Dyskeratosis congenita: delay in diagnosis and successful treatment of pancytopenia by bone marrow transplantation.

Dyskeratosis congenita is an inherited disorder characterized by nail dystrophy, skin pigmentary changes, mucosal leukoplakia, pancytopenia and an increased incidence of malignancy. Because of a widely held view that the outcome of bone marrow transplantation in dyskeratosis congenita is poor, this treatment option is sometimes not considered when pancytopenia develops. We present a child currently doing well 3 years after bone marrow transplantation, and review the literature.

Thrombotic cutaneous nodules and hepatic vein thrombosis in the anticardiolipin syndrome.

A patient with the anticardiolipin syndrome presented with hepatic vein thrombosis (Budd-Chiari syndrome) and non-ulcerating, thrombotic, cutaneous nodules. This cutaneous manifestation of the anticardiolipin syndrome has not been described before, though its histological appearances resemble those of previously reported cutaneous complications, such as chronic ulceration, livedo reticularis and distal cutaneous ischaemia. Recognition of the skin lesion could allow the prevention of major systemic complications.

Lysosomal enzyme abnormalities in preleukaemic Sweet's disease: case report.

Quantitative and qualitative abnormalities in marrow lysosomal enzymes, suggestive of acute myeloid leukaemia, were detected in a patient with Sweet's disease and monocytosis 12 months before she presented with acute myelomonocytic leukaemia. Biochemical characterisation of blood and marrow cell extracts may help to identify those patients with Sweet's disease and other preleukaemic conditions who are most at risk of developing leukaemia.

The efficacy and toxicity of VAD in the treatment of myeloma and related disorders.

Impressive results have recently been reported in 29 cases of advanced refractory myeloma treated with 4-d infusions of Vincristine and Adriamycin (VAD). We report our own experience with this protocol in 13 cases of myeloma and related lymphoproliferative disorders. In 2 of these cases, VAD was used as first line treatment. Objective responses were seen in two-thirds of cases and the major complication of therapy was infection, predominantly bacterial in nature.

Sweet's syndrome and non-Hodgkin's lymphoma: the first report of this association.

A 58-year-old woman developed Sweet's syndrome one week after a flu-like illness. She was later found to have a centrocytic/centroblastic non-Hodgkins lymphoma. Six courses of chemotherapy were given during which the lesions of Sweet's syndrome resolved completely. As far as we are aware this is the first report of the association of Sweet's syndrome with a lymphoma.
