A semantic-based kernel for advanced health information systems.

This paper reports on the design and development of an infrastructure allowing one to share and exchange multimedia data in the context of a health network. A single technology exploiting a semantic model of the hospital universe provides users with information and data of diverse origins, generated by the various actors or departments of the health organization. Functions provided include act management and patient record management governed by domain semantics. The functionality has been validated through laboratory experiments against the requirements of protocol directed care and health networks. The functionality is integrated into a clinician workstation exploited in the Internet/Intranet environment thanks to a commercial browser. These results have been obtained with the support of several projects in the frame of the Health-Care Telematics Applications Programme of the European Community and of the Eurêka Programme.

UMLS-based conceptual queries to biomedical information databases: an overview of the project ARIANE. Unified Medical Language System.

OBJECTIVE: The aim of the project ARIANE is to model and implement seamless, natural, and easy-to-use interfaces with various kinds of heterogeneous biomedical information databases. DESIGN: A conceptual model of some of the Unified Medical Language System (UMLS) knowledge sources has been developed to help end users to query information databases. A query is represented by a conceptual graph that translates the deep structure of an end-user's interest in a topic. A computational model exploits this conceptual model to build a query interactively represented as query graph. A query graph is then matched to the data graph built with data issued from each record of a database by means of a pattern-matching (projection) rule that applies to conceptual graphs. RESULTS: Prototypes have been implemented to test the feasibility of the model with different kinds of information databases. Three cases are studied: 1) information in records is structured according to the UMLS knowledge sources; 2) information is able to be structured without error in the frame of the UMLS knowledge; 3) information cannot be structured. In each case the pattern-matching is processed by the projection rule according to the structure of information that has been implemented in the databases. CONCLUSION: The conceptual graphs theory provides with a homogeneous and powerful formalism able to represent both concepts, instances of concepts in medical contexts, and associations by means of relationships, and to represent data at different levels of details. The conceptual-graphs formalism allows powerful capabilities to operate a semantic integration of information databases using the UMLS knowledge sources.

The RICHE Reference Architecture.

The RICHE Reference Architecture defines an open framework for health care information systems. Developed within the ESPRIT II programme, it is now the basis of several implementations in hospitals in Italy, Portugal, Ireland, France and the UK, and of follow-up initiatives such as EDITH (ESPRIT, no. 7508), NUCLEUS (AIM II, no. 2025) and REFERENCE (a French hospitals' project). This contribution describes the RICHE Reference Architecture in detail. Special concern is centred on the modulus that constitute the basic platform of RICHE, and on the methodology applied to design the interactions between them.

[Complications of diverticular disease of the colon. Apropos of 247 cases].

In the last five years, 7 teams, members of a surgical club of lower Normandy, have performed 1,136 colon procedures, including 247 for severe diverticular disease. The patients were grouped in two categories according to the severity of the complications presented: 159 sub-acute complications including 137 acute cases of sigmoiditis (diverticulitis, peri-colic micro abscesses, pseudo-tumoral forms) and 22 fistulae, 88 severe acute complications including 60 generalized peritonites, 12 huge abscesses, 9 occlusions and 7 hemorrhages. The total post-operative mortality is 6 p. cent. It is quite different in the surgery of sub-acute complications (0.6%) and the surgery of acute severe complications (16%). The total morbidity reaches 14.8 p. cent. Complications are more frequent and more severe in acute forms (18.5%) than in subacute forms (10.5%). At long term, 10.8 p. cent of the patients operated upon, kept a permanent colostomy (25 p. cent in acute forms vs 2.3 p. cent in subacute forms). 89.2 p. cent have a normal digestive tract following treatment (97.7 p. cent in subacute forms and 53 p. cent in acute forms).

[Cadmium, zinc, pseudo-cholinesterase and aldolase blood levels in hypertensive and alcoholic patients. Preliminary statistical computerised study on 124 cases (author's transl)].

Changes in blood-level of cadmium, zinc, pseudocholinesterase and aldolase were studied in 44 normal subjects, 40 hypertensive patients and 40 alcoholic ones. The highest serum concentration of cadmium and pseudocholinesterase, the lowest serum and blood concentrations of zinc, the lowest serum concentrations of aldolase were found in the two groups of patients, with still lower levels in alcoholic patients than in hypertensive ones. There was a negative correlation between zinc and cadmium levels in the three groups of subjects. There was a high correlation between systolic and diastolic blood pressure value and cadmium, and negative correlation between blood pressure and zinc blood and serum levels.

[Thrombocytosis and anaemia (author's transl)].

The authors found a significant statistical association between thrombocytosis and anaemia in 600 patients admitted to Hotel-Dieu of Marseille: p < 10(-3) with Chi 2 test and p < 5.10(-4) with Mann-Whitney test. There is also a significant correlation (linear and non-parametric Spearmann): r = -0,212; p < 10(-3); and a partial correlation between platelets count and weight of haemoglobin, which is independant of sedimentation rate.
