Current concepts in gene therapy of the musculoskeletal system.

The purpose of this article is to review the remarkable progress in the field of musculoskeletal system gene therapy. Since the introduction of this concept 15 years much of the preclinical and clinical data have emerged. The original target, rheumatoid arthritis, has been subjected to clinical phase II efficacy protocol, and osteoarthritis gene therapy efficacy is being thoroughly investigated in various animal models. The most promising area of research in this field however, is the tissue repair, because it doesn't require prolonged period of gene expression, local delivery is reasonably simple and it avoids substantial risk associated with systemic delivery, and levels of gene expression don't need to be so finely regulated. Gene transfer is successfully being used to aid the repair and regeneration of bone, cartilage, ligament tendon, meniscus and intervertebral disc. Other potential applications of gene therapy in musculoskeletal system include osteoporosis, aseptic loosening, genetic diseases and tumors. Highly encouraging data gained from these studies have confirmed that gene therapy is a promising therapeutic solution to treat various musculoskeletal system disorders.

Vertical profile of 137Cs in soil.

In this paper, a vertical distribution of 137Cs in undisturbed soil was investigated experimentally and theoretically. Soil samples were taken from the surroundings of the city of Kragujevac in central Serbia during spring-summer of 2001. The sampling locations were chosen in such a way that the influence of soil characteristics on depth distribution of 137Cs in soil could be investigated. Activity of 137Cs in soil samples was measured using a HpGe detector and multi-channel analyzer. Based on vertical distribution of 137Cs in soil which was measured for each of 10 locations, the diffusion coefficient of 137Cs in soil was determined. In the next half-century, 137Cs will remain as the source of the exposure. Fifteen years after the Chernobyl accident, and more than 30 years after nuclear probes, the largest activity of 137Cs is still within 10 cm of the upper layer of the soil. This result confirms that the penetration of 137Cs in soil is a very slow process. Experimental results were compared with two different Green functions and no major differences were found between them. While both functions fit experimental data well in the upper layer of soil, the fitting is not so good in deeper layers. Although the curves obtained by these two functions are very close to each other, there are some differences in the values of parameters acquired by them.

Arthroplasty in treating knee osteoarthritis and proximal tibia stress fracture.

The authors describe two cases of severely angulated tibial non-unions after proximal tibia stress fractures associated with ipsilateral osteoarthritis treated with modular knee endoprosthesis with a long tibial stem to stabilize non-union fragments. During the procedure, no additional osteosynthesis or bone grafting was added. Both non-unions healed after 6 months with no post-operative complications. The authors suggest implantation of modular knee endoprosthesis, as a single procedure, in treatment of proximal tibia non-union after a stress fracture as a result of severe varus/valgus deformity. It provides a solution for osteoarthritis treatment, axis correction and non-union osteosynthesis.

Experience in prosthetic supply of patients with lower limb amputations in Croatia.

This study analyses some demographic characteristics of the amputees in Croatia, reason and level of amputation, care the amputees receive, the first prosthetic supply and functional level after rehabilitation. Anonymised data on all amputees in the Clinical Institute for Rehabilitation and Orthopaedic Aids in Zagreb (a national centre) were collected during the year 2000. Follow-up was undertaken for one year. As a result, the authors analysed 221 patients, classified into 3 groups according to the level of amputations: trans-tibial, except foot (TT), trans-femoral (TF) and bilateral amputations (bilateral). Among the population there were: 76% men and 24% women with average age of about 62 years (the average age of women was 8 years more than men). The most common diseases that resulted in amputation were: diabetes mellitus (DM) 48.9%; obstructive vascular diseases (OD): occlusive peripheral arterial disease, Buerger's disease and atherosclerosis 27.1%; trauma 11.3%; both OD and DM 7.2%; osteomyelitis (OM) 3.2% and tumours (TM) 2.3%. Average period from the amputation to admission for prosthetic supply was over 190 days but the average period from admission to discharge from the Institute was about 40 days. Prosthetic supply was accompanied by certain complications: flexion contractures of neighbouring joints, knee 37.9% and hip 35.2%; local complications of soft tissues: necrosis, wound dehiscence, soft tissue surplus, ischaemic tissue damage (the most frequent in TT amputation 35.9%) and phantom pain (55.7% of patients). Hours of daily use of the prosthesis at discharge was about 5 hours. Greater independence in fitting and removing the prosthesis was observed in patients with TT amputation (86.4%). In conclusion, it can be said that the time between the amputation and the beginning of the prosthetic supply, mobility at the time of admission, frequency of general and local complications and number of days in prosthetic supply, are very important for the result of rehabilitation.

Medical rehabilitation in Croatia impact of the 1991-1995 war: past problems, present state, future concerns.

AIM: To assess whether the 1991-1995 war has stimulated development of the medical rehabilitation system in Croatia. METHODS: Review of documents pertaining to the system, study of clinical reports describing rehabilitation activities as published in the Croatian medical literature, study of data obtained and their consideration in terms of effectiveness and quality of rehabilitation care, and comparison of data from 1991 with those from 1995. RESULTS: There has been no significant change in the number of rehabilitation facilities, beds, and rehabilitation professionals. However, elements of structure and process of rehabilitation care have improved in respect of 1) education and composition of rehabilitation professionals, 2) availability of specialized facilities for rehabilitation of patients with complex impairments (traumatic brain and spinal cord injuries), 3) interdisciplinary team approach, 4) use of functional status measurements, and 5) laying the foundations for community-based rehabilitation in the country. CONCLUSION: The 1991-1995 war has stimulated the development of medical rehabilitation system in Croatia. Other factors may have played a complementary role, too. This proves that medical rehabilitation is a field that develops in association with war.

Treatment of defect pseudoarthroses with bone segment transport.

PURPOSE OF THE STUDY: The most severe injuries during defense war in Croatia were caused by projectiles of high kinetic energy. Shell fragments bone they break the bone, transferring bone segments into additional individual projectiles. During this process periostal and endomedullary circulation is cut off reducing to a minimum the possibility of healing, leading to defects in long bone diaphyses. These are referred to as defect pseudoarthroses and are among the most difficult problems in reconstructive surgery of the musculo-articular system. This article presents bone segment transport as a method of choice for treating defect pseudoarthrosis. MATERIAL: During the defense Croatian war, in six-years period, 62 wounded persons were treated with Ilizarov method of bone segment transport. Among them, only one was female. Average age was 33 years. METHODS: The authors performed modified Ilizarov's method as standard procedure for bone segment transport in treatment of defect pseudoarthroses. After all soft tissues are held, corticotomy in metaphysis is performed and free bone segment transport started with Ilizarov method. Physical therapy provided adequate mechanical load, essential for bone healing and restoring the functionality of injured extremity. RESULTS: In the group of treated patients, the largest bridged defect was 20 cm, and the smallest one was only 1 cm. Infection arises in 20 of 60 patients. Contracture complications were ankle contractures, knee contractures and one elbow contracture. We also found one n. peroneus palsy and one limb amputation should be performed. DISCUSSION: Ilizarov method of bone segment transport presents the only procedure that could successfully and completely treat pseudarthroses, thus avoiding necessaries for limb amputations. Requirements in this procedure are experienced and highly qualified surgical team and high motivation and compliance of the patient.

[Overuse injury syndromes of the hand, forearm and elbow].

The basic role of the shoulder, upper arm, elbow, forearm, and wrist is to place the hand in the appropriate position, so that it can fulfill its function. The upper extremity, especially the wrist and the hand, has a very complex anatomy with a large number of bones, muscles, tendons and nerves. They are essential to work and sport activities and are subject to acute and/or chronic mechanical injuries. This is why overuse injuries are the most common in these regions. They are often characterised by tendinitis, tenosynovitis, tunnel syndromes, or stress fractures. This paper gives an overview of the most common overuse syndromes of the hand, wrist, forearm and elbow, describing their characteristic symptoms and methods of diagnosis and treatment. The syndromes are trigger finger, dorsal radiocarpal impingement syndrome (gymnast's wrist), DeQuervain's disease, tenosynovitis of other dorsal compartments, intersection syndrome (oarsman's wrist), flexor carpi ulnaris tendinitis, flexor carpi radialis tendonitis, humeral epicondylitis (tennis elbow), posterior impingement syndrome of the elbow, medial tension and lateral compression syndrome, stress fracture in the hand, wrist, forearm and elbow, and tunnel syndromes.

Computed tomographic imaging to determine the frequency of anatomical variations in pneumatization of the ethmoid bone.

The present study was performed on axial and coronal CT scans of 212 patients. Scans were analyzed by an anatomist and a radiologist for the presence of Haller's cells, agger nasi cells, Onodi's cells and pneumatized crista galli. Results demonstrated the presence of Haller's cells in 21.2%, Onodi's cells in 10.4% and pneumatized crista galli in 2.4% of patients. A pneumatized anterior clinoid process was found in 0.5% of the patients. The data obtained in our study were compared with those reported in other anatomic and radiologic studies.

Regeneration of articular cartilage defects in rabbits by osteogenic protein-1 (bone morphogenetic protein-7).

Osteogenic protein-1 (OP-1, BMP-7), a member of the transforming growth factor-beta family, induces cartilage and bone formation when implanted at intra and extraskeletal sites in vivo. The human OP-1 gene has been cloned and biologically active recombinant OP-1 homodimers have been produced. In the present study, the authors investigated the influence of OP-1 on healing of full-thickness articular cartilage defects, made by drilling two adjacent (phi 3mm) holes through articular cartilage of NZW rabbit knee joint were dissected and examined histomorphometrically. Results indicated that OP-1 induced articular cartilage healing and regeneration of the joint surface which contained cells resembling mature joint chondrocytes. These data imply a new strategy for biological repair of damaged joint surfaces in humans.

[A full-term second abdominal pregnancy].

A case is described of abdominal pregnancy in a 27-year-old patient with a previous salpingectomy of the right tube performed because of tubal pregnancy. The patient was hospitalized in the 22nd to the 28th week of pregnancy on suspicion of imminent abortion. Clinical and ultrasound examinations indicated intrauterine pregnancy, by just before the expected date of delivery, clinical symptoms suggested abdominal pregnancy. The oxytocin test was negative. Following laparotomy a live normal girl was born, weighting 3200 g. The placenta was inserted at the front wall and the right horn of the uterus from the outer side. In the right horn there was an opening at the site of the oviduct excision. Because of profuse bleeding, the uterus was amputated supravaginally.

[Pneumatization of the middle nasal turbinate: a CT study].

Anatomic variations of the middle turbinate may be an important factor in the pathogenesis of inflammatory processes of the paranasal sinuses. A detailed analysis of coronal and axial plane CT scans of the paranasal sinuses obtained from 120 patients was performed. Paradoxical curve of the middle concha was detected in 24.2% of patients. Pneumatized middle turbinate was found in 42.5% and "true" concha bullosa in 8.3% of patients. Bilateral concha bullosa was found in 1.7% of patients.
