrence rate) are suggested to be characterized by [(18)F]FDG PET. The aim of this study was to assess which other tumor biologic characteristics of squamous-cell carcinoma of the head and neck are correlated with [(18)F]FDG PET. METHODS: [(18)F]FDG PET was performed in 14 patients with squamous-cell carcinomas of the upper digestive tract (TNM classification T(2)-T(4), N(1)-N(3)). After attenuation correction, predefined areas of the tumor were semiquantitatively analyzed by the technique of the region of interest and calculated as standard uptake values (SUV). Afterwards, 5 biopsies of different tumor regions were obtained during endoscopy in each patient under general anesthesia, and a correlation between SUV of [(18)F]FDG PET and tumor biologic parameters was attempted. These parameters included: quantitative DNA measurements (i.e. 2c deviation index, 5c exceeding rate), immunohistochemical assessment of growth fraction (i.e. Ki67-MIB-1, PCNA) along with morphological tumor front grading. RESULTS: The results revealed a marked variation of proliferation and cellular differentiation in various regions of the tumor for all parameters examined. There was a close correlation between [(18)F]FDG uptake and growth fraction (r = 0.83 for Ki67-MIB-1 and r = 0.8 for PCNA). A poor correlation was found between DNA aneuploidy (r = 0.4) or tumor front grading (r = 0.12) and [(18)F]FDG uptake. CONCLUSIONS: Our results confirm previous clinical and histologic observations that squamous-cell carcinomas of the upper digestive tract are heterogeneous tumors. Ki67 antigen, which has been shown to be of predictive value for proliferation and individual prognosis, correlated with [(18)F]FDG uptake. Using [(18)F]FDG PET, the main proliferation centers of inhomogeneous squamous-cell carcinomas could be identified with possible clinical implications for patient management.
