Ehlers-Danlos Syndrome (EDS) is a group of related genetic disorders of connective tissue presenting with joint hypermobility, skin extensibility, and tissue fragility. Although the pathophysiology of EDS is increasingly understood, the psychosocial effects of having EDS have not been examined. We psychologically tested and interviewed 41 adults and 7 children with EDS. Anxiety, depression, anger, and interpersonal concerns were significantly elevated, varying from one-quarter to one-third of patients; over 70% had a history of some mental health care use. Psychological difficulties appear to result from chronic pain and disability, ostracism or avoidance of relationships and social activities, sexual difficulties and reproductive concerns, and frustration with the medical system. Specific types of EDS (e.g., EDS Type I) are associated with greater pain and psychological distress. Psychological intervention, prescribed with the recognition that psychiatric features are secondary to EDS, is recommended for some patients.

Lungs from pigs at slaughter were examined and the extent of the lesions was estimated as the percentage by weight of the lung affected by pneumonia in relation to the weight of the whole lung. Linear regression analyses of mean daily gain in liveweight and days to slaughter on the percentage by weight of pneumonic lung were performed. Within a range between 3.32 and 74.55 per cent for the weight of pneumonic lung, a 10 per cent increase in the weight of pneumonic lung was associated with a decrease in mean daily gain of 31.4 g and a 13.2-day increase to slaughter at 104.2 kg liveweight. When subgroups of pigs were evaluated by farm of origin, significant associations between indicators of performance and pneumonia for pigs from only one of the farms were found.
Interleukin 1 (IL-1) gene expression was investigated in mice following oral infection with Yersinia enterocolitica 08. In Peyer's patches (PP), the primary site of bacterial invasion, induction of IL-1 alpha mRNA was delayed when compared to IL-1 beta mRNA. As shown by in situ hybridization. IL-1 alpha and IL-1 beta mRNA were found to be expressed within different cell types. These results indicate that expression of the two forms of IL-1 is regulated in a cell-specific manner at the transcriptional level. Moreover, IL-1 (alpha and beta) mRNA was increased in other organs such as spleen and lung. In spleens, IL-1 beta mRNA was found within the red pulp, and IL-1 alpha mRNA was located to the marginal zone confirming that differential expression of IL-1 alpha and IL-1 beta mRNA does not represent a tissue-specific event. However, as revealed by immunohistochemistry and measuring IL-1 activity in tissue homogenates, synthesis of IL-1 proteins was not detectable in spleens, unless mice were challenged with LPS. Because IL-1 synthesis was inducible in spleen cells following actinomycin D treatment, the results indicate that at distant sites of infection IL-1 (alpha and beta) mRNA is expressed but not translated into protein. It is concluded that cell-specific transcription of IL-1 alpha and IL-1 beta as well as dissociation between IL-1 mRNA and protein synthesis are two mechanisms effective in regulating the production of IL-1 during infection.
BACKGROUND: Outcome after renal transplantation in children has been variable. We undertook a retrospective study of our experience over the past five years. STUDY DESIGN: From January 1, 1988, to October 15, 1992, 60 renal transplantations were performed upon 59 children at the Children's Hospital of Pittsburgh. Twenty-eight (47 percent) of the kidneys were from cadaveric donors, and 32 (53 percent) were from living donors. The recipients ranged in age from 0.8 to 17.4 years, with a mean of 9.8 +/- 4.8 years. Forty-six (77 percent) recipients were undergoing a first transplant, while 14 (23 percent) received a second or third transplant. Eight (13 percent) of the patients were sensitized, with a panel reactive antibody of more than 40 percent. Eleven of the 14 patients undergoing retransplantation and seven of the eight patients who were sensitized received kidneys from cadaveric donors. Thirty-three (55 percent) patients received cyclosporine-based immunosuppression, and 27 (45 percent) received FK506 as the primary immunosuppressive agent. RESULTS: The median follow-up period was 36 months, with a range of six to 63 months. The one- and four-year actuarial patient survival rate was 100 and 98 percent. The one- and four-year actuarial graft survival rate was 98 and 83 percent. For living donor recipients, the one- and four-year actuarial patient survival rate was 100 and 100 percent; for cadaveric recipients, it was 100 and 96 percent. Corresponding one- and four-year actuarial graft survival rates were 100 and 95 percent for the living donor recipients and 96 and 69 percent for the cadaveric recipients. Patients on cyclosporine had a one- and four-year patient survival rate of 100 and 97 percent, and patients on FK506 had a one- and three-year patient survival rate of 100 and 100 percent. Corresponding one- and four-year actuarial graft survival rates were 100 and 85 percent in the cyclosporine group, while one- and three-year actuarial graft survival rates were 96 and 84 percent in the FK506 group. The mean serum creatinine level was 1.24 +/- 0.64 mg per dL; the blood urea nitrogen level was 26 +/- 13 mg per dL. The incidence of rejection was 47 percent; 75 percent of the rejections were steroid-responsive. The incidence of cytomegalovirus was 10 percent. The incidence of post-transplant lymphoproliferative disorder was 8 percent. None of the patients on cyclosporine were able to be taken off prednisone; 56 percent of the patients receiving FK506 were taken off prednisone successfully. Early growth and development data suggest that the patients receiving FK506 off prednisone had significant gains in growth. CONCLUSIONS: These results support the idea that renal transplantation is a successful therapy for end-stage renal disease in children. They also illustrate the potential benefits of a new immunosuppressive agent, FK506.
Previous clinical evaluation of FK506 in renal transplantation has demonstrated equivalent patient and graft survival when compared with cyclosporine-based regimens. However, lower steroid and anti-hypertensive mediation requirements and lower serum cholesterol levels have been seen in patients receiving FK506. In August, 1991, a prospective, randomized trial was begun, comparing FK506/prednisone with FK506/azathioprine/prednisone. Two-hundred-and-four adults were entered into this trial between August 1, 1991, and October 11, 1992. The mean recipient age was 43.8 +/- 13.7 years, with a range of 17.6-78.0 years. Sixty-one (30%) recipients received a 2nd, 3rd or 4th transplant, while 35 (17%) had a PRA greater than 40% at the time of transplant. Thirty-three (16%) of the transplants were in recipients over 60 years of age, Thirteen percent of the kidneys were from living donors; 13% of the cadaveric kidneys were from pediatric donors less than 3 years of age and were transplanted en bloc. The mean cold ischemia time was 31.4 +/- 8.4 hours, and the mean donor age was 34 +/- 2.10 years, with a range from 4 months to 75 years. With a mean follow-up of 9 +/- 4 months, the 1-year actuarial patient survival is 93%; for the two-drug group it is 95%, and for the three-drug group it is 91% (p = NS). One-year actuarial graft survival is 86%; in the two-drug group it is 90%, while in the three-drug group it is 82% (p = NS).(ABSTRACT TRUNCATED AT 250 WORDS)

In an unselected adult renal transplant population, FK506 as the primary immunosuppressive agent yielded one- and 2-year actuarial patient survival rates of 95% and 93% and one- and 2-year actuarial graft survival rates of 89% and 83%, respectively. Forty-nine percent of successfully transplanted patients were weaned off steroids. 2. In pediatric renal transplant patients, FK506 has been associated with 100% one- and 3-year actuarial patient survival rates and 98% and 85% one- and 3-year actuarial graft survival rates, respectively. Sixty-two percent of successfully transplanted patients were taken off prednisone, with dramatic improvements in height. 3. FK506 has been used successfully in rescuing 70-74% of adult or pediatric renal transplant patients with an acute rejection that failed conventional therapy. 4. Kidney/bone marrow transplantation under FK506 therapy has been successfully performed without graft-versus-host disease and with routine augmentation of chimerism. 5. The side effects of FK506 included nephrotoxicity, neurotoxicity, and diabetogenicity; they were comparable to those seen with CsA. 6. FK506 is an important new addition to the immunosuppressive armamentarium in renal transplant patients.
Between 1980 and 1990 there were 12,216 bombing incidents in the United States, the majority involving pipe-bomb type devices. Victims of such devices may suffer a combination of blast, penetrating, and thermal injuries requiring special surgical approaches. A series of cases is presented which illustrates the management dilemmas faced in such situations. Given the nationwide incidence of bombings, both local emergency medical systems and trauma surgeons should be prepared to deal with blast injuries, be aware of their regional incidence of bombings, and have a planned response that is coordinated with law enforcement agencies.
A three-dimensional computer modelling system has been developed for use in biology, and is currently running on a Sun3 computer. The data originate as a series of two-dimensional micrographs which are digitised via a TV camera. The two-dimensional images are used to select features of interest and to construct a three-dimensional model. This model can be viewed in vector or solid format, it can be rotated about three orthogonal axes and can be viewed in three dimensions as a stereo pair or an anaglyph. The system has been used in a large number of projects over the past 10-15 years, for example, to examine physiological and nerve structures. The time-consuming part of the process is the selection of features, which involves a high level of biological expertise. Present developments are concerned with reduction of the time spent in feature recognition and involve the introduction of expert systems together with human-computer interaction to deal with problems of identification.
The majority of women with dysfunctional uterine bleeding ovulate and have normal cyclical changes in gonadotrophins, oestrogen and progesterone. To investigate whether the hormonal milieu at tissue level is different in these women, we measured the endometrial concentration of oestrogen and progesterone receptors at various stages of the menstrual cycle in women with normal menstrual loss (< or = 80 ml/cycle, n = 40) and dysfunctional uterine bleeding (> 80 ml/cycle, n = 44). Menstrual blood loss was measured using the alkaline haematin method. Receptor levels were measured in nuclear and cytosol extracts of endometrium using solid phase immunoassays, based on monoclonal antibodies against receptor protein, which measure the bound and unbound fractions of the receptors. We found endometrial oestrogen (P < 0.01) and progesterone (P < 0.05) receptor levels were higher in the late secretory phase in women with dysfunctional uterine bleeding compared with women with normal menstrual loss. The receptor levels were the same in both groups at all other stages of the menstrual cycle. There was a strong positive correlation between the level of late secretory endometrial oestrogen receptor and measured menstrual blood loss (r = 0.81, P < 0.01). Increased local oestrogen effect is present in the premenstrual endometrium in dysfunctional uterine bleeding.
