Long-term alteration in the central monoaminergic systems of the rat by 2,4,5-trihydroxyamphetamine but not by 2-hydroxy-4,5-methylenedioxymethamphetamine or 2-hydroxy-4,5-methylenedioxyamphetamine.

The long-term effects of three metabolites of 3,4-methylenedioxymethamphetamine (MDMA) on the central monoaminergic systems of the rat were examined. Seven days after the intracerebroventricular administration of 0.25 and 0.5 mumol 2,4,5-trihydroxyamphetamine, hippocampal tryptophan hydroxylase (TPH) activity was reduced to 5 and 1% of control, respectively, while norepinephrine (NE) concentration was depressed to 10 and 18% of control. These two respective dosages also decreased striatal tyrosine hydroxylase (TH) activity to 67 and 10% of control, respectively, while nigral TH activity was reduced to 59 and 20% of control. Striatal TPH activity was reduced to 74 and 81% of control, respectively, while the activity in the dorsal and median raphe remained unaltered. The intracerebroventricular administration of 1 mumol 2-hydroxy-4,5-methylenedioxymethamphetamine (6-OH-MDMA) failed to alter TPH activity, TH activity or NE concentration after 14 days. In contrast, 1 mumol of 2-hydroxy-4,5-methylenedioxyamphetamine (6-OH-MDA) induced a 30% increase in striatal TPH activity and a 50% increase in nigral TH activity. The study of the formation of 2,4,5-trihydroxyamphetamine after MDMA treatment may provide insight as to how MDMA destroys serotonergic nerve terminals.

3,4-Methylenedioxyamphetamine

Protection from environmental tobacco smoke in California. The case for a smoke-free workplace.

OBJECTIVE: To determine the extent of exposure of nonsmoking indoor workers to environmental tobacco smoke (ETS) according to type of work-site smoking policy, work area, workplace size, and demographic characteristics. DESIGN AND PARTICIPANTS: Participants included 7162 adult, nonsmoking, indoor workers who were interviewed as part of the 1990 California Tobacco Survey. Respondents were asked whether anyone had smoked in their work area within the past 2 weeks. RESULTS: An estimated 2.2 million California nonsmokers were exposed to tobacco smoke at indoor work sites in 1990. Nonsmoker exposure to ETS was 9.3% for those working in a smoke-free worksite, 23.2% for those working where there was only a work-area restriction, 46.7% for those working where there was a policy that did not include the work area, and 51.4% for those working where there was no work-site smoking policy. After adjustment for type of work area (eg, office, open area), workplace size, and demographic factors, it was determined that nonsmokers working where there was only a work-area ban were 2.8 times more likely to be exposed to ETS than those working in a smoke-free work site. In workplaces with no policy or a policy not covering the work area, nonsmokers were over eight times more likely to be exposed to ETS than those who worked in a smoke-free work site. Nonsmokers who were 18 to 24 years of age, male, or Hispanic, and those with less than a high school education had more exposure to ETS. CONCLUSION: These results indicate that adequate protection of nonsmokers from ETS exposure requires a smoke-free work site.

Response of monoaminergic and neuropeptide systems to 4-methylaminorex: a new stimulant of abuse.

4-Methylaminorex is an amphetamine analog which has recently gained attention due to its potential as a stimulant of abuse and the ease with which it is synthesized. Administration of acute and multiple doses of 4-methylaminorex caused rapid (3-h) and long-term (7-day) declines in striatal tryptophan hydroxylase activity with few changes in other serotonergic parameters. The acute response by tryptophan hydroxylase to this drug was reversed by incubating the tissues in a reducing environment suggesting that 4-methylaminorex alters this enzyme through oxidative mechanisms. The 4-methylaminorex-induced long-term reduction in tryptophan hydroxylase activity might be due to neurotoxic action on serotonergic systems. In contrast, although a decline in striatal tyrosine hydroxylase occurred 3 h following a single dose of 4-methylaminorex, no changes in this enzyme were observed at 7 days after acute or multiple dosing with this drug. This result suggests that 4-methylaminorex is not neurotoxic to the dopaminergic neurons. Even though this amphetamine analog apparently does not have long-term effects on dopaminergic systems, it does appear to enhance substantially dopaminergic activity. Evidence for increased dopamine activity resulting from 4-methylaminorex administration included dramatic but temporary rises in the levels of nigral neurotensin, dynorphin A and substance P following multiple drug administration. Similar peptide changes have been observed with other amphetamine-related stimulants and are mediated by increases in dopaminergic activity. In summary, 4-methylaminorex has significant impact on monoaminergic pathways. In general, its spectrum of effects on these systems is like that of the ring-substituted amphetamines, such as methylenedioxymethamphetamine.

Problem drug use in a central London general practice.

OBJECTIVE: To describe the profile of problem drug users presenting in general practice and to determine whether they can be cared for in general practice. DESIGN: Study of consultations by problem drug users. SETTING: Central London general practice. SUBJECTS: 150 problem drug users presenting over two years. MAIN OUTCOME MEASURES: Stopping drug use, alterations in lifestyle, obtaining paid work, and stopping drug related crime. RESULTS: Of the 150 patients, 111 were men and 39 women, and 106 were unemployed. 121 patients used heroin, 112 of whom injected. 145 patients accepted a methadone reduction programme and 81 completed it. A further 25 were stabilised on reducing doses of methadone, until places became available for them as inpatients at drug dependency units or rehabilitation projects. CONCLUSION: Withdrawal programmes for patients who misuse drugs can be successfully managed in general practice.

Fluorescence lifetime imaging.

We describe a new fluorescence imaging methodology in which the image contrast is derived from the fluorescence lifetime at each point in a two-dimensional image and not the local concentration and/or intensity of the fluorophore. In the present apparatus, lifetime images are created from a series of images obtained with a gain-modulated image intensifier. The frequency of gain modulation is at the light-modulation frequency (or a harmonic thereof), resulting in homodyne phase-sensitive images. These stationary phase-sensitive images are collected using a slow-scan CCD camera. A series of such images, obtained with various phase shifts of the gain-modulation signal, is used to determine the phase angle and/or modulation of the emission at each pixel, which is in essence the phase or modulation lifetime image. An advantage of this method is that pixel-to-pixel scanning is not required to obtain the images, as the information from all pixels is obtained at the same time. The method has been experimentally verified by creating lifetime images of standard fluorophores with known lifetimes, ranging from 1 to 10 ns. As an example of biochemical imaging we created life-time images of Yt-base when quenched by acrylamide, as a model for a fluorophore in distinct environments that affect its decay time. Additionally, we describe a faster imaging procedure that allows images in which a specific decay time is suppressed to be calculated, allowing rapid visualization of unique features and/or regions with distinct decay times. The concepts and methodologies of fluorescence lifetime imaging (FLIM) have numerous potential applications in the biosciences. Fluorescence lifetimes are known to be sensitive to numerous chemical and physical factors such as pH, oxygen, temperature, cations, polarity, and binding to macromolecules. Hence the FLIM method allows chemical or physical imaging of macroscopic and microscopic samples.

The quantitative autoradiographic distribution of [3H]MK-801 binding sites in the normal human brainstem in relation to motor neuron disease.

The distribution of N-methyl-D-aspartate (NMDA) receptors in the normal human brainstem has been investigated using MK-801. Specific [3H]MK-801 binding showed a heterogeneous distribution, the greatest density of binding sites being found in the substantia nigra, locus coeruleus, and the hypoglossal and inferior olivary nuclei. Brainstem motor nuclei subserving eye movements, which tend to be spared in motor neuron disease (MND), had significantly lower densities of binding compared to other cranial nerve motor nuclei (V, VII, X, XII) which tend to be affected. The anatomical distribution of NMDA receptors may be one factor determining selective vulnerability to excitotoxic injury.

Juxtacanalicular tissue in pigmentary and primary open angle glaucoma. The hydrodynamic role of pigment and other constituents.

We tested the hypothesis that obstruction of the juxtacanalicular tissues, by melanin granules in pigmentary glaucoma and by other impermeable material in primary open angle glaucoma, leads to the development of a chronic glaucomatous condition. The distribution and concentration of melanin and other impermeable materials in the juxtacanalicular tissues and elsewhere in the trabecular meshwork was determined in 13 specimens. Six specimens were from patients with pigmentary glaucoma, two from patients with pigment dispersion syndrome, and three from patients with primary open angle glaucoma, as well as two from normal subjects. The effect of these materials on flow resistance was estimated using two hydrodynamic models. In model A, the electron-lucent spaces of the juxtacanalicular tissue were assumed to be open spaces, while in model B, these spaces and spaces filled with ground substance were assumed to be gel filled. In pigmentary glaucoma, 3.5% of the pigment was found in the juxtacanalicular tissue, while 96.5% was found in the corneoscleral and uveoscleral tissues. Permeabilities calculated according to model A were much higher than those expected from estimates of outflow facility in all groups, in agreement with the previous report of Ethier et al. The gel-filled spaces available for fluid flow, as determined by model B, showed no statistically demonstrable differences (pigmentary glaucoma, 32.9%; primary open angle glaucoma, 36.6%; pigment dispersion syndrome, 43.4%; normal, 44.1%). Furthermore, the amount of pigment present in the juxtacanalicular tissue was determined to have a negligible influence on permeability. Thus, the development of the chronic glaucomatous condition cannot be directly attributed to pigment accumulation in the juxtacanalicular tissue in pigmentary glaucoma.

Recognizing staff nurse job performance and achievements.

While recognition for work has been positively related to nurses' job satisfaction in the research literature, specific types of recognition that would be most meaningful have not been identified. In this descriptive survey, 341 randomly selected staff nurses rated the extent of meaningfulness of 38 head nurse recognition behaviors. The most meaningful, in order of importance, were monetary rewards commensurate with performance, private verbal feedback, and written acknowledgment. Three other categories of recognition were identified as moderately meaningful: public acknowledgement, schedule adjustment, and opportunities for growth and development.

Age-related changes of sulfated proteoglycans in the normal human trabecular meshwork.

Ultrastructural changes in sulfated proteoglycans were studied in 12 pairs of normal eye-bank eyes (aged 1 day to 92 years), using the cationic dye, cuprolinic blue, in a 'critical electrolyte concentration'. Pretreatment of trabecular meshwork sections with various glycosaminoglycanases and nitrous acid served to characterize these proteoglycans. Three sizes of proteoglycan-cuprolinic blue (PG-CB) complexes were found in association with different extracellular matrix components. Small, thin PG-CB complexes were closely associated with collagen fibrils. Large, thick PG-CB complexes, although located close to collagen fibrils in a variety of places, were most commonly seen between the boundaries of the collagen bundles, where they were associated with fine filaments. Both types of collagen-associated PG-CB complexes contained chondroitin sulfate and dermatan sulfate, with dermatan sulfate predominant. Basal lamina-associated PG-CB complexes contained heparan sulfate. An age-related, progressive coalescence of collagen was found in normal trabecular meshwork in a statistically significant fashion; the regions of collagen coalescence were associated with a decrease of small, collagen-associated PG-CB complexes and an increase of a previously unrecognized matrix material. The measurement of areas of coalescence of collagen was used as an indirect indicator of small, collagen-associated PG-CB complex loss with age. Large collagen-associated PG-CB complexes and basal lamina-associated PG-CB complexes decreased from infant to young adult; no additional loss with age was found. Further studies will be needed to determine whether loss of sulfated PGs plays a role in increased aqueous outflow resistance that characterizes glaucoma.

A silent conspiracy?: some ethical issues of participant observation in nursing research.

In this paper I suggest that too little attention has been paid to ethical problems underlying health care research, particularly that which uses covert participant observation. Some of the chief arguments surrounding the use of deception in data collection in health care settings are evaluated, and a study by Field (1989) [Nursing the Dying. Routledge, Tavistock] in which covert observation was used is examined in relation to two moral principles--human rights and general utility. Conclusions include the responsibility of researchers to explicitly justify their approach in terms rather more than mere technical advantage, and the need to encourage the teaching and use of non-deceptive methods of data collection at this most crucial developmental stage in the evolution of nursing and health care research.

A technique for fabrication of interim midfacial prostheses.

The interim midfacial prosthesis uses a rigid backing support with flexible margins and can be made with most types of commercially available medical grade silicone elastomers. The technique may be modified for fabrication of definitive midfacial prostheses. The disadvantage of the technique is that an extra step is required to make a duplicate master cast, and a vacuum adaptor is needed.

Public objections to environmental tobacco smoke.

METHODS. A randomly selected sample of Californian adults was surveyed by telephone to assess their knowledge, attitudes, and behaviors with respect to tobacco use and its control. The questions asked of this sample were whether they had recently (in the past 12 months) asked someone not to smoke in certain situations and whether they would ever consider doing such a thing. RESULTS. Results of the survey indicated that 59.1% of nonsmokers and 44.4% of smokers had asserted themselves within the past 12 months, while another 28.2 and 29.1% of the nonsmokers and smokers, respectively, indicated that they would consider asking someone not to smoke. Only 12.7% of the nonsmokers and 26.4% of the smokers indicated that they would not consider doing such a thing. Those more likely to object publicly to tobacco smoke are generally younger, are more educated, and have anti-tobacco attitudes related to either a specific health belief or social influence.

Event-related potentials in the dual task paradigm: P300 discriminates engaging and non-engaging films when film-viewing is the primary task.

Five groups of subjects (11-16/group) were run in three experiments. In each study P300 amplitude and latency were studied as a function of three sensory task conditions: (1) baseline (oddball task: target and non-target tones only); (2) boring film viewing plus oddball task; and (3) exciting film viewing plus oddball task. In Experiment I, target probability was P = 0.22. In Experiments II and III, each of two groups of subjects was run at P = 0.22 or P = 0.33. Two different exciting films and two different boring films were used. Oddball-target amplitude was found to consistently decrease from baseline to boring to exciting film-viewing conditions at P = 0.22. At P = 0.33, the 2-film discrimination was successful in one of two experiments. P300 amplitude consistently differed from baseline to film-viewing conditions. For the non-target (frequent) tones, the smaller P300 amplitudes consistently discriminated boring and exciting films, as well as single from dual tasks. For target P300 latency, single (baseline) and dual (film-watching) conditions were always discriminable, but boring and exciting films were not discriminable. There were no effects on non-target P300 latency. N100 amplitude discriminated baseline and film viewing conditions, but not boring and engaging films.

Psychological and social indicators of suicide ideation and suicide attempts in Zuni adolescents.

Suicide behavior is a significant problem for many American Indian populations, often more so than in the general population, but little tribal-specific data available. In this study, baseline data on the correlates of suicide ideation and the social and psychological differences between suicide attempters and nonattempters were collected on a sample of 84 Zuni adolescents. Results show significant correlations between a measure of suicide ideation and past suicide attempt behavior, drug use, depression, hopelessness, stress, psychological symptomatology, social support, liking for school, and interpersonal communication. Significant differences between the 30% of the students who reported having previously attempted suicide and the nonattempters were also found on these measures. Areas for education and prevention efforts are suggested.
